Carbapenem-resistant Acinetobacter baumannii (CRAB)는 치료가 어렵고 전파력이 높아 의료기관에서 지속적인 주의가 필요한 다제내성균으로, 환자의 사망률 및 재원일수를 증가시켜 의료시스템에 중대한 부담을 초래한다. 특히 응급중환자실은 응급의료 환경과 중환자실의 특성이 결합된 고위험 진료 환경으로, 침습적 처치가 빈번하고 개방형 병상 구조로 운영되어 CRAB 전파에 더욱 취약하다. 이에 본 연구는 응급중환자실의 CRAB 획득률과 환경적 특성의 변화를 파악하고, 환자·치료·환경 요인을 통합적으로 분석하여 응급중환자실의 CRAB 관리 및 실질적인 예방 전략의 근거를 마련하고자 수행되었다.
본 연구는 2020년 4월 1일부터 2023년 12월 31일까지 서울 소재 상급종합병원 응급중환자실에 입실한 환자를 대상으로 한 후향적 환자-대조군 연구이다. 월별 지표를 이용하여 CRAB 획득률과 환경적 특성(일평균 CRAB 재원 환자 수, 병상가동률, 일평균 인공호흡기 사용 환자 수, 일평균 고유량 비강 캐뉼라 사용 환자 수, 손위생 수행률)의 추이를 Spearman 상관분석을 통해 확인하였으며, CRAB 획득군 48명과 대조군 144명을 대상으로 일반적 특성, 질병 관련 특성, 치료 관련 특성, 환경 관련 특성의 4단계 위계적 다변량 로지스틱 회귀분석을 시행하여, 모델의 단계적 설명력 변화와 CRAB 획득의 독립적인 위험요인을 확인하였다.
연구 결과, 월별 분석 추이에서 CRAB 획득률은 일평균 CRAB 재원 환자 수와 유의한 양의 상관관계를 보였다(r = .399, p = .007). 위계적 회귀분석 결과 모델의 설명력(Nagelkerke R2)은 환자 및 질병 단계의 .043에서 치료 및 환경 단계가 추가됨에 따라 .609로 유의하게 증가하였다. 최종 모델에서 CRAB 획득의 독립적인 위험요인은 카바페넴 사용(OR = 8.58; p < .001), 배액관 거치(OR = 6.37; p = .004), 경장영양 시행(OR = 6.36; p = .002), 인공호흡기 사용(OR = 4.79; p = .037)의 치료 관련 특성과 CRAB 재원 환자 수(OR = 3.56; p < .001)의 환경 관련 특성으로 확인되었다. 환자 관련 특성에서는 연령(OR = 0.95; p = .008)과 기저질환 수(OR = 1.75; p = .020)가 영향을 미치는 것으로 나타났다.
본 연구는 응급중환자실이라는 특수한 환경에서 CRAB 획득에 영향을 미치는 요인을 환자·치료·환경 영역에서 종합적으로 분석했다는 점에서 의의가 있다. 특히 체외 배출물과 관련된 침습적 처치와 CRAB 재원환자 수와 같은 환경적 특성이 주요 위험요인으로 확인된 것은, 조직 차원의 감염관리 노력이 필요함을 시사한다. 따라서 고위험 처치 과정에 대한 표준화된 프로토콜을 정립하고, 보균자 증가 시 선제적 격리 및 환경관리를 적극적으로 시행하는 등 침습적 처치 및 환경 요인을 통합적으로 고려한 감염관리 전략 수립이 요구된다. 본 연구 결과는 향후 중환자실 간호사를 대상으로 한 감염관리 교육과 실무 지침 개발에 활용될 수 있으며, 나아가 결과의 일반화를 위한 전향적 다기관 후속 연구를 제언한다.

Carbapenem-resistant Acinetobacter baumannii (CRAB) is a multidrug-resistant organism that requires continuous vigilance in healthcare settings due to its treatment difficulty and high transmissibility. It increases mortality and length of stay, thereby imposing a substantial burden on healthcare systems. The Emergency Intensive Care Unit (EICU), characterized by the combined features of emergency care and critical care, represents a high-risk clinical environment where invasive procedures are frequent and open-bed structures are common, making it particularly vulnerable to CRAB transmission. Accordingly, this study aimed to identify trends in CRAB acquisition rates and environmental characteristics in the EICU and to comprehensively analyze patient, treatment, and environmental factors to establish evidence for CRAB prevention and management strategies tailored to this setting.
This retrospective case–control study included patients admitted to the EICU of a tertiary hospital in Seoul from April 1, 2020, to December 31, 2023. Monthly indicators were used to examine the trends of CRAB acquisition rates and environmental characteristics (daily mean number of inpatients with CRAB, bed occupancy rate, daily mean number of patients using mechanical ventilation, daily mean number of patients using high-flow nasal cannula, and hand hygiene compliance) through Spearman correlation analysis. A four-block multivariable hierarchical logistic regression analysis, consisting of general, disease-related, treatment-related, and environmental characteristics, was performed with 48 cases of CRAB acquisition and 144 controls to identify the independent risk factors for CRAB acquisition and to evaluate the incremental explanatory power of the model.
The results indicated a significant positive correlation between CRAB acquisition rates and the daily mean number of inpatients with CRAB(r = .399, p = .007). The hierarchical regression analysis revealed that the model's explanatory power (Nagelkerke R2) increased significantly from .043 in the general and disease-related stages to .609 after the treatment and environmental stages were added. Regarding independent risk factors, the analysis identified treatment-related characteristics such as carbapenem use (OR = 8.58; p < .001), placement of a drainage catheter (OR = 6.37; p = .004), enteral nutrition (OR = 6.36; p = .002)), and mechanical ventilation (OR = 4.79; p = .037). Additionally, the number of inpatients with CRAB (OR = 3.56; p < .001) was found to be a significant environmental risk factor. Among general and disease-related factors, age (OR = 0.95; p = .008) and the number of comorbidities (OR = 1.75; p = .020) were associated with increased risk.
This study is significant in that it provides a comprehensive analysis of the determinants of CRAB acquisition across general, disease-related, treatment-related and environmental factors within the unique context of the EICU. The identification of invasive procedures involving body fluid exposure and environmental factors, particularly the number of inpatients with CRAB, as major risk factors implies the need for systemic infection control efforts at the organizational level. Therefore, it is essential to establish infection control strategies that integrally consider both invasive procedures and environmental factors, such as establishing standardized protocols for high-risk procedures and actively implementing preemptive isolation and enhanced environmental management when the number of carriers increases. These findings provide foundational data for developing infection control education and practice guidelines for ICU nurses, and prospective multicenter studies are recommended to enhance generalizability.
