급격한 고령화는 노화로 인한 인체의 해부학적 변화에 대한 관심을 증대시키고 있으며, 이에 따른 임상적 연구의 필요성 또한 증가하고 있다. 노화는 배안 압력의 재분포, 지지 구조의 약화, 척주뒤굽음(kyphosis) 등의 해부학적 변화를 유발하며, 이로 인해 배안 및 골반 장기의 처짐이 발생할 수 있다. 이러한 장기 처짐 현상은 물콩팥증(hydronephrosis), 배대동맥자루(abdominal aortic aneurysm), 골반장기탈출증(POP, pelvic organ prolapse) 등 다양한 중증 질환과 밀접하게 연관된다. 기존 시각 자료는 정적인 이미지나 개별 장기 변화에 초점을 맞추어 장기 간 해부학적 연관성과 시간 경과에 따른 위치 및 형태 변화를 통합적으로 설명하는 데 한계가 있다. 본 연구는 해부학 및 임상 문헌을 기반으로, 노화에 따라 배안 장기들이 상호 연관되어 변화하는 과정을 통합적으로 시각화한 3D 시뮬레이션 모델을 개발한다. 연구는 배안 장기 처짐(Visceral Ptosis) 현상을 정량적으로 구현하기 위해, 허리 앞굽이(Lumbar Lordosis, LL) 감소, 몸통 앞쪽 기울임, 배안 압력(Intra-abdominal pressure, IAP) 감소 등 노화의 주요 해부학적 변화를 선행 연구의 정량적 수치 데이터를 기반으로 반영한다. 특히 콩팥의 수직 처짐 및 POP-Q 기준 등 정량화된 지표를 시뮬레이션에 적용한다. 3D 시뮬레이션 제작에는 MetaHuman Creator, Maya, ZBrush, Substance 3D Painter를 활용하며, 수집된 정량적 데이터를 3D 모델에 정밀하게 반영한다. 본 모델은 척주 기울기 변화와 배안 압력 감소에 따른 장기들의 연쇄적 하강 및 물콩팥증에 따른 오줌관의 변화 등 병변 형태를 구현한다. 이를 통해 척주의 형태 및 배안 압력 변화가 콩팥, 오줌관, 방광, 자궁 등 다중 장기 간 해부학적 관계 변화와 장기 처짐의 진행 양상을 어떻게 유발하는지 입체적인 동적 영상으로 재구성한다. 본 연구의 가장 큰 의의는 기존 정적 자료에서 파악하기 어려웠던 노화에 따른 장기 위치 및 형태 변화를 임상 수치를 기반으로 한 동적 시뮬레이션으로 구현한 데 있다. 특히, CT 데이터 기반의 문헌과 장기 처짐 거리 등의 정량적 데이터를 3D 모델에 직접 반영하여 시뮬레이션의 객관성과 신뢰도를 높인다. 또한, 배대동맥의 확장을 임상적 위험 기준인 50mm 이상과 연관 지어 시각화함으로써 의학적 중요성을 갖는 병리적 기전을 효과적으로 제시한다. 본 모델은 시간에 따른 변화를 보여주는 영상의 특성상 일부 가설적 요소를 포함하나, 해부학 교육 및 노화 관련 메디컬 일러스트레이션 연구를 위한 심도 있는 기초 시각 자료로 활용될 수 있을 것으로 기대한다.

The rapid increase in the elderly population has amplified interest in age-related anatomical changes and, consequently, the necessity for clinical research in this area. Aging induces anatomical changes such as the redistribution of intra-abdominal pressure (IAP), weakening of supporting structures, and kyphosis, which can ultimately lead to the ptosis of abdominal and pelvic organs. This phenomenon of organ ptosis is closely associated with various severe diseases, including hydronephrosis, abdominal aortic aneurysm (AAA), and pelvic organ prolapse (POP). Conventional visual resources often focus on static images or changes in individual organs, resulting in a significant limitation in providing integrated, dynamic visual materials that explain inter-organ anatomical relationships and their evolution in position and morphology over time. This study develops a 3D simulation model that comprehensively visualizes the interconnected changes among intra-abdominal organs in relation to aging, based on anatomical and clinical literature. To quantitatively implement the phenomenon of Visceral Ptosis, the study reflects key age-related anatomical changes based on quantitative numerical data from prior research, such as the reduction of Lumbar Lordosis (LL), forward truncal inclination, and decrease in Intra-abdominal pressure (IAP). Specifically, quantified indices, including the vertical displacement of the kidney and the POP-Q standard for pelvic organ prolapse, are applied to the simulation. The 3D simulation is developed using MetaHuman Creator, Maya, ZBrush, and Substance 3D Painter, and the collected quantitative data is precisely reflected in the 3D model. This model implements the sequential descent of organs resulting from changes in spinal curvature and reduced IAP, as well as pathological morphologies such as the changes in the ureter associated with hydronephrosis. Through this dynamic visualization, the model reconstructs how changes in spinal morphology and IAP lead to alterations in the anatomical relationships among multiple organs—including the kidney, ureter, bladder, and uterus—and the progression of organ ptosis. The greatest significance of this study lies in its implementation of a dynamic simulation of age-related organ position and morphological changes, grounded in clinical quantitative metrics, which is difficult to achieve with existing static resources. Notably, quantitative data such as organ ptosis distance derived from CT data-based literature is directly reflected in the 3D model, enhancing the simulation's objectivity and reliability. Furthermore, the visualization of the abdominal aortic expansion in relation to the clinical risk standard of 50mm or more effectively presents the medical significance of this pathological mechanism. Although the time-dependent nature of the video inherently incorporates some hypothetical elements, which imposes limitations on its overall interpretation, this model is expected to serve as in-depth foundational visual material for anatomical education and medical illustration research related to aging.
