    본 연구의 목적은 공감 수업을 경험한 의과대학생의 공감 인식이 어떠한지를 질적으로 탐구하는 것이다. 공감은 의사에게 필수적인 핵심 역량이지만, 의학교육 과정 중 공감 능력이 저하되는 '공감 침식(empathy erosion)' 현상이 지속적으로 나타나고 있다. 그러나 기존 연구들은 주로 교육의 효과를 양적으로 측정하는 데 집중하여, 학습자가 공감을 실제로 어떻게 인식하고 의미화하는지에 대한 심층적 이해는 부재한 실정이었다.
    이에 본 연구는 질적 사례연구 방법을 적용하여 공감 수업을 수강한 의과대학생 11명을 대상으로 심층 면담을 실시하고, Creswell의 나선형 자료분석 절차에 따라 분석을 진행하였다. 연구 결과, 학생들의 공감 인식은 '추상적이고 모호한 개념'에서 출발하여, 입시 과정을 통해 '공감에 대한 자각과 성찰의 기회'로 발전하였으며, 이후 공감 수업을 통해 다양한 배경을 가진 타자와의 관계 맺기를 경험하면서 '타자의 존재에 대한 수용'으로 확장되었다. 이 과정에서 학생들은 구체적인 경험을 공유하며 '경청과 상호작용의 중요성'을 실천적 차원에서 이해하게 되었고, 궁극적으로 '환자 이해의 핵심 역량이자 의사로서의 사회적 책임'이라는 다차원적 개념으로 심화되는 양상을 보였다.
    학생들은 입학 전 공감을 막연하게만 인식했으나, 발달장애인 파트너와의 지속적인 관계 속에서 공감을 "입시 때는 말로써의 공감을 배웠다면, 지금은 마음 자체를 키우는 과정"이라고 표현하며 질적 전환을 경험하였다. 특히 공감을 "같은 감정을 느끼는 것"에서 "다를 수밖에 없는 존재임을 기억하는 것"으로, "말하기"에서 "경청하기"로 재정의하였으며, 나아가 '정확한 진료를 위한 도구'이자 '관계 지향적 의사'로서의 정체성과 연결된 전문적 역량으로 인식하게 되었다.

    The purpose of this study is to qualitatively explore how medical students who have experienced an empathy-focused course perceive empathy. Although empathy is an essential core competency for physicians, the phenomenon of “empathy erosion” during medical training has been consistently reported. Previous studies have mainly concentrated on quantitatively measuring the effects of educational interventions, leaving a limited understanding of how learners actually perceive and make meaning of empathy.
    Accordingly, this study employed a qualitative case study approach, conducting in-depth interviews with 11 medical students who had taken an empathy course and analyzing the data following Creswell’s spiral data analysis procedure. The findings show that students’ perceptions of empathy began as “an abstract and ambiguous concept,” developed into “an opportunity for awareness and reflection on empathy” through the university admissions process, and eventually expanded into “acceptance of the other’s existence” as they formed relationships with individuals of diverse backgrounds during the course. Throughout this process, students shared concrete experiences and came to understand the importance of “listening and interaction” at a practical level. Ultimately, their understanding deepened into a multidimensional concept in which empathy is viewed as “a core competency for understanding patients and a social responsibility of physicians.”
    Students initially perceived empathy vaguely prior to entering medical school, but through sustained relationships with partners with developmental disabilities, they experienced a qualitative transformation. One student expressed this shift as “If during admissions we learned empathy as something expressed in words, now it’s a process of cultivating the heart itself.” In particular, they redefined empathy from “feeling the same emotion” to “recognizing that others are inevitably different,” and from “speaking” to “listening.” They further came to view empathy as both “a tool for accurate clinical care” and a professional competency linked to the identity of becoming a “relationship-oriented physician.”
    Meanwhile, students’ perceptions of empathy were shaped through interactions with various contexts both inside and outside the curriculum. Experience-centered course design and the instructor’s authentic attitude played important roles in helping students develop deeper understandings of empathy. However, excessive academic workload, competitive environments, and implicit messages within the hidden curriculum suggesting that “empathy is unnecessary” created cognitive dissonance for students.
    In conclusion, this study demonstrates that medical students’ perceptions of empathy evolve through a dynamic and context-dependent process of exploring and reconstructing meaning within relationships and experiences. These findings suggest that future medical education should move beyond knowledge-transmission-oriented environments and instead design learning contexts that enhance relational experiential learning and foster students’ epistemological transformation through reflection.
