According to data from the National Institute of Dementia (Central Dementia Center), the population of older adults aged 65 and over is continuously increasing, leading to a concurrent rise in the number of dementia patients. Dementia involves cognitive impairment that restricts patients' participation in activities of daily living, thereby deteriorating their subjective quality of life. Consequently, this results in increased physical and psychological burdens, as well as economic costs, for both the patients and their caregivers.
This study aims to develop a magic-based program as a non-pharmacological intervention to prevent dementia and maintain residual functions in older adults. To achieve this, a multisensory stimulation program specifically tailored for older adults was constructed by integrating the therapeutic elements of performing magic with the therapeutic principles of occupational therapy.
The program development process followed a modified ADDIE model, consisting of four stages: analysis, design, development, and evaluation. In the analysis stage, a total of 16 prior studies related to magic and programs targeting older adults and those with mild cognitive impairment (MCI) were analyzed. A needs assessment was also conducted using a questionnaire with experts currently facilitating various programs for older adults in clinical and community settings, including four occupational therapists, three magicians, two social workers, and one nurse.
In the design stage, the foundation of the program, including its objectives and the number of sessions, was established based on the analysis results. Considering the continuity and effectiveness of the intervention, the program was structured into a total of eight sessions. Eight types of magic tools were selected, meeting the criteria that reflect the physical and cognitive characteristics of older adults while being suitable for achieving the specific goals of each session.
In the development stage, the progression of each session was categorized into the following steps: introduction, warm-up exercises, magic demonstration, deducing the solution, solution lecture and imitation, magic practice, occupational therapy task performance, presentation, and wrap-up. Subsequently, occupational therapy activities corresponding to the characteristics of the eight selected magic tools were strategically integrated, completing the draft of the eight-session program structure and detailed scripts.
In the evaluation stage, an expert panel was formed to assess the appropriateness of the developed program's overall objectives, structural coherence, ease of field application, and teaching strategies. Feedback on the program was collected from a total of ten experts via a questionnaire. The program was finalized by incorporating this feedback.
The conclusions of this study are as follows.
First, by reviewing previous literature, programs targeting older adults were examined, and the direction of the program was determined by analyzing the needs of experts working with older adults in clinical and community settings.
Second, teaching and learning lesson plans and detailed scripts for each session were created so that anyone, including occupational therapists, magicians, and social workers, can utilize this program developed by integrating magic and occupational therapy.
Third, the program underwent review and revision by an expert panel and was designed to allow adjustable difficulty levels for both magic and occupational therapy tasks, tailoring them to the characteristics of the participants, such as older adults, individuals with mild cognitive impairment, and early-stage dementia patients. Furthermore, it is also applicable to adolescents or adults requiring cognitive stimulation.
Dementia in older adults is a disease that brings hardship not only to the individuals themselves but also to their families. Preventing dementia can serve as a method to maintain the quality of life for older adults and reduce social costs.
It is hoped that this study will be of significant help to older adults in clinical and community settings, and that more magicians and occupational therapists will continue research exploring the therapeutic effects of magic.
