연구목적: 성장기 아동·청소년의 두개악안면 형태를 정확히 파악하는 것은 성공적인 교정 치료 계획 수립의 핵심 요소이다. 그러나 기존의 정적 측정 방식은 단일 시점 측정값에 기반하여, 성장 과정에서 나타나는 동적 변화와 개인 간의 변이를 충분히 반영하지 못하였다. 또한 수동 두부계측 분석은 노동 집약적이고 숙련된 전문지식을 필요로 한다.
이에 본 연구에서는 8세에서 16세 사이 성장기 환자의 측모두부규격방사선사진을 이용하여, ANB 각도 변화를 추정할 수 있는 새로운 해부학적 사전 정보 기반 심층학습 모델(Skel-Net)을 제안하였다. Skel-Net은 해부학적 구조에 대한 사전 정보를 네트워크 학습 과정에 통합함으로써, 기존의 단순 영상 기반 모델이 갖는 한계를 극복하고 성장 양상의 정량적 추정 가능성을 평가하고자 하였다. 또한 기존 주요 합성곱 신경망 기반 모델들과의 비교를 통해 제안한 모델의 성능과 임상 적용 가능성을 종합적으로 검증하였다.
연구방법: 본 연구에서는 해부학적 선행 정보 기반의 2단계 심층학습 구조를 적용하였다. 1단계에서는 Ceph-Net을 통해 측모두부규격방사선사진에서 주요 두부 계측점을 자동 검출하고, 2단계에서는 생성된 2차원 히트맵과 ANB 각도 정보를 포함한 다채널 입력을 활용하여 Skel-Net의 추정 정확도를 향상시켰다. 성장기 환자 245명의 방사선영상 612장을 학습 및 검증에 사용하였으며, 모델의 성능은 DenseNet121, MobileNetV2, ResNet101, VGG16과 비교하여 평가하였다.
연구결과: Skel-Net은 시험 세트에서 평균절대오차(MAE) 1.021°, 평균제곱근오차(RMSE) 1.338°, 결정계수(R2) 0.517을 기록하여, 비교 모델 대비 가장 낮은 오차와 가장 높은 설명력을 나타냈다. 또한 추정된 ANB 각도 값을 기준으로 한 전후방 골격 부조화 분류(I급, II급, III급)에서도 가장 우수한 성능을 보였으며, 분류 정확도 81.6%, 정밀도 83.8%를 나타냈다. Bland–Altman 분석에서도 Skel-Net은 좁은 신뢰구간과 낮은 편향을 보여 추정값과 실제값 간의 높은 일치도를 확인하였다.
결론: 본 연구에서는 성장기 아동·청소년의 측모두부규격방사선사진을 활용하여, 해부학적 사전 정보를 통합한 딥러닝 기반 ANB 각도 변화 추정 및 전후방 골격 부조화 분류 모델 Skel-Net을 제안하였다. Skel-Net은 해부학적 계측점 기반 히트맵과 ANB 각도를 다채널 입력으로 활용함으로써, 기존 합성곱 신경망 기반 모델보다 정밀하고 안정적인 성장 변화 분석이 가능함을 확인하였다. 향후 다양한 인구집단과 영상 환경에서의 검증을 통해 일반화 가능성을 높이고, 여러 두부계측 지표를 통합 분석하는 확장형 모델로 발전시킬 수 있을 것으로 기대된다.

Purpose: Accurate assessment of craniofacial morphology in growing children and adolescents is essential for establishing effective orthodontic treatment plans. However, conventional static measurements based on single time points fail to adequately capture dynamic growth-related changes and inter-individual variability. In addition, manual cephalometric analysis is labor-intensive and requires substantial expert knowledge. Accordingly, this study proposes a novel deep learning model, termed Skel-Net, to estimate longitudinal changes in the ANB angle using lateral cephalometric radiographs of growing patients aged 8–16 years. By integrating anatomical prior information into the learning process, Skel-Net is designed to overcome the limitations of conventional image-only models and to enable quantitative estimation of craniofacial growth patterns. The performance and potential clinical applicability of the proposed model were comprehensively evaluated through comparison with representative convolutional neural network (CNN)–based models.
Methods: A two-stage deep learning framework incorporating anatomical prior information was employed. In the first stage, Ceph-Net was used to automatically detect key cephalometric landmarks from lateral cephalometric radiographs. In the second stage, multi-channel inputs consisting of landmark-based two-dimensional heatmaps and baseline ANB angle information were integrated to improve the estimation accuracy of Skel-Net. A total of 612 radiographs from 245 growing patients were used for model training and evaluation. The performance of Skel-Net was compared with that of DenseNet121, MobileNetV2, ResNet101, and VGG16.
Results: On the test dataset, Skel-Net achieved a mean absolute error (MAE) of 1.021°, a root mean square error (RMSE) of 1.338°, and a coefficient of determination (R2) of 0.517, demonstrating the lowest estimation errors and the highest explanatory power among the compared models. In addition, Skel-Net showed superior performance in ANB angle–based anteroposterior skeletal discrepancy classification (Class I, II, and III), achieving a classification accuracy of 81.6% and a precision of 83.8%. Bland–Altman analysis further confirmed a high level of agreement between estimated and reference values, with narrow limits of agreement and minimal bias.
Conclusion: This study presents Skel-Net, a deep learning–based framework that integrates anatomical prior information to estimate longitudinal changes in the ANB angle and to classify anteroposterior skeletal discrepancies using lateral cephalometric radiographs of growing patients. By incorporating landmark-based heatmaps and baseline ANB angle values as multi-channel inputs, Skel-Net enables more precise and stable analysis of craniofacial growth patterns than conventional CNN-based models. Future studies involving more diverse populations and imaging conditions may further enhance the generalizability of the proposed framework and facilitate its extension to the integrated analysis of multiple cephalometric parameters.
