The purpose of this study was to investigate the effects of closed kinetic chain exercises performed on stable and unstable support surfaces on balance ability and muscle thickness in adults with Functional Ankle Instability (FAI). Functional ankle instability is characterized by recurrent ankle injuries, impaired proprioception, and neuromuscular control deficits, which are known to contribute to decreased postural control and functional weakness of the lower extremity muscles. Therefore, this study aimed to examine the effects of exercise stimuli under different support surface conditions on balance performance and muscle thickness and to provide evidence for rehabilitation strategies.
Thirty adults in their 20s and 30s with FAI participated in this study and were randomly assigned to either a stable support surface exercise group (n = 15) or an unstable support surface exercise group (n = 15). Both groups performed the same exercise program consisting of closed kinetic chain exercises, including squats and lunges, three times per week for four weeks, with each session lasting 40 minutes. Static balance was assessed using the Romberg test, and dynamic balance was evaluated using the Limits of Stability (LOS) test before and after the intervention. Structural changes in lower extremity muscles were measured using ultrasonography to determine the muscle thickness of the vastus medialis, vastus lateralis, tibialis anterior, and medial gastrocnemius.
The results showed that dynamic balance significantly improved following the intervention in the unstable support surface exercise group; however, no statistically significant differences were observed between the groups. Likewise, no significant between-group differences were found in static balance. In contrast, muscle thickness of the vastus medialis and medial gastrocnemius increased significantly more in the unstable support surface exercise group than in the stable support surface exercise group (p 〈 .05).
These findings suggest that closed kinetic chain exercises performed on an unstable support surface are effective in enhancing lower extremity muscle stabilization by improving balance ability and promoting selective muscular adaptations under weight-bearing conditions. In particular, the observed increases in the thickness of the medial gastrocnemius and vastus medialis are associated with improved stability of the ankle and knee joints, which may contribute to the prevention of recurrent injuries and functional recovery in individuals with functional ankle instability.
In conclusion, closed kinetic chain exercises performed on an unstable support surface may be utilized as an effective rehabilitation intervention to enhance lower extremity stabilization strategies in individuals with functional ankle instability. Future studies should include larger sample sizes and longer intervention periods to further clarify the relationship between changes in muscle thickness, functional recovery, and the prevention of recurrent injuries.
