    배경 및 목적
    아테졸리주맙-베바시주맙(Atezo-Bev) 병합요법에 대한 비반응군의 조기 예측은 진행성 간세포암에서 치료 성과를 최적화하는데 필수적이다. 본 연구는 치료 시작 2번째 항암 치료 후(약 1.5 개월)의 혈액학적 및 영상학적 변화를 통합하여 조기 반응 예측 모델을 개발하고 검증하는 것을 목표로 하였다.

    방법 및 결과
    2022년 7월부터 2025년 5월까지 단일 3차 의료기관에서 등록된 136명의 환자 중, 영상 평가가 가능하고 추적관찰이 완료된 55명을 전향적으로 분석하였다. 다변량 Cox 회귀 분석에서 기저 MoRAL 점수 (The Model for tumor Recurrence After living donor Liver transplantation (MoRAL = 11 × √[protein induced by vitamin K absence or antagonist-II (PIVKA-II)] + 2 × √[alpha-fetoprotein (AFP)]); HR=1.0003, P=0.05), MoRAL 점수 변화율(ΔMoRAL (%); HR=1.0042, P<0.001), 동맥기 조영 증강 변화율(ΔEnhancement (%); HR=1.0133, P=0.042) 이 독립적인 간세포암의 진행 예측인자로 확인되었다. 이 세 변수를 통합하여 MoRAL-ART score를 다음과 같이 정의하였다:
    MoRAL-ART=1 × MoRAL_baseline + 12 × ΔMoRAL (%) + 19 × ΔEnhancement (%).
    MoRAL-ART score는 우수한 성능을 보였으며 (C-index=0.692, area under the receiver operating characteristic curve=0.777), −226을 절단 값으로 적용했을 때, 고위험군은 저위험군 대비 질병 진행 위험이 3.6배 높았다 (P<0.001). 또한 조기 비반응의 임상적 유용성을 평가하기 위해 치료 3개월 이내의 조기 진행(Early progression)을 분석한 결과, –226 절단값에서 높은 민감도(0.86) 및 높은 음성예측도(0.96)를 보여 조기 비반응군을 효과적으로 배제할 수 있는 강점을 확인하였다. 이러한 결과는 치료 5주 후의 혈액학적 및 영상학적 변화가 조기 치료 반응을 반영함을 시사한다.

    결론
    MoRAL-ART는 Atezo–Bev 요법을 받는 진행성 간세포암 환자에서 조기 치료 반응을 예측할 수 있는 객관적이고 생물학적 기반의 새로운 도구이다. 혈액학적 지표와 영상학적 반응 지표를 통합함으로써, MoRAL-ART는 면역항암제 및 항혈관신생제의 이중 작용 기전을 반영하여 조기 비반응군을 식별하고 맞춤형 치료 전략 수립에 기여할 수 있다. 향후 대규모 다기관 연구를 통한 외부 검증을 통해 그 일반화 가능성과 임상적 활용성이 확인될 것으로 기대된다.

    Background and Aims
    Timely identification of non-responders to atezolizumab–bevacizumab (Atezo-Bev) therapy is critical for optimizing outcomes in advanced hepatocellular carcinoma (HCC). This study aimed to develop and validate an early predictive model based on laboratory and radiologic changes assessed after two cycles of Atezo–Bev therapy (approximately 1.5 months after treatment initiation).

    Approach and Results
    Among 136 patients prospectively enrolled at a single tertiary center between July 2022 and May 2025, 55 with evaluable radiologic data and adequate follow-up were analyzed. Multivariable Cox regression identified three predictors of disease progression: The Model for tumor Recurrence After living donor Liver transplantation (MoRAL = 11 × √[protein induced by vitamin K absence or antagonist-II (PIVKA-II)] + 2 × √[alpha-fetoprotein (AFP)]) at baseline (hazard ratio [HR]=1.0003, P=0.05), rate of change in MoRAL (ΔMoRAL (%); HR=1.0042, P<0.001), and rate of change in arterial-phase enhancement measured in Hounsfield units (ΔEnhancement (%); HR=1.0133, P=0.04). These variables were integrated into a composite model, the MoRAL-ART score, defined as follows: MoRAL-ART = 1 × MoRALbaseline + 12 × ΔMoRAL (%) + 19 × ΔEnhancement (%). A higher MoRAL-ART score was associated with an increased risk of non-response (HR=1.0004, 95% confidence interval=1.0002–1.0005, P<0.0001).
    MoRAL-ART showed favorable discriminatory performance (area under the receiver operating characteristic curve=0.777, C-index=0.692). When the high-risk group was defined as patients with a MoRAL-ART score≥−226, this group exhibited a 3.6-fold greater risk of disease progression than the low-risk group (MoRAL-ART score<−226) (P<0.001). At the same cutoff, the model demonstrated high sensitivity (0.86) and a high negative predictive value (0.96) for identifying early non-responders who experienced disease progression within 3 months.

    Conclusions
    MoRAL-ART score serves as an objective, non-invasive tool for the early clinical prediction of therapeutic response to Atezo-Bev in advanced HCC. By facilitating the early stratification of non-responders, this model supports clinical decision-making and guides timely treatment modifications. Multicenter external validation is warranted to confirm its generalizability and establish its potential for clinical implementation.
