    본 연구는 오늘날 건강이 사회적 중심에 놓이는 현상에 주목하며, 건강이 개인의 일상과 삶의 태도를 형성하고 사회적 규범의 핵심으로 기능하고 있다는 관찰에서 출발한다. 기존의 보건학 연구는 건강을 주로 정책의 달성 목표로 간주하여, 행태 변화의 결과 혹은 측정 가능한 지표로 활용하였으나, 과연 한국 사회에서 건강이란 어떤 의미인지, 즉 건강의 개념 그 자체에 천착하여 이를 밝혀내는 작업은 상대적으로 주목받지 못하였다. 오늘날 건강 개념을 탐구하는 일은 보건학적으로 여러 차원에서 중요한 의의를 지닌다. 건강 개념은 보건정책과 제도의 토대로서 문제 정의 방식, 개입의 범위, 책임의 귀속을 결정한다. 또한 개인이 의료 서비스를 이용하고 예방과 치료, 방임과 자기관리 사이에서 선택하는 일상적 판단과 행동, 즉 보건학적 개입 지점이 되는 실천 역시 건강에 대한 이해에서 비롯된다. 이처럼 건강 개념의 파악은 보건학적으로 매우 중요한 과업임에도 불구하고 이를 위한 실증적 작업은 충분히 이루어지지 않았다.
    본격적으로 건강 개념에 대한 탐구에 앞서, 본 연구에서는 건강 개념에 대한 층위를 구분하였다. 건강 개념은 개인의 인식과 사회에서의 통념이 상호작용하며 형성되는 다층적인 구조를 가지고 있는 사회적 산물이라고 보기 때문이다. 구체적으로, 개인이 의식적으로 인지하고 표현하는 표면적 층위와 인식의 기저에 있는 심연적 층위로 나눌 수 있는데, 후자와 같이 내면화된 인지 구조이자 사회적 합의에 해당하는 건강 개념은 언어에서 드러나는 인식적 차원의 내용을 심층 분석함으로써 파악될 수 있다고 판단하였다.
    진공 상태에서 건강 개념의 구조와 변화를 분석하는 방식보다는 기존에 제시된 보건학 및 보건사회학 이론을 검토하여 분석 결과의 이론적 정합성을 고려하였다. 특히, 생의료화, 감시의학, 건강주의 및 사회적 조건에 대한 이론을 중심으로 하여 이론적 틀을 기술, 위험, 규범의 차원으로 구분하였다. 생의료화 이론은 건강 개념이 기술과학적으로 재편되고 확장되는 과정을 설명하기 위해 채택하였다. 이는 한국 사회에서 생명과학과 정보기술이 의료 영역과 융합하며 건강이 적극적으로 설계하고 관리해야 할 기술적 프로젝트로 변모하는 현상을 설명하기에 적절하다고 판단하였다. 감시의학은 위험의 조기 탐지와 질병이 없는 상태에서조차 지속적인 관찰과 모니터링을 요구하는 등 위험 관리의 영역으로 확장되는 과정을 설명한다. 특히 디지털 기술의 발달은 일상적 위험 관리를 촉진하는 배경이 되고 있다. 건강주의는 건강이 개인의 도덕적 의무이자 책임으로 내면화되는 사회적 분위기를 설명하기에 적절한 이론으로, 기술과 위험의 논리가 결국 건강 관리 주체를 사회나 제도로부터 개인으로 옮겨오게 한다. 특히 한국 사회는 압축적 근대화와 디지털 기술의 발달, 정부 주도의 건강검진 제도를 비롯한 정책적 분위기가 결합하여 위 세 가지 기제가 중첩적으로 작동하는 특성을 보인다.
    본 연구에서는 하나의 연구 질문, 즉 한국 사회의 건강 개념을 확인하기 위해 서로 다른 분석 수준을 설정하였다. 특히 텍스트를 중심으로 한국 사회에서 건강 개념이 어떻게 형성되고 변화했는지 건강 인식 차원을 밝히고 궁극적으로 건강 개념이라는 추상적인 구조에 다다르고자 하였다. 대규모 텍스트는 개별 사례를 넘어 사회적 의미 구조를 포착할 수 있기 때문에 언어적 규범과 반복되는 패턴을 발견하기에 적절한데, 특히 언론 기사와 같은 공적 텍스트는 사회적으로 승인된 의미 구조를 반영하며, 이는 개인이 건강을 인식할 때 참조하는 사회적 사실을 드러낸다. 또한, 대규모 텍스트에 존재하는 언어적 패턴은 사람들의 인지적 유사도와 상당 부분 일치하므로, 텍스트 분석을 통해 건강을 인지하는 방식을 보여주기에 적합하다. 오랜 시간 축적된 대규모 텍스트는 건강 인식의 동적 변화를 통해 시대적 맥락에 따른 변동을 파악하여 거시적으로 조망할 수 있을 것으로 기대하였다.
    이상의 대규모 텍스트의 이점을 활용하여 먼저 언론 기사에서 나타나는 건강과 관련된 주제 구조의 지형과 시간에 따른 주제 비중의 변화를 구조적 토픽모델로 분석하였다. 1994년부터 2024년까지 국내 주요 언론사의 건강 관련 기사 691,320건을 분석하여 주제의 구조적 분포, 토픽 네트워크 구조, 시기별 비중의 변화를 분석하였다. 최적의 토픽 수 60개 중 연구 주제와 밀접한 16개 핵심 토픽을 선별하여 취약 집단의 돌봄과 지원, 생활습관과 건강 위험 인식, 치료적 개입과 의료 공간, 보건정책과 제도적 관리, 생명과학과 디지털 기술 적용, 국제적 보건 문제와 외연 확장 등 6개 유목으로 재구성하였다. 토픽 간의 네트워크 구조에서 중심부에는 정책, 제도, 기술 관련 토픽들이 위치하여 주로 제도적 관리의 대상으로 건강이 의미화되고 있음을 확인하였다. 시기적 변화를 살펴 보면, 한국 사회에서 건강에 대한 관심사가 치료와 제도에서 생활습관과 예방, 이후 디지털 기술과 돌봄으로 이동하였다.
    다음으로, 개별 개념들 사이의 구체적인 의미적 관계의 변화를 더욱 정밀하게 추적하고 단어 간 관계의 밀접성을 분석하였다. 단어의 의미는 함께 쓰이는 주변 단어들로부터 알 수 있다는 분포 가설의 전제를 따르는 워드임베딩(Word2Vec) 방법을 사용하였고, 3,785,806건의 기사를 코퍼스로 하여 한국어 학습 방법에 적합하다고 알려진 Skip-gram 학습 방식으로 단어를 벡터화하였다. 분석의 초점을 명확하게 하기 위해 세 가지 의미축을 설정하였다. 먼저, 건강 개입 방식이 적극적으로 설계하고 관리하는 프로젝트로의 전환을 확인하기 위해 주요 질병에 대한 예방-치료 중심축에서의 위치 이동을 확인하였다. 전체적인 경향은 치료 중심으로의 회귀였다. 기술의 발전은 암과 같은 질환에 대한 치료 가능성의 기대를 높였으며, 예방이 강조되는 당뇨, 고혈압, 고지혈증과 같은 만성질환 역시 언어적으로는 치료 중심 구조가 우세했다. 감염병은 위기 시에는 예방이 강조되나 안정화 되면서 다시 임상적 관리가 일상화되었다. 이로서 예방은 독립적 실천으로 여겨지기보다는 치료적 연장선상에 있는 상시적 의료 관리 체계로 이해되고 있음을 확인하였다. 다음으로, 위험 관리와 실천 양식의 변화를 포착하기 위해 일상적 감시 축을 설정하였다. 감시, 관찰, 모니터링, 추적, 확인 등 일상적인 감시를 의미하는 단어들과 다른 단어와 맺는 의미적 거리를 시계열적으로 분석한 결과, 주로 신체적 질병이나 제도적 관리에 한정되었던 감시 관련 용어가 유전적 생물학, 생활습관, 정신적 상태로 대상이 확장된 것으로 나타났다. 관리 방식은 유전적 선천적 특성의 관리에서 데이터 기반 관리로 전환되었는데, 운명적인 소인을 확인하는 것보다 매일의 데이터 관리가 일상화 되었다. 이는 감시의 의미가 위계적인 통제에서 제도와 기술이 개인의 자기 점검 행위로 내면화된 자발적인 자기관리의 윤리로 전환되었음을 의미한다. 마지막으로 개인 책임 축을 설정하여 건강의 규범화와 도덕적 내면화 경향을 확인하였다. 1990년대에는 외부적 규범이 강하게 작동하였으나, 시간이 갈수록 내면적 책임이 강화되는 것으로 나타났다. 의무, 책임, 양심과 같이 내면적 가치의 언어들이 2010년대 이후 다시 건강과 강하게 결합하는 양상을 보였다. 또한, 자기관리는 줄곧 건강과 높은 의미적 관계를 유지하며 절대적인 위상을 보여주었으며, 의지와 노력, 성실과 같은 태도 관련 단어가 높은 순위를 유지하여 타고난 체질보다는 자기계발의 과제로 건강이 인식되고 있음을 보여주었다. 또한, 죄책감, 후회와 같은 부정적 감정의 단어들이 지속적으로 건강과 높은 연관성을 보여, 건강 관리의 실패가 도덕적 자책으로 이어짐을 시사하였다. 낙인이라는 단어는 최근 급격히 건강과의 유사도가 증가하였다. 이는 건강하지 못한 상태가 자기관리 실패의 증거로 간주되어 사회적 비난의 대상이 되는 경향을 반영하고 있음을 보여주었다.
    이와 더불어, 주관적 건강의 판단 근거를 질문한 개방형 설문조사 문항을 활용하여 거시적인 언론 기사 분석만으로는 포착하기 어려운 개인의 구체적인 인식과 판단 과정을 확인하였다. 건강 개념이 미시적으로 작동하는 원리를 규명하기 위함이었는데, 이는 주관적 건강 응답이 단순히 건강 상태의 지표로만 활용할 것이 아니라 응답 과정에서 개인이 ‘건강이란 무엇인가’라는 스키마가 작동하는 과정이 수반된다고 보았기 때문이다. 이를 위해 본 연구는 1,000명을 대상으로 실시한 설문조사 결과를 활용하였으며, 불성실한 응답을 제외하고 988개 응답을 분석하였다. 먼저, 응답을 질적 코딩을 통해 범주화 하여 15개 코딩 범주를 형성하였는데, 건강행동, 신체적 질병 경험, 신체적 감각과 기능, 건강검진 결과, 심리적 상태나 성격 등이 포함되었다. 응답이 상대적으로 간결하다는 점을 고려하여 Biterm 토픽모델링으로 토픽을 확인하였는데, 일상적 자기관리, 신체 감각과 경험, 의료적 진단과 관리 등 세 토픽이 도출되었다. 응답한 건강 수준에 따라 판단 기준이 달라졌는데, 건강하다고 느끼는 집단은 자기관리와 실천을 주로 사용하여 노력하고 성취하는 대상으로 보는 반면, 건강하지 않다고 느끼는 집단은 의학적 진단과 고통을 중심으로 응답하여 통제 불가능한 제약 조건으로 인식하는 경향을 보였다. 이를 통해 거시적 언론 기사에서 나타난 결과와 개인 인식 사이의 모순과 일치를 확인하였는데, 자기관리 담론의 확산에도 불구하고 치료 중심성이 약화되지 않는 측면이나 개인 책임 담론이 강화되는 동시에 취약 집단에 대한 돌봄 담론이 확대되는 측면이 한국 사회의 건강 개념이 지닌 구조적 이중성을 드러냈다.
    이상의 대규모 텍스트와 설문조사 응답을 통한 건강 인식 분석 결과를 바탕으로 한국 사회의 건강 개념을 도출하였다. 무엇을 건강으로 간주하는지를 의미하는 건강의 기준, 건강을 어떻게 달성하고 유지하는지를 설명하는 건강 실천의 방식, 건강을 어떤 가치로 대하는지 알 수 있는 건강에 대한 태도라는 세 가지 차원이 동시에 작동하는 복합적인 건강 개념 틀을 제시하였다. 건강의 기준 측면에서 한국 사회는 이중적인 기준 구조를 가지고 있다고 평가된다. 즉, 건강 관리의 범위가 확대되었으며 개입 시점도 잠재적 위험 단계로 앞당겨졌음에도 불구하고, 여전히 의학적 개입과 치료 가능성에 맞춰 있다. 그리고 건강검진 결과와 수치, 의사의 소견과 같은 객관적 데이터를 건강의 확실한 증거로 삼고 수치로 끊임없이 증명하는 것을 기준으로 삼는다. 다음으로, 건강은 기술 기반의 상시적 자기감시 방식으로 실천되고 있다. 국가 주도의 제도적 관리에서 개인의 자율적 관리로 전환되면서 일상 속에서 자신의 상태를 지속적으로 기록하고 추적하는 감시가 내면화되고, 꾸준히 성실하게 수행해야 하는 자기 규율의 성격을 가지게 되었다. 마지막으로 건강은 개인의 성실성을 증명하는 도덕적 가치이자 의무로 자리 잡았고, 건강하지 못한 상태를 자기관리의 실패와 나태함, 무절제의 결과로 해석하여 사회적 낙인을 유발할 뿐만 아니라, 개인에게 죄책감, 수치심, 후회와 같은 부정적 정서를 일으킨다고 해석된다.
    본 연구는 한국 사회가 건강을 가장 중시하는 동시에 가장 불안해하는 사회라는 모순적 상황에 놓여 있음을 진단하며, 개인 책임 중심의 건강 담론이 낳는 한계를 실천적 함의로 제시한다. 한국 사회에서 건강은 도덕적 의무로 내면화되었고, 건강하지 못한 상태는 개인의 실패나 도덕적 결함으로 해석되는 경향이 강화되어 왔다. 그 결과, 건강을 추구할수록 오히려 건강에 대한 불안과 결핍감이 증폭되는 역설적인 구조가 형성되었다. 특히 현재의 건강 담론은 건강 문제를 개인의 의지와 라이프스타일 선택의 결과로 환원하지만, 실제 건강 실천은 소득 수준, 노동 조건, 주거 환경과 같은 구조적 제약에 의해 크게 좌우된다. 이러한 조건을 외면한 채 개인의 노력만을 강조하는 접근은 건강 불평등을 심화시키고, 취약 계층에게 좌절과 자기비난을 전가하는 결과를 낳는다. 따라서 보건학적 실천은 개인에게 더 많은 자기관리를 요구하는 방식에서 벗어나, 건강을 둘러싼 사회적 긴장과 불안을 완화하고 건강을 성과나 책임이 아닌 돌봄과 연대의 관계 속에서 재구성하는 방향으로 전환될 필요가 있다.
    아울러 본 연구는 건강을 자명한 전제가 아닌 분석의 대상으로 삼음으로써, 한국 사회 고유의 건강 개념이 어떻게 구조화되어 있는지를 경험적으로 규명하여 보건학 이론의 확장을 시도하였다. 분석 결과, 한국의 건강 개념은 ‘건강의 기준’, ‘실천 방식’, ‘태도’라는 세 차원이 결합된 구조로 작동하고 있음이 드러났다. 또한 주관적 건강은 사회적으로 공유된 건강 개념이 개인의 판단 속에 반영된 결과로 재해석될 수 있음을 보여주었다. 더 나아가 본 연구는 생의료화, 감시의학, 건강주의와 같은 서구 이론을 압축적 근대화, 국가 주도 제도, 집단주의 문화라는 한국적 맥락에서 비판적으로 재해석함으로써, 한국 사회에서는 개인의 자율적 선택보다 제도적 규율과 사회적 기대가 중첩된 형태로 자기관리가 구성된다는 점을 밝혔다. 대규모 텍스트 분석과 설문조사를 결합한 다층적 방법론 역시 추상적인 건강 개념을 실증적으로 규명할 수 있는 분석 가능성을 제시한다는 점에서 중요한 의의를 지닌다.
    끝으로 본 연구는 보건정책이 단순히 건강 행동을 촉진하는 수단을 넘어, 개인의 정서와 주체성 형성에 깊이 개입하는 장치임을 지적하며 정책적 전환의 필요성을 제기한다. 1995년 국민건강증진법 제정 이후 국가는 국민을 스스로 건강을 관리해야 하는 주체로 규정하며 건강 관리의 책임을 개인에게 이양해 왔고, 이는 소비사회로의 전환과 맞물려 건강을 개인의 경쟁력 자산으로 인식하게 만들었다. 예방 중심 정책은 표면적으로 자율을 강조하지만, 실제로는 상시적인 자기 점검과 자기 감시를 요구하는 초자아적 감시 기제로 작동하면서 건강 실천을 선택이 아닌 의무로 경험하게 한다. 특히 디지털 헬스케어의 확산은 건강을 지속적으로 측정하고 데이터화함으로써 새로운 정상성과 규범을 생산하고, 사회적 조건을 고려하지 않은 개입은 관리 가능한 사람과 그렇지 못한 사람을 구분하여 건강 격차를 심화시킬 위험을 내포한다. 따라서 향후 보건정책은 더 많은 관리와 통제를 요구하는 방향이 아니라, 건강 규범이 개인에게 과도한 정서적 부담과 책임을 지우지 않도록 설계되어야 하며, 개인의 행동 교정보다 건강을 수행할 수 있는 사회적 조건을 조성하고 불안을 완화하는 공적인 실천으로 방향성을 설정할 필요가 있다.

    This study begins with the observation that health has become a central organizing principle in contemporary society, shaping everyday life, individual orientations toward living, and core social norms. While public health research has largely treated health as a policy goal, a behavioral outcome, or a measurable indicator, relatively little attention has been paid to the question of what health itself means in Korean society. Yet examining the concept of health is of fundamental importance for public health, as it defines how problems are framed, how far interventions extend, and where responsibility is assigned. Individual decisions regarding medical service use, choices between prevention and treatment, as well as neglect and self-care, also arise from underlying understandings of health. Despite its significance, empirical research that directly interrogates the concept of health has remained limited.
    Prior to an in-depth analysis, this study conceptualizes health as a multilayered social construct formed through the interaction between individual perceptions and socially shared understandings. It distinguishes between a surface level that individuals consciously recognize and articulate, and an underlying level that consists of internalized cognitive structures and social shared consensus. The latter, which operates beneath explicit awareness, is examined through in-depth analysis of semantic patterns that reveal implicit modes of recognition in textual data.
    Rather than analyzing changes in the health concept in the absence of theoretical context, this study situates its findings within established theories in public health and health sociology to ensure theoretical coherence. Drawing on theories of biomedicalization, surveillance medicine, healthism, and social conditions, the analytical framework is organized along the dimensions of technology, risk, and norms. Biomedicalization is used to explain how health has been reconfigured and expanded through technoscientific developments, particularly in the Korean context where life sciences and information technologies have merged with medicine, transforming health into a technical project that must be actively designed and managed. Surveillance medicine accounts for the expansion of health into a domain of risk management that requires continuous observation and monitoring prior to the onset of disease, a process further intensified by digital technologies. Healthism explains how health has become internalized as a moral obligation and individual responsibility, shifting the locus of health management from institutions to individuals. In Korea, these mechanisms operate simultaneously and in overlapping ways due to compressed modernization, rapid digitalization, and state-led health screening policies.
    To identify the health concept in Korean society, this study adopts multiple levels of analysis centered on text. Large-scale text data allow for the detection of social meaning structures beyond individual cases, making it possible to identify shared meanings and recurring patterns. Public texts such as news articles reflect socially sanctioned meanings and reveal the shared frames of reference individuals draw upon when perceiving health. Because semantic patterns in large corpora have been shown to correspond closely to cognitive similarity, text analysis provides a suitable means for examining how health is perceived. Long-term accumulated texts further enable a macro-level examination of dynamic changes in health perceptions across historical contexts.
    Using these advantages, the study first analyzes the thematic structure of health-related discourse in news media and changes in topic prevalence over time. A total of 691,320 health-related news articles published by major Korean media outlets between 1994 and 2024 were analyzed using structural topic models to examine topic distributions, network structures among topics, and temporal shifts. From an optimal model of 60 topics, 16 core topics closely related to the research focus were selected and reorganized into six thematic clusters: care and support for vulnerable groups, lifestyle and health risk perception, therapeutic interventions and medical spaces, health policy and institutional management, applications of life sciences and digital technologies, and global health issues and outward expansion. Network analysis showed that policy, institutional, and technology-related topics occupied central positions, indicating that health is primarily framed as an object of institutional management. Over time, societal attention shifted from treatment and institutions toward lifestyle and prevention, and later toward digital technologies and care.
    Next, the study traced changes in specific semantic relationships between concepts using word embedding techniques based on the distributional hypothesis that word meaning is shaped by surrounding words. Using a corpus of 3,785,806 news articles, words were vectorized with a skip-gram Word2Vec model suited to Korean language data. Three semantic axes were constructed. First, to examine the transformation of health interventions into actively designed and managed projects, the study analyzed shifts along a prevention-treatment axis for major diseases. Overall, a return toward treatment-centered structures was observed. Technological advances increased expectations of treatability for diseases such as cancer, and even chronic conditions typically associated with prevention remained linguistically dominated by treatment-oriented patterns. Infectious diseases emphasized prevention during crises but reverted to routine clinical management once stabilized, suggesting that prevention is understood less as an independent practice and more as an extension of continuous medical management. Second, an everyday surveillance axis was used to capture changes in risk management practices. Over time, surveillance-related terms expanded from physical disease and institutional control to include genetics, lifestyle, and mental states. Management shifted from identifying innate predispositions to routine data-based self-monitoring, indicating a transformation from hierarchical control to an internalized ethic of voluntary self-management. Third, an individual responsibility axis revealed increasing moralization of health. While external norms dominated in the 1990s, internal responsibility strengthened over time. Terms related to obligation, responsibility, and conscience became strongly associated with health after the 2010s. Self-care consistently maintained a central semantic position, and attitude-related terms such as will, effort, and diligence ranked highly, indicating that health is perceived as a task of self-improvement rather than innate constitution. Persistent associations between health and negative emotions such as guilt and regret suggest that failure in health management leads to moral self-blame, while the rapid rise in similarity between health and stigma reflects growing tendencies to socially blame unhealthy states.
    To complement the macro-level media analysis, the study also examined individual-level perceptions using open-ended survey responses on subjective health judgment. Based on responses from 988 participants, qualitative coding identified 15 categories, including health behaviors, disease experiences, bodily sensations, health screening results, and psychological states. Biterm topic modeling revealed three main topics: everyday self-management, bodily sensations and experiences, and medical diagnosis and management. Perceived health status influenced judgment criteria, with healthier respondents emphasizing effort and practice, while less healthy respondents focused on medical diagnoses and uncontrollable constraints. These findings revealed both convergence and tension between media discourse and individual perceptions, highlighting a structural duality in the Korean health concept where treatment-centered logic persists alongside expanding self-responsibility and care discourses.
    Based on these analyses, the study proposes a composite framework of the health concept consisting of three interrelated dimensions: criteria of health, modes of health practice, and attitudes toward health. In Korea, health criteria exhibit a dual structure in which the scope of health management expands and intervention points move earlier into risk stages, yet legitimacy remains anchored in medical intervention and treatability, relying heavily on objective data such as screening results and clinical evaluations. Health practices take the form of technology-based continuous self-surveillance, internalized as disciplined self-regulation. Health attitudes frame health as a moral value and obligation, where unhealthy states are interpreted as failures of self-management, generating stigma as well as guilt, shame, and regret.
    This study diagnoses a paradoxical condition in which Korean society both prioritizes health and remains deeply anxious about it. Individual responsibility-centered health discourse has intensified health-related anxiety by framing health as a moral duty and unhealthy states as personal failure, despite the fact that health practices are strongly constrained by structural factors such as income, labor conditions, and housing environments. Emphasizing individual effort while ignoring these conditions risks deepening health inequalities and transferring frustration and self-blame onto vulnerable groups. Public health practice should therefore shift away from demanding more self-management and toward alleviating social tension and anxiety around health, reconstructing health within relationships of care and solidarity rather than performance and responsibility.
    By treating health not as a self-evident premise but as an object of analysis, this study empirically elucidates how the Korean health concept is structured, contributing to the expansion of public health theory. The findings demonstrate that the Korean health concept operates through the combined dimensions of criteria, practice, and attitude. Self-related health is reinterpreted as an outcome shaped by socially shared health concepts embedded in individual judgment. Furthermore, by critically reinterpreting Western theories such as biomedicalization, surveillance medicine, and healthism within the Korean context of compressed modernization, state-led governance, and collectivist culture, the study shows that self-management in Korea is constructed through overlapping institutional discipline and social expectations rather than purely autonomous choice. The integration of large-scale text analysis and survey data also demonstrates a viable methodological approach for empirically analyzing abstract health concepts.
    Finally, the study argues that health policy functions not only as a tool for promoting health behaviors but also as a mechanism that deeply intervenes into emotional experiences and processes of self-understanding. Since the enactment of the National Health Promotion Act in 1995, the Korean state has increasingly positioned citizens as responsible self-managers of health, aligning health with individual competitiveness in a consumer society. While prevention-oriented policies emphasize autonomy, they operate in practice as mechanisms of constant self-monitoring and self-surveillance, transforming health practices into obligations rather than choices. The expansion of digital healthcare further produces new norms by continuously measuring and datafying health, while interventions that overlook social conditions risk exacerbating health disparities by distinguishing between those who are manageable and those who are not. Future directions for health policy should therefore shift away from expanded control and toward policy designs that reduce emotional burdens and moral pressure on individuals, prioritizing the creation of social conditions that enable health and alleviate anxiety rather than the correction of individual behavior.

Based on responses from 988 participants, qualitative coding identified 15 categories, including health behaviors, disease experiences, bodily sensations, health screening results, and psychological states. Biterm topic modeling revealed three main topics: everyday self-management, bodily sensations and experiences, and medical diagnosis and management. Perceived health status influenced judgment criteria, with healthier respondents emphasizing effort and practice, while less healthy respondents focused on medical diagnoses and uncontrollable constraints. These findings revealed both convergence and tension between media discourse and individual perceptions, highlighting a structural duality in the Korean health concept where treatment-centered logic persists alongside expanding self-responsibility and care discourses.
    Based on these analyses, the study proposes a composite framework of the health concept consisting of three interrelated dimensions: criteria of health, modes of health practice, and attitudes toward health. In Korea, health criteria exhibit a dual structure in which the scope of health management expands and intervention points move earlier into risk stages, yet legitimacy remains anchored in medical intervention and treatability, relying heavily on objective data such as screening results and clinical evaluations. Health practices take the form of technology-based continuous self-surveillance, internalized as disciplined self-regulation. Health attitudes frame health as a moral value and obligation, where unhealthy states are interpreted as failures of self-management, generating stigma as well as guilt, shame, and regret.
    This study diagnoses a paradoxical condition in which Korean society both prioritizes health and remains deeply anxious about it. Individual responsibility-centered health discourse has intensified health-related anxiety by framing health as a moral duty and unhealthy states as personal failure, despite the fact that health practices are strongly constrained by structural factors such as income, labor conditions, and housing environments. Emphasizing individual effort while ignoring these conditions risks deepening health inequalities and transferring frustration and self-blame onto vulnerable groups. Public health practice should therefore shift away from demanding more self-management and toward alleviating social tension and anxiety around health, reconstructing health within relationships of care and solidarity rather than performance and responsibility.
    By treating health not as a self-evident premise but as an object of analysis, this study empirically elucidates how the Korean health concept is structured, contributing to the expansion of public health theory. The findings demonstrate that the Korean health concept operates through the combined dimensions of criteria, practice, and attitude. Self-related health is reinterpreted as an outcome shaped by socially shared health concepts embedded in individual judgment. Furthermore, by critically reinterpreting Western theories such as biomedicalization, surveillance medicine, and healthism within the Korean context of compressed modernization, state-led governance, and collectivist culture, the study shows that self-management in Korea is constructed through overlapping institutional discipline and social expectations rather than purely autonomous choice. The integration of large-scale text analysis and survey data also demonstrates a viable methodological approach for empirically analyzing abstract health concepts.
    Finally, the study argues that health policy functions not only as a tool for promoting health behaviors but also as a mechanism that deeply intervenes into emotional experiences and processes of self-understanding. Since the enactment of the National Health Promotion Act in 1995, the Korean state has increasingly positioned citizens as responsible self-managers of health, aligning health with individual competitiveness in a consumer society. While prevention-oriented policies emphasize autonomy, they operate in practice as mechanisms of constant self-monitoring and self-surveillance, transforming health practices into obligations rather than choices. The expansion of digital healthcare further produces new norms by continuously measuring and datafying health, while interventions that overlook social conditions risk exacerbating health disparities by distinguishing between those who are manageable and those who are not. Future directions for health policy should therefore shift away from expanded control and toward policy designs that reduce emotional burdens and moral pressure on individuals, prioritizing the creation of social conditions that enable health and alleviate anxiety rather than the correction of individual behavior.
