The purpose of this study was to investigate the effects of perturbation-based balance training using pressure biofeedback on balance ability, hip function, and fall efficacy in patients with total hip arthroplasty (THA).
Thirty patients who underwent THA were randomly assigned to either an experimental group (n=15) or a control group (n=15). The experimental group performed perturbation-based balance training using pressure biofeedback in addition to conventional physical therapy, while the control group received traditional balance training with conventional physical therapy. Both groups participated in the intervention for 15 minutes per session, 5 times per week, for 6 weeks. Outcome measures included the Berg Balance Scale (BBS), Functional Reach Test (FRT), Falls Efficacy Scale (FES), Harris Hip Score (HHS), Lower Extremity Functional Scale (LEFS), and center of pressure (COP) variables. Data were analyzed using a linear mixed model.
Both groups showed significant improvements over time in BBS, FES, HHS, and LEFS (p 〈 .05). However, no significant group differences or interaction effects were observed in these variables. In the FRT, a significant time × group interaction effect was found (p 〈 .05). In COP analysis, significant interaction effects were observed in velocity and sway path length under both standing and eyes-closed conditions, as well as mediolateral range under the eyes-closed condition (p 〈 .05). The experimental group demonstrated reduced postural sway and improved postural stability after the intervention.
These findings suggest that perturbation-based balance training using pressure biofeedback may improve postural control and reactive balance strategies in patients with THA. Although significant superiority in clinical functional outcomes was not observed compared with traditional balance training, the intervention may contribute to qualitative improvements in postural stability and neuromuscular control.
