목적
본 연구는 한국인 비만 성인을 대상으로 사회경제적 지위가 근감소성 비만 및 관련 만성질환(고혈압, 당뇨병, 이상지질혈증)의 장기적 발생 위험에 미치는 영향을 평가하고자 하였다.
방법
KoGES(Korean Genome and Epidemiology Study) 코호트 데이터를 기반으로, 비만(BMI ≥25kg/m²)이면서 근감소증이 없는(sex-specific SMM/BMI 〉20th percentile) 성인 2,794명을 평균 17년간 추적 관찰하였다. 사회경제적 지위는 교육 수준, 가계 소득, 거주 지역, 혼인 상태를 기준으로 평가하였다. 근감소성 비만(1차 결과)과 고혈압, 당뇨병, 이상지질혈증(2차 결과)에 대한 누적 발생률은 Kaplan–Meier 방법을 통해 산출하였으며, 발생 위험은 Cox 비례위험모형을 이용해 평가하였다.
결과
사회경제적 지위가 낮을수록 근감소성 비만 및 관련 만성질환의 누적 발생률과 발생 위험이 유의하게 증가하였다. 특히 교육 수준과 가계 소득이 높을수록 근감소성 비만의 발생 위험은 각각 HR 0.77 (p for trend = 0.009), HR 0.67 (p for trend = 0.005)로 낮았으며, 이는 모든 교란 변수를 보정한 결과이다. 도시 거주자와 기혼자 역시 상대적으로 낮은 위험을 보였다. 이러한 경향은 고혈압과 당뇨병의 누적 발생에서도 유사하게 나타났으며, 교육 및 소득 수준이 높을수록 해당 질환의 누적 발생률과 발생 위험 역시 감소하였다.
결론
사회경제적 지위는 비만 성인의 근감소성 비만 및 관련 만성질환의 누적 발생에 중요한 영향을 미치며, 특히 교육 수준과 소득 수준은 건강 형평성에 중요한 결정 요인으로 확인되었다. 이는 단순한 건강 행태 중재를 넘어, 사회경제적 취약계층을 대상으로 한 구조적 공중보건 개입이 필요함을 강조한다.

This study aimed to evaluate the long-term effects of socioeconomic status on the risk of developing sarcopenic obesity and related chronic diseases (hypertension, diabetes mellitus, and dyslipidemia) among Korean adults with obesity.
Methods
Using data from the Korean Genome and Epidemiology Study (KoGES), a total of 2,794 adults with obesity (BMI ≥25 kg/m²) but without sarcopenia (defined as a sex-specific MMI 〉20th percentile) were followed for an average of 17 years. Socioeconomic status was assessed based on educational attainment, household income, residential area, and marital status. The cumulative incidence rates of sarcopenic obesity (primary outcome) and major chronic diseases including hypertension, diabetes mellitus, and dyslipidemia (secondary outcomes) were analyzed using Kaplan–Meier survival curves and the risk of occurrence was assessed using the Cox proportional hazards models.
Results
Lower socioeconomic status was significantly associated with a higher cumulative incidence and risk of sarcopenic obesity and related chronic diseases. In particular, higher levels of education and income were associated with significantly lower risks of sarcopenic obesity HR 0.77 (p for trend = 0.009) and HR 0.67 (p for trend = 0.005), even after adjusting for all confounding variables. Urban residents and married individuals also showed relatively lower risks. Similar patterns were observed in the cumulative incidence of hypertension and diabetes mellitus, with higher education and income levels also decreasing the cumulative incidence and risk of these diseases.
Conclusion
Socioeconomic status plays a crucial role in the cumulative incidence and development of sarcopenic obesity and related chronic diseases among adults with obesity. In particular, education and income levels were found to be key determinants of health inequality. These findings highlight the necessity of structural public health interventions targeting socioeconomically disadvantaged populations, beyond individual-level behavioral approaches, to improve health equity.
