The purpose of this study was to explore the process of death acceptance among middle-aged adults and to identify its structure and meaning. While previous studies on death attitudes and death acceptance have tended to conceptualize death acceptance as a state or an outcome achieved at a specific point in time, this study sought to understand death acceptance as a dynamic, process-oriented experience that is formed and transformed within an individual's life context. Thus, this study sought to derive a grounded substantive theory that explains the process of death acceptance among middle-aged adults by examining how death is accepted, what conditions shape this process, and what transformations occur. For this purpose, in-depth interviews were conducted with 20 middle-aged adults aged between 40 and 64 who exhibited accepting orientations toward death. The collected data were analyzed using the grounded theory methodology of Strauss and Corbin(1998).
The analysis yielded 119 concepts, 48 subcategories, and 22 categories through open coding. In the paradigm model analysis, the causal conditions were identified as ‘a vague preexisting attitude toward death’ and ‘the occurrence of death/death-crisis events threatening existing perceptions’, while the central phenomenon was ‘confronting the reality of death’. This central phenomenon was experienced within contextual conditions such as ‘social factors’, ‘developmental factors’, and ‘situational factors’. Participants responded to this through action/interaction strategies, including ‘initial defensive response’, ‘emotional approach’, ‘transcendental coping’, ‘relational communication’, ‘cognitive restructuring and meaning seeking’, and ‘managing real life’. Furthermore, ‘transcendence/worldview factors’, ‘socio-relational factors’, ‘life-cycle(middle-age) factors’, and ‘personal factors’ acted as intervening conditions that facilitated or inhibited these strategies. Consequently, participants experienced changes such as ‘self-acceptance’, ‘acceptance of death as an agent’, ‘realignment of life through awareness of finitude’, ‘expanded understanding of human existence’, and ‘integrative reconstruction of meaning of life and death’. Concurrently, a persistent tension within death acceptance remained alongside these changes.
The process of death acceptance among middle-aged adults was structured into a seven-stage process model consisting of: ‘vague awareness of death’, ‘collision with the reality of death’, ‘defensive responses’, ‘confrontation and exploration for adaptation’, ‘regulating the balance between life and death’, ‘death acceptance and life reorganization’, and ‘expanded understanding of existence’. Rather than culminating in a single completed state, this process was characterized by a recursive restructuring process, in which adaptation and regulation were continually re-engaged whenever individuals encountered new realities of death.
Through selective coding, the core category was derived as “reconstructing the meaning of life and existence through repeated encounters with the reality of death.” In addition, four types of death acceptance among middle-aged adults were identified according to their approaches to death and patterns of meaning-making: the Reflective–Integrative Type, the Faith-Meaning Entrustment type, the Reality-Agentic Management Type, and the Contemplative Distance-Regulation Type.
This study reconceptualizes death acceptance among middle-aged adults not as a one-time event or a fixed psychological state, but as a dynamic process in which the meanings of life and existence are continuously reconstructed through repeated encounters with and adaptations to the reality of death. Moreover, it provides theoretical and practical implications by understanding death acceptance as a multidimensional adaptive process integrating emotional, cognitive, and behavioral dimensions and by empirically elucidating the structure and mechanisms of death acceptance specific to the developmental period of middle adulthood.
