Persistent Postural-Perceptual Dizziness (PPPD) is a functional vestibular disorder characterized by persistent unsteadiness, posture-related dizziness, and heightened sensitivity to sensory stimuli. Although it lacks identifiable structural pathology, PPPD significantly impairs postural control, emotional well-being, and social participation. Currently, evidence supporting individualized, multidimensional rehabilitation strategies for PPPD remains limited.
This study employed a Single-Case Experimental Design (SCED) to examine the effects of an integrated intervention—comprising breathing exercises, vestibular rehabilitation, and balance training—on postural control and emotional status in individuals with PPPD. Three female participants (two in their 30s and one in her 40s) received a structured three-phase intervention comprising breathing pre-regulation, vestibular–balance training, and breathing consolidation. Postural control was assessed using static balance parameters (velocity, path length, and sway area) and the Y-Balance Test. Emotional status was measured with the Hospital Anxiety and Depression Scale (HADS), and subjective dizziness symptoms were evaluated using the Vertigo Symptom Scale–short form (VSS-sf). Intervention effects were analyzed through visual analysis, outlier screening, and Tau-U statistics.
The results indicated that static balance parameters decreased markedly following the intervention and remained stable during the follow-up period. The removal of heterogeneous outliers yielded smoother and more interpretable trend lines. In the Tau-U analysis, velocity, path length, and sway area showed significant negative effect sizes (p 〈 0.05), indicating improved postural stability. Dynamic balance progressively improved throughout the intervention, with all participants consistently completing the Y-Balance Test during the mid-to-late intervention phases. Regarding emotional and subjective symptoms, HADS scores returned to the normal range, and VSS-sf total scores decreased by 55–65%, reflecting substantial reductions in anxiety, autonomic tension, and dizziness.
In summary, the integrated intervention effectively improved both static and dynamic balance performance in individuals with PPPD, while substantially alleviating emotional symptoms and subjective dizziness. Breathing exercises played a key role in enhancing autonomic homeostasis and training tolerance, whereas vestibular and balance training facilitated sensory reweighting and the reconstruction of postural control strategies. This intervention model demonstrates strong feasibility and promising clinical utility. Future research should involve larger sample sizes and longer follow-up periods to consolidate evidence regarding its applicability and underlying mechanisms.
