이 연구는 의료 이용자의 의학적 필요와 지역의 의료 자원 공급 특성을 고려하여 지역별 의료비 지출의 변이를 체계적으로 분석하고 이를 모니터링할 수 있는 체계를 구축하는 것을 목적으로 수행되었다. 의료비의 지역 간 변이는 단순한 지출 규모의 차이를 넘어 의료 자원 배분의 효율성과 지역 간 형평성 문제를 드러내는 중요한 지표다. 미국과 영국 등 주요 선진국에서는 이미 다트머스 아틀라스나 NHS 아틀라스와 같은 상시적인 모니터링 체계를 구축하여 지역 간 의료 이용과 지출의 불필요한 변이를 감시하고 연구 결과를 정책에 반영하여 비효율을 개선하고 있다. 한국에서도 여러 연구가 진행되었으나 특정 시점이나 질환에 국한된 연구가 주를 이루었으며 환자의 의학적 필요 요인과 지역의 공급 요인을 포괄적으로 고려하여 의료비를 설명한 연구는 없었다. 따라서 이 연구는 국민건강보험공단 표본 코호트 DB를 활용하여 시군구별 의료비의 변이를 분석하고 정당한 변이와 불필요한 변이를 구분하여 시군구별 의료비의 특성을 규명하는 모니터링 체계를 제안하고자 한다.
이 연구는 국민건강보험공단 표본 코호트 DB의 2016년부터 2019년까지 4개년 자료를 이용하여 전국 246개 시군구를 대상으로 분석을 진행하였다. 연구 대상은 건강보험 및 의료급여 자격을 유지한 모든 대상자이며 분석 단위는 연도별 개인으로 각 연도의 개인을 독립적으로 취급하여 분석하였다. 분석 절차는 크게 네 단계로 구성된다. 첫째, 시군구별 1인당 평균 의료비를 산출하여 지역 간 변이를 확인했다. 둘째, 의료비 설명 모형을 구축하여 요인별 영향력과 지역 간 변이 설명력을 확인하였다. 이때 설명 요인은 개인 수준의 필요 요인과 지역 수준의 의료 공급 요인으로 범주화했다. 필요 요인에는 인구학적, 사회경제적, 건강 요인이 포함되었으며 의료 공급 요인에는 300병상 이상 종합병원 유무 및 병상 수, 의사 수가 포함되었다. 의료비 데이터의 분포적 특성을 고려하여 로그 연결 함수와 Tweedie 분포를 가정한 일반화 선형 모형을 적용하였다. 셋째, 환자의 필요 요인을 반영한 기대 의료비 대비 실제 의료비의 비율인 O/E ratio를 산출하여 지역별 의료비의 상대적 수준을 평가하고 이에 대해 의료 공급 요인이 미치는 영향을 정량적으로 분석했다. 넷째, 지역별 의료비와 O/E ratio 그리고 의료 공급 요인의 영향을 종합하여 시군구를 유형화하여 구분하고 각 유형의 특성을 분석하여 정책적 시사점을 제시했다. 이 연구는 전체 의료비에 대한 분석을 중심으로 진행하였으며, 보조적으로 외래와 입원을 분리하여 하위그룹 분석을 시행하였다.
연구 결과 의료비에는 상당한 지역 간 격차가 나타나 상위 지역과 하위 지역 간에 최대 3배 이상의 차이가 확인되었다. 구축된 의료비 설명 모형은 지역 간 의료비 변이의 89%를 설명하는 것으로 나타났다. 이 중 대상자의 필요 요인이 변이의 대부분인 86% 수준을 설명하였으며, 의료 공급 요인은 약 3% 수준의 추가적인 설명력을 나타냈다. 이는 국내 지역 간 의료비 차이의 상당 부분이 의학적 필요에 기인한 정당한 변이임을 의미한다. 요인별 영향력을 살펴보면 일반 병상 수와 요양 병상 수는 의료비와 양의 상관관계를 보인 반면 300병상 이상 종합병원의 존재와 의사 수의 증가는 의료비 감소와 연관된 것으로 나타났다. 이는 병상 공급의 증가가 의료 이용을 유발한다는 뢰머의 법칙을 지지하는 한편 양질의 병상과 일차 의료가 입원과 같은 고비용 의료 이용을 대체하거나 예방하는 효과가 있음을 시사한다.
필요 요인을 반영한 분석 결과 총 39개 지역에서 O/E ratio가 1표준편차(8.3%) 이상 높은 것으로 나타났다. 전라도는 의료비와 O/E ratio가 모두 높았다. 서울 및 수도권, 경상남도는 의료비에 비해서 O/E ratio가 높은 양상을 보였으며, 반대로 경상북도는 의료비는 높은 반면 O/E ratio는 중간 수준으로 나타났다. 이와 같은 결과는 지역별 의료비를 해석할 때 구성원의 의학적 필요를 반드시 함께 고려해야 함을 시사한다. 또한, 총 38개 지역에서 의료 공급 요인의 영향이 1표준편차를(3.9%) 초과하는 것으로 나타나 공급민감의료 현상이 시군구 수준에서도 작동하고 있음을 실증적으로 확인하였다. 전라도와 경상도에서 공급 요인 영향이 나타났으며, 서울 및 수도권, 충청도, 강원도는 공급 요인 영향이 크지 않은 것으로 나타났다. 공급 요인 영향이 나타나는 지역 중에서는 O/E ratio가 높지 않은 지역도 포함되어 전체 의료비가 기대 수준으로 나타나더라도 세부적인 영역에서 과도한 의료 이용이 존재할 수 있음을 밝혔다.
유형 분류 결과, 의료비와 O/E ratio가 모두 중간 수준인 유형 5 지역이 112개로 가장 많이 분류되었다. 이외에 의료비와 O/E ratio가 모두 높은 유형 1은 14개, 의료비는 중간 수준이지만 O/E ratio가 높은 유형 2는 27개 지역이 나타났다. 의료비와 O/E ratio가 모두 낮은 유형 9의 경우 3개 지역이 포함되었고, 의료비와 O/E ratio가 상이한 패턴을 보이는 유형 3과 유형 7에 대해서는 유형 7에만 1개의 지역이 포함되었다. 유형 1과 유형 2 지역에 대해서는 기대를 상회하는 의료비의 원인에 대한 탐색과 의료비 효율화가 필요하며, 유형 9의 경우 효과적 의료의 과소 이용을 주의할 필요가 있다. 유형 3과 유형 7의 경우 의료비와 O/E ratio가 상이한 패턴을 나타내는 원인을 탐색할 필요가 있다.
이 연구에서 개발한 모니터링 체계는 시군구별 의료비를 단순한 총량 수준이 아닌 의학적 필요를 반영한 상대적 기준으로 평가함으로써, 지역 간 의료비 비교의 공정성을 제고하고, 정책적 개입의 우선순위를 판단하는 근거를 제공한다. 의료비, O/E ratio, 의료 공급 요인 영향이라는 세 가지 차원의 복합적 분석 틀은 정책 담당자가 해당 지역의 의료비 현황과 원인을 직관적으로 파악할 수 있게 하며, 이는 의료 자원 배분의 효율성 제고와 지역 맞춤형 보건정책 수립에 실질적으로 활용될 수 있다.
이 연구는 학문적인 측면에서 Wennberg의 정당한 변이와 불필요한 변이 개념에 기반하여 국내 의료비의 지역 간 변이를 체계적으로 분석한 최초의 시도이며, 개인 수준 자료를 활용하여 필요 요인을 정교하게 반영함으로써 의료비 지역 간 변이의 상당 부분이 정당한 변이임을 밝혔다는 의미를 가진다. 정책적인 측면에서 기존의 접근 방식으로는 파악하기 어려웠던 지역 의료비 지출 양상을 유형화를 통해 이해하기 쉽도록 제시하는 실질적 용도를 가진다.
향후 이 모니터링 체계를 활용한 지역별 유형 분류가 활용되어 시군구 의료비 현황을 파악하고, 정책 개입의 효과를 평가하는 데 사용될 수 있기를 기대한다. 또한, 특정 질병군이나 시술, 인구집단을 대상으로 한 세분화된 분석, 정교한 방법론을 활용하여 인과성을 입증할 수 있는 연구, 간접 의료비를 포함한 전체 의료비에 대한 연구 등을 통해 지역 의료비에 대한 이해를 심화하고, 보다 정교한 정책적 함의를 도출할 수 있기를 기대한다.

This study aims to systematically analyze regional variations in healthcare expenditures and establish a monitoring system by accounting for the medical needs of healthcare users and the supply characteristics of regional healthcare resources. Regional variation in healthcare expenditure is a critical indicator that reveals issues regarding the efficiency of resource allocation and regional equity, beyond mere differences in spending magnitude. While major developed countries, such as the US and UK, have utilized systems like the Dartmouth Atlas or NHS Atlas to monitor unwarranted variations and improve inefficiencies, research in South Korea has been limited to specific time points or diseases without comprehensively considering both patient need and regional supply factors. Therefore, this study proposes a monitoring framework using the National Health Insurance Service-National Sample Cohort Database to identify the characteristics of healthcare expenditure by administrative district (Si, Gun, Gu), distinguishing between warranted and unwarranted variations.
We analyzed data from the NHIS Sample Cohort Database spanning four years (2016–2019), covering 246 administrative districts nationwide. The study population included all individuals maintaining National Health Insurance or Medical Aid eligibility, with the analysis unit being the individual per year. The analysis proceeded in four steps: 1) Calculating per capita average healthcare expenditure to identify regional variations; 2) Constructing an expenditure model to determine the influence of factors and their explanatory power on regional variation, categorized into individual-level need factors (demographic, socioeconomic, health) and regional-level supply factors (presence of general hospitals with more than 300 beds, number of beds, number of long-term care beds, number of doctors), utilizing Generalized Linear Models assuming a Tweedie distribution with a log link function; 3) Calculating the Observed Expenditure/Expected Expenditure ratio (O/E ratio) to evaluate relative expenditure levels and quantitatively analyze the impact of supply factors; and 4) Classifying districts into types based on expenditure, O/E ratio, and supply factor influence to derive policy implications.
The results revealed significant regional disparities, with up to a threefold difference in healthcare expenditure between the highest and lowest regions. The constructed model accounted for 89% of the regional variation, with need factors explaining the majority (86%) and supply factors adding approximately 3% of explanatory power. This suggests that a substantial portion of regional variation in South Korea is warranted variation attributable to medical needs. Regarding specific factors, the number of acute and long-term care beds showed a positive correlation with expenditure, whereas the presence of general hospitals with more than 300 beds and an increased number of doctors were associated with reduced expenditure. These findings support Roemer’s Law—that increased bed supply induces utilization—while also suggesting that quality hospital beds and primary care may substitute for or prevent high-cost medical services such as admission.
Analysis using the O/E ratio identified 39 regions with a ratio more than one standard deviation (8.3%) above the mean. While the Jeolla region showed high levels in both expenditure and O/E ratio, Seoul/Metropolitan areas and Gyeongsangnam-do exhibited high O/E ratios relative to their expenditure levels. Conversely, Gyeongsangbuk-do showed high expenditure but moderate O/E ratios. Supply-sensitive care was empirically confirmed in 38 regions where supply factors influenced expenditure by over 3.9%. Notably, some regions with significant supply factor influence did not exhibit high O/E ratios, implying that excessive utilization may exist in specific domains even if total expenditure appears expected.
Based on the typology, Type 5 (moderate both expenditure and O/E ratio) was the most common (112 districts). Type 1 (high both expenditure and O/E ratio) included 14 districts, and Type 2 (moderate expenditure, high O/E ratio) included 27 districts. Type 1 and Type 2 regions require investigation into the causes of excessive expenditure and efforts toward efficiency, while Type 9 (low both expenditure and O/E ratio) requires caution against the underutilization of effective care.
The monitoring system developed in this study evaluates regional healthcare expenditure not as a simple aggregate but as a relative level reflecting need factors, thereby enhancing the fairness of regional comparisons and providing a basis for prioritizing policy interventions.
The multi-dimensional analysis framework—comprising expenditure, O/E ratio, and supply factor influence—allows policymakers to intuitively grasp regional status and causes. This study is academically significant as the first in South Korea to systematically analyze regional variation using Wennberg’s concepts of warranted and unwarranted variation with individual-level data. Policy-wise, it provides a practical tool for visualizing complex expenditure patterns.
Future research utilizing this system is expected to deepen the understanding of regional healthcare through detailed analyses of specific diseases, causal inference methodologies, and indirect medical cost.
