This study aimed to develop an objective language-based rating framework for differentiating individuals at high risk of cognitive impairment from normal elderly adults using discourse indicators derived from a story retelling task, and to empirically verify its statistical validity and clinical discriminability. Participants consisted of 10 normal elderly adults and 10 individuals at high risk of cognitive impairment selected through cognitive screening assessments. The 'Magic Millstone' story retelling task, developed by Lee (2026) for Korean older adults, was administered, and the produced speech data were rated using quantitative indicators (total number of Information Units [IUs] and target IU achievement rate) based on a 120-item target IU checklist developed by Lee (2026), as well as three qualitative indicators (main idea, narrative structure, and temporal sequencing). Independent samples t-tests, ROC analyses, and Pearson and Spearman correlation analyses were conducted sequentially.
First, independent samples t-tests revealed that the high-risk group showed significantly lower performance than the normal elderly group across all quantitative and qualitative discourse indicators (all indicators p〈.001), with Cohen's d ≥ 2.23 confirmed for all indicators. Second, ROC analyses indicated that the main idea indicator achieved an AUC of 1.000, demonstrating the highest level of discriminability, while all other indicators—including total IU count (.990), target IU achievement rate (.990), temporal sequencing (.980), and narrative structure (.910)—showed AUC values of .90 or above, confirming excellent discriminant validity (all indicators p〈.001). Notably, all indicators achieved a sensitivity of 1.000, indicating that the rating framework demonstrates high sensitivity in screening for individuals at high risk of cognitive impairment. Third, correlation analyses revealed that all discourse indicators showed significant positive correlations with cognitive function level (MoCA-K; all indicators p≤.002), with main idea showing the highest correlation (ρ=.823).
This study empirically confirmed that language-based discourse indicators derived from the story retelling task can function as a valid and reliable clinical tool for early screening of individuals at high risk of cognitive impairment. In particular, the finding that the main idea indicator was confirmed as the most powerful discriminant indicator across both quantitative and qualitative dimensions demonstrates that macrostructural discourse production ability sensitively reflects cognitive decline. The main idea indicator, which demonstrated an AUC of 1.000 as a single indicator, suggests clinical applicability as a simple and non-invasive cognitive screening tool, and verification through future large-scale standardization studies is required. The findings of this study can serve as foundational data for the development of language-based cognitive screening tools and the establishment of clinical guidelines.
