    CiPA in silico 심장 안전성 평가 체계는 CiPA-ORDv1.0 인간 심실 심근세포 모델을 사용하여 여러 이온채널 차단 정보를 바탕으로 약물 유발 tor-sades de pointes (TdP) 위험을 예측한다. 그러나 기존 CiPA 체계는 일반적으로 동일한 전기생리학적 조건을 가정하므로, 실제 환자 집단에서 나타나는 이온채널 발현과 약물 반응의 개인 간 차이를 충분히 반영하지 못할 수 있다. 본 논문에서는 개인 간 전기생리학적 변이가 qNet 기반 TdP 위험 분류에 미치는 영향을 평가하고, 두 가지 바이오마커 조합이 이질적인 집단 조건에서 발생하는 분류 성능 저하를 극복할 수 있는지를 분석하였다.
    시뮬레이션은 동적 hERG 반응속도론이 포함된 CiPA-ORDv1.0 모델을 사용하여 수행하였으며, 28개의 CiPA 기준 약물을 대상으로 1×1×–4×Cmax⁡4×Cmax 농도에서 평가하였다. 40명의 가상 개체군은 생리학적으로 타당한 범위 안에서 9개의 이온 전도도를 변화시켜 생성하였다. 총 네 가지 학습–검증 시나리오를 평가하였다. 첫 번째 분석에서는 qNet을 단일 입력 특성으로 사용하여 순서형 로지스틱 회귀(OLR) 분류기를 구성하였다. 두 번째 분석에서는 11개의 전기생리학적 바이오마커로부터 가능한 모든 55개의 두 바이오마커 조합을 평가하였다. 모델 성능은 사전에 정의된 CiPA 기준의 허용 기준에 따라 ROC 곡선하면적(AUC), 양성 및 음성 가능도비(LR+, LR−), 그리고 분류 오류를 사용하여 평가하였다.
    qNet 기반 OLR 분류기는 균일 조건인 시나리오 1에서 ROC 1의 AUC가 0.90, ROC 2의 AUC가 0.95, 분류 오류가 25.9%로 높은 성능을 보였다. 그러나 시나리오 2에서는 성능이 크게 감소하였으며, ROC 1의 AUC는 0.60으로 낮아지고 분류 오류는 96.3%로 증가하였다. 학습 단계에 개인 간 변이를 포함한 시나리오 3과 시나리오 4에서는 성능이 회복되어 AUC는 0.80–0.87, 분류 오류는 약 50%를 보였다. 이는 이질적인 가상 집단과 학습에 포함되지 않은 새로운 가상 개체에 대한 일반화 성능이 향상되었음을 보여준다.두 바이오마커 분석에서는 55개 조합 중 시나리오 1에서 40개 조합이 허용 기준을 만족하였으나, 시나리오 2에서는 APD90–max_dv 한 개 조합만 허용되었다. 학습 단계에 변이를 포함한 경우, 시나리오 3과 시나리오 4에서 각각 19개와 20개 조합이 허용되었다. 이 중 16개 조합은 시나리오 1, 3, 4에서 일관되게 허용되었으며, 대부분 APD90 또는 APD50을 포함하였다. 시나리오 3과 4에서 가장 우수한 조합인 APD90–CaTD90 및 APD90–dvdtmax는 변이 인식 조건에서 ROC 2의 AUC가 0.95, 분류 오류가 약 36%로 나타났다. 이는 qNet 단일 바이오마커 모델보다 더 우수한 성능을 보였으며, 개인 간 전기생리학적 변이로 인한 성능 저하를 완화하는 데 기여하였다.

    The CiPA in silico cardiac safety framework uses the CiPA-ORDv1.0 human ventricular cardiomyocyte model to predict drug-induced torsades de pointes (TdP) risk from multi-channel ion-block data. However, its standard implementation commonly assumes homogeneous electrophysiological conditions, which may not fully represent inter-individual differences in ion channel expression and drug response. This thesis evaluates how inter-individual electrophysiological variability affects qNet-based TdP risk classification and investigates whether two-biomarker combinations can overcome the degradation in classifier performance observed under heterogeneous population conditions.
    Simulations were performed using the CiPA-ORDv1.0 model with dynamic hERG kinetics for the 28 CiPA reference drugs at 1×–4× Cmax. A virtual population of 40 individuals was generated by varying nine ionic conductances within physiologically plausible ranges. Four training–testing scenarios were evaluated. The first analysis used qNet as a single input feature for ordinal logistic regression (OLR). The second analysis evaluated all 55 possible two-biomarker combinations derived from 11 electrophysiological biomarkers. Model performance was assessed using AUC, positive and negative likelihood ratios (LR+, LR−), and classification error based on predefined CiPA-aligned acceptance criteria.
    The qNet-based OLR classifier showed strong performance under homogeneous conditions in scenario 1, with AUC ROC 1 of 0.90, AUC ROC 2 of 0.95, and classification error of 25.9%. However, performance declined sharply in scenario 2, where AUC ROC 1 decreased to 0.60 and classification error increased to 96.3%. When variability was included during training, performance improved in scenarios 3 and 4, with AUC values of 0.80–0.87 and classification error of approximately 50%, indicating improved generalization to heterogeneous and unseen virtual individuals.
    For the two-biomarker analysis, 40 of 55 combinations were accepted in scenario 1, whereas only one combination, APD90–max_dv, was accepted in scenario 2. When variability was incorporated during training, 19 and 20 combinations were accepted in scenarios 3 and 4, respectively. Sixteen combinations were consistently accepted across Scenarios 1, 3, and 4, most of which involved APD90 or APD50. The top performing combinations, including APD90–CaTD90 and APD90–dvdtmax on scenario 3 and 4, achieved AUC ROC 2 of 0.95 and classification error of approximately 36% under variability-aware conditions, showing better performance than the qNet single biomarker model and helping overcome the performance loss observed under inter-individual variability.
    This thesis demonstrates that high performance under homogeneous conditions does not guarantee reliable TdP risk classification under inter-individual electrophysiological variability. Incorporating variability during classifier training improves robustness and supports generalization to unseen virtual individuals. More importantly, two-biomarker combinations can further overcome the limitations of the qNet single-biomarker approach by capturing complementary electrophysiological information. Overall, this work provides a scenario-based framework for evaluating variability-aware in silico cardiac safety models and identifies robust two-biomarker candidates that may support future extensions of the CiPA framework.
