배경 및 목적
본 연구는 성인 ADHD의 표현형의 발현 경향을 성별에 따라 구분하고 이를 불안, 우울, 충동성 등 다른 심리검사와의 양상을 쌍둥이 코호트 자료를 통해 비교함으로써 유전적 요인과 환경적 요인의 기여도를 분석하고자 하였다.
방법
1031명의 쌍둥이와 그의 가족들의 설문지 응답을 점수화하여 ADHD, 상태-특성 불안, 우울, 충동성을 연속형 변수로 환산하였다.
결과
ADHD와 불안, 충동성, 우울은 성별에 따라 유의미한 연관을 보인다. 남성의 경우 ADHD와 다른 심리검사 결과와 유의미한 유전적요인에 의한 연관을 보이지 않았다. 그러나 ADHD와 우울 및 충동 사이에서는 유의미한 환경적 요인에 의한 상관을 보였다. 여성의 경우 ADHD와 우울, 충동성, 특성 불안 심리검사와 유의미한 유전적 상관관계를 보였으며 그보다 덜하지만 ADHD와 우울 및 충동성 사이에 유의미한 환경적 요인에 의한 상관을 보였다.
결론
ADHD 성인과 청소년 스크리닝 단계에서 우울과 특성불안은 공통 유전적 요인 관점에서 분석해야 하며 불안 특성 중 일시적인 상태 불안은 ADHD와 병리적으로 분리된 대상으로 처리해야 한다는 결론이 가능하다. BID-II의 충동성 척도는 ADHD 증상을 잘 반영하지만 동일한 유전적 원인으로 접근하지 말고 개인환경요인이나 응답 방식 등 행동 및 성격 단계에서 동일한 양상을 보인다는 결론이 가능하다. 성별에 따라 ADHD와 우울 및 충동성은 유전적요인에 의한 상관을 다르게 보일 수 있다는 점에서 성별에 따라 ADHD와 우울 및 충동성의 유전적 요인이 다를 가능성을 보여준다.

Background and Objectives:
This study aimed to characterize the manifestation patterns of adult Attention-Deficit/Hyperactivity Disorder (ADHD) by sex and to examine how these patterns relate to anxiety, depression, and impulsivity. Using a twin cohort, we further investigated the extent to which genetic and environmental factors contribute to the associations between ADHD symptom dimensions and internalizing/behavioral traits.
Methods:
Questionnaire data from 1,031 twins and their family members were quantified and converted into continuous measures of ADHD symptoms, state and trait anxiety, depressive symptoms, and BIS-11–based impulsivity traits. Phenotypic correlations and cross-twin cross-trait (CTCT) analyses were conducted separately by zygosity to infer the relative contributions of shared genetic and environmental influences.
Results:
Significant sex differences were observed in the associations among ADHD, anxiety, impulsivity, and depression. In males, no significant genetic correlations were found between ADHD and other psychological traits. However, significant environmental correlations were detected between ADHD and both depression and impulsivity. In contrast, females showed significant genetic correlations of ADHD with depression, impulsivity, and trait anxiety. Significant environmental correlations were also observed between ADHD and depression/impulsivity, although the genetic correlations were comparatively stronger.
Conclusions:
These findings show the importance of evaluating depression and trait anxiety during adult and adolescent ADHD screening, though these internalizing traits share underlying genetic liabilities with ADHD. In contrast, transient state anxiety appears etiologically distinct and should be considered separately in clinical and research settings. Although BIS-11 impulsivity correlates strongly with ADHD symptoms, it should not be interpreted as reflecting an identical genetic cause; instead, similarities likely arise from individual-specific environmental influences, response styles, or broader behavioral tendencies. These findings suggest that sex specific differences in the shared genetic architecture of ADHD, depression and impulsivity.
