Graduate students are continuously exposed to academic and research-related stress, which is closely associated with sleep disturbances and emotional difficulties. The purpose of this study was to compare and examine the effects of an 8-week Pilates intervention alone and a Pilates intervention combined with relaxation-inducing music on sleep, emotional outcomes, and autonomic nervous system indicators in graduate students.
This study employed a single-blind randomized controlled trial design. A total of 59 participants were randomly assigned to a control group (CG), a Pilates group (PG), or a Pilates plus music group (PMG). The intervention was conducted twice per week for 60 minutes per session over 8 weeks. In the PMG, slow-tempo relaxation-inducing music was applied only during the warm-up and cool-down phases. Fifty-three participants completed the intervention and were included in the primary outcome analyses, and 44 participants with valid signal quality were included in the heart rate variability (HRV) analyses.
Pre- and post-intervention outcome measures included subjective sleep (Insomnia Severity Index [ISI], Pittsburgh Sleep Quality Index [PSQI]), emotional status (Depression Anxiety Stress Scales–21 [DASS-21]), and autonomic nervous system function assessed by HRV. In addition, app-based daily sleep data were collected on weekdays throughout the 8-week intervention period. Repeated-measures ANCOVA revealed significant main effects of time for ISI, PSQI, and DASS-21 (all p < .001). A significant Time × Group interaction was observed for ISI (F(2,49) = 6.36, p = .003), indicating that both the Pilates group and the Pilates plus music group showed greater reductions in insomnia severity compared with the control group, whereas no significant difference was found between the two intervention groups. For HRV outcomes, only main effects of time were significant (p < .001), indicating overall pre–post improvements without significant between-group differences.
Analysis of app-based daily sleep data, dichotomized into early and late intervention periods, revealed significant Group × Period interactions for sleep efficiency, sleep cycle duration, and sleep cycle count (p < .05). Notably, the Pilates plus music group demonstrated a significantly greater increase in sleep efficiency during the late intervention period compared with the control group (p = .030).
In conclusion, an 8-week Pilates intervention was confirmed as an effective non-pharmacological approach for reducing insomnia symptoms in graduate students. Although the addition of relaxation-inducing music did not yield distinct benefits on subjective sleep and emotional questionnaires, it was associated with improvements in sleep efficiency during the later phase of the intervention, suggesting a potential gradual or supportive effect.
