This study aims to examine, from the perspective of those directly affected, how the coverage of social care breaks down throughout the entire process by which older adults with visual impairments apply for, are assessed for, and use social care services, and to provide a policy basis for strengthening the coverage of Korea's adult care system. Older adults with visual impairments have complex care needs under the dual conditions of aging and disability. However, the current social care system is divided into the Activity Support Service for Persons with Disabilities and the Long-Term Care Insurance for older adults, with eligibility and benefit content shifting at the threshold of age 65. As a result, these systems fail to adequately encompass the care needs of older adults with visual impairments, who simultaneously possess both characteristics.
This study adopted directed content analysis, a qualitative research method, as its analytic approach. For its analytic framework, it drew on the two dimensions of social service blind spots presented by Noh Hye-jin (2016)—namely, the non-inclusiveness of the target population and the insufficiency of benefits—and analyzed these by intersecting them with the application, assessment, and utilization stages of care services. In addition, an intersectional perspective was drawn upon in interpreting the findings, in order to make sense of the ways in which multiple social positions—such as visual impairment, old age, and regionality—combine to deepen these blind spots. The study participants comprised a total of nine individuals: five older adults with visual impairments residing in Pohang and Gyeongju, North Gyeongsang Province, who were either currently using or had prior experience applying for care services; two social welfare public officials; and two social workers at a welfare center for persons with disabilities. Data were collected through semi-structured in-depth interviews conducted from March to April 2026.
The analysis revealed that the blind spots experienced by older adults with visual impairments were not confined to any single stage but accumulated continuously across the entire process of application, assessment, and utilization. The non-inclusiveness of the target population was termed "a system that fails to see the visually impaired." At the application stage, information was conveyed primarily through visual media, so that the very existence of the systems failed to reach those concerned, and the application-based delivery system structurally excluded those with limited access to information. At the assessment stage, the long-term care evaluation system, centered on declines in physical and cognitive function, did not adequately reflect the care needs arising from visual impairment; moreover, the division of the two systems at the threshold of age 65 narrowed the available options for entry at precisely the point when care needs were greatest. The insufficiency of benefits was termed "care that cannot sustain life." The benefits provided at the utilization stage were concentrated on survival-oriented tasks such as meals, cleaning, and laundry, and did not sufficiently encompass domains necessary for sustaining life—such as going out, hospital visits, and social relationships—while the continuity of care was also unstable owing to staffing gaps and frequent turnover. These blind spots arose and were intensified at the points where multiple social positions—including not only visual impairment and old age but also living alone and regionality—intersected.
Based on this analysis, the study proposed the following directions for policy improvement. First, to compensate for the limitations of the application-based delivery system, older adults with visual impairments should be actively identified, and information should be provided in formats that visually impaired persons can actually comprehend—such as voice, telephone, and in-person guidance—rather than relying on notices and written guidance. In addition, public case management spanning the entire process from application through assessment to service use and adjustment should be strengthened. Second, the long-term care eligibility assessment system should be supplemented so that, beyond its focus on physical and cognitive function, it can reflect as care needs the difficulties in information access, mobility, meal preparation, and risk avoidance that arise from sensory disability. Third, the structure that divides the Activity Support Service for Persons with Disabilities and the Long-Term Care Insurance at the threshold of age 65 should be relaxed so that the systems can be used on the basis of actual care needs. Fourth, the content of care should not be confined to survival-oriented household support but expanded into support for the continuation of life—including going out, hospital visits, the maintenance of social relationships, and community participation; and stable staffing and handover systems should be established to reduce the care gaps caused by service interruptions and frequent turnover in the assignment of care workers.
This study is significant in that it analyzed the coverage of social care not on the basis of whether the systems exist, but on the basis of how they actually reach and operate within the lives of those directly affected. It is further significant in that it renders visible older adults with visual impairments—who have hitherto been treated within the single category of either "older adults" or "persons with disabilities"—as a group positioned at the intersection of both characteristics, and reveals that the care blind spots they experience stem not from a mere shortage of services but from the structural limitations of the current care system in its target setting, assessment criteria, benefit content, and delivery methods.
