고정익 항공기(fixed-wing aircraft)를 이용한 중환자 이송에서 고도변화, 소음, 진동 등의 변수들은 중환자의 상태변화에 유의미하게 작용한다. 따라서 이송 업무를 수행하는 간호사에게는 지상(ground) 근무보다 숙련된 임상수행 능력이 요구되며, 근거 중심의 판단을 통해 비행 중 발생 가능한 위험요인들을 대응한다. 그러나 실제 현장에는 표준화된 간호 가이드라인이 부재하고, 이송 과정에서 간호사가 개인의 임상 경험이나 직관적인 판단에 의존하는 경향이 있다. 이러한 실무적 한계를 근거기반의 간호실무 가이드라인을 개발을 통해 해결하고자 하였다.
본 연구는 수용개작(adaptation) 방법론에 따라 중환자 항공이송 간호실무 가이드라인(evidence-based nursing practice guideline for critical care air transport)이 개발되었다. 또한 간호의 지식과 경험을 과학적 근거로 전환하기 위해 ACE Star Model의 단계별 절차를 따르고, 환자의 임상적·심리적·환경적 특성과 간호사의 역량이 조화를 이룰 수 있도록 Synergy Model을 이론적 기틀로 권고문을 작성하였다.
국내·외 중환자 이송관련 가이드라인을 체계적으로 검색하고, AGREE (Appraisal of Guidelines, Research and Evaluation) II 도구로 후보 가이드라인을 평가하여 수용개작을 위한 기준을 선정하였다. 이후 고정익 항공이송 환경에 적합하도록 가이드라인 초안을 작성하고, 외부검토와 수정를 통해 최종안을 확정하였다.
연구 결과, 최종 가이드라인은 △시스템/운영, △이송 전 준비, △이송 중 간호관리, △이송 후 간호관리, △질 관리/역량 강화의 5개 영역으로 범주화되었다. 세부적으로 위 영역에서 20개의 핵심 업무가 도출되었으며, 이를 총 125개의 권고문으로 구체화하였다. 또한 각 권고문에는 근거 수준(level of evidence)과 권고 등급(recommendation grade)을 부여하여 사용자가 근거의 타당성을 판단할 수 있도록 하였다.
대한민국 현재 상황에 맞춰 수용개작한 본 가이드라인은 중환자 항공이송 간호의 표준화를 이끄는 첫 시도로서, 실무적 신뢰성 확보와 환자안전 증진에 중대한 역할을 수행할 것이다. 방법론 측면에서는 간호사가 주도하여 4인의 개발그룹을 통해 가이드라인 개발이 가능함을 시사하며, 현장의 요구도가 높은 핵심 업무를 중심으로 실효성 있는 지침을 마련하였다는 점에서 실무적 가치가 있다. 본 연구의 결과물은 향후 정책적 제도 개선과 교육·훈련 인프라 구축을 위한 실증적 토대가 될 것이다. 나아가 중환자 항공 간호의 전문역량 강화와 환자안전이라는 두 가지 가치를 실현하는데 중요한 밑거름이 될 것이다.

Critical care air transport (CCAT) via fixed-wing aircraft involves unique environmental stressors, such as altitude fluctuations, noise, and vibration, which significantly impact the physiological stability of critically ill patients. Consequently, flight nurses require higher-level clinical competencies and rigorous evidence-based judgment compared to ground-based settings. Despite these complexities, the absence of standardized nursing protocols in actual field settings has forced clinicians to rely heavily on subjective intuition and personal experience. This study addressed these practical limitations by developing evidence-based nursing practice guidelines tailored to the unique environment of aeromedical transport.
The guidelines were developed using a systematic adaptation methodology. To transform clinical knowledge into scientific evidence, the study adhered to the structured phases of the ACE Star Model. Furthermore, the Synergy Model served as the theoretical framework to ensure strategic alignment between the patient’s clinical, psychological, and environmental needs and the nurse’s specialized competencies. A comprehensive search of domestic and international guidelines was conducted, and candidate guidelines were appraised using the AGREE II (Appraisal of Guidelines, Research and Evaluation II) instrument. Following the selection of source guidelines, a preliminary draft optimized for fixed-wing environments was formulated and finalized through a rigorous process of external expert validation and subsequent revisions.
The final guidelines were categorized into five comprehensive domains: (1) System and Operations, (2) Pre-transport Preparation, (3) In-flight Nursing Management, (4) Post-transport Nursing Care, and (5) Quality Management and Competency Building. Within these domains, 20 core tasks were identified and further developed into a total of 125 specific recommendations. To facilitate clinical decision-making, each recommendation was assigned a level of evidence and a recommendation grade, allowing users to assess the scientific validity and practical strength of each intervention.
As the first initiative to standardize nursing practice for critical care air transport in South Korea, these guidelines are poised to play a pivotal role in enhancing clinical reliability and patient safety. Methodologically, this study validates the feasibility of developing high-quality guidelines through a nursing-led expert group. By providing evidence-based protocols centered on high-demand core tasks, the findings offer an empirical foundation for future policy development and the establishment of robust training infrastructures. Ultimately, this work serves as a cornerstone for advancing professional nursing expertise and ensuring the highest standards of safety in aeromedical environments.
