This study examined the psychosocial mechanisms underlying college students’ intention to engage in the nonmedical use of prescription stimulants (NMUPS) in South Korea. Rather than viewing NMUPS intention as a simple outcome of drug availability or individual deviance, this study conceptualized it as a structured process involving social anxiety, use motives, and medicalization beliefs. In particular, the study examined NMUPS intention as part of a medicalized self-management process in which academic performance difficulties are interpreted as problems that may be corrected through medical or pharmaceutical intervention.
A multi-phase, mixed-methods research design was employed. First, nationwide methylphenidate use reported to the Narcotics Information Management System (NIMS) from January 2020 to December 2024 was analyzed to examine long-term trends, seasonality, regional heterogeneity, and age-specific patterns. Second, semi-structured in-depth interviews were conducted with 22 college students in Seoul who had used methylphenidate within the past year. Third, based on the literature review and qualitative findings, survey items measuring social anxiety, use motives, medicalization beliefs, and vignette-based NMUPS intention were developed, and a validation sample of 500 students was split into exploratory and confirmatory factor analysis subsamples to evaluate the measurement model. Fourth, structural equation modeling (SEM) was performed (n = 1,103) to test the direct and indirect effects, as well as moderated mediation pathways, within the proposed structural model.
The NIMS analysis showed a steady increase in methylphenidate use after 2020, with significant monthly seasonality and regional heterogeneity. Although these findings do not directly indicate an increase in NMUPS, they provide a contextual background in which prescription stimulants have become increasingly visible and potentially perceived as more accessible. The qualitative findings reconstructed social anxiety into four dimensions: outcome evaluation anxiety, process exposure anxiety, relational burden anxiety, and loss of performance control. Use motives were classified into functional coping, emotional coping, and competitive enhancement. Participants also tended to interpret difficulties such as poor concentration, fatigue, lethargy, and difficulty initiating tasks as problems that could be corrected through medical or pharmaceutical intervention, providing the empirical basis for medicalization beliefs.
The SEM results showed that loss of performance control was the most consistent antecedent of all three use motives. Among the motives, emotional coping and competitive enhancement had significant positive effects on NMUPS intention, with competitive enhancement showing the stronger effect. Functional coping did not have a significant direct effect on NMUPS intention. This suggests that functional difficulties such as fatigue, lethargy, and poor concentration do not necessarily translate directly into NMUPS intention; rather, the functional coping pathway should be interpreted as a limited conditional pathway in which the size of the indirect effect increases as medicalization beliefs become stronger. Medicalization beliefs were the strongest direct predictor of NMUPS intention and functioned as a common cognitive condition that strengthened the translation of use motives into NMUPS intention, rather than as a moderator limited to a single motive pathway. The most salient indirect pathway was from loss of performance control to NMUPS intention through competitive enhancement, and this pathway was strengthened by higher levels of medicalization beliefs.
In conclusion, college students’ NMUPS intention in South Korea should be understood not as a direct result of social anxiety itself, but as a conditional process in which loss of performance control is translated into use motives and legitimized by medicalization beliefs. These findings indicate that NMUPS intention is not merely a function of anxiety reduction or achievement striving, but is also related to a medicalized form of self-management in which academic performance difficulties are reframed as targets of medical or pharmaceutical correction. Prevention strategies should therefore move beyond risk-focused drug education and address loss of performance control, competitive academic pressure, inflated expectations about medication effects, and medicalization beliefs. In particular, critical medication literacy education and multidisciplinary early intervention strategies linking counseling, student health, and academic support services are needed to help students distinguish therapeutic use from nonmedical use and to prevent academic performance difficulties from being immediately interpreted as problems requiring pharmaceutical correction.
