Digital therapeutics (DTx) have emerged as novel software-based interventions for the prevention, management, and treatment of diseases. In particular, digital therapeutics for mental disorders have been recognized as promising alternatives for improving patient access to treatment and addressing unmet healthcare needs by delivering non-pharmacological interventions such as cognitive behavioral therapy (CBT) through digital platforms. Although the introduction of digital therapeutics has expanded in South Korea, their actual prescription and utilization remain limited. This suggests that the structure and operation of reimbursement systems may play an important role in facilitating the real-world utilization of digital therapeutics.
This study aimed to examine how the structural characteristics and operational frameworks of reimbursement systems influence the utilization of digital therapeutics for mental disorders and to derive policy improvement strategies for the Korean reimbursement system. A literature-based comparative research method was employed, and academic articles, laws and regulations, government and public agency documents, policy reports, and insurer policy documents were collected and analyzed. South Korea, the United States, and Germany were selected as comparative cases. Reimbursement systems were compared in terms of reimbursement listing procedures, reimbursement pricing mechanisms, patient cost-sharing structures, and the level of disclosure of reimbursement-related information. Based on the comparative analysis, SWOT analysis and the TOWS matrix were applied to identify barriers to utilization and derive policy improvement strategies.
The findings indicated that all three countries had established reimbursement frameworks to support the utilization of digital therapeutics for mental disorders, although substantial differences existed in reimbursement listing procedures and operational structures. South Korea operated multiple market entry pathways, including the Integrated Review and Assessment System for Innovative Medical Devices, the New Health Technology Assessment Deferral Program, and the Immediate Market Entry Medical Technology Program. The United States maintained a decentralized reimbursement system centered on Medicare, Medicaid, and private insurers, resulting in variations in reimbursement coverage and payment levels across insurers. Germany operated a reimbursement pathway through the DiGA Fast-Track system based on an early access and post-evaluation approach and adopted a value-based pricing structure.
The SWOT analysis identified five major institutional barriers to the utilization of digital therapeutics for mental disorders in South Korea: the absence of clearly established criteria and operational frameworks for permanent reimbursement, high patient cost-sharing burdens, reimbursement mechanisms that do not sufficiently reflect clinical value and real-world performance, limited disclosure of reimbursement-related information, and challenges associated with maintaining user persistence and adherence as well as demonstrating long-term treatment effectiveness. These factors were found to interact with one another and potentially restrict both the expansion of utilization and the accumulation of real-world evidence.
Based on the TOWS matrix, this study proposed several policy improvement strategies for the Korean reimbursement system. The SO strategy focused on expanding utilization by leveraging multiple market entry pathways and information disclosure systems. The WO strategy emphasized establishing permanent reimbursement evaluation frameworks, implementing value-based reimbursement mechanisms, reducing patient cost-sharing burdens, and expanding the disclosure of reimbursement-related information. The ST strategy proposed the development of real-world data-based performance management systems to evaluate persistence, adherence, and long-term treatment outcomes, while the WT strategy emphasized reassessment and post-market management systems to enhance the stability andsustainability of reimbursement policies.
This study contributes to the literature by examining reimbursement systems not merely in terms of reimbursement eligibility but also in relation to real-world utilization environments. Furthermore, it provided a balanced analysis of the institutional characteristics and limitations of different national reimbursement systems rather than assuming a single ideal model. By identifying institutional barriers to utilization and proposing policy improvement strategies through SWOT and TOWS analyses, this study offers policy implications for the future development of reimbursement systems for digital therapeutics in South Korea.
