The Fundamentals of care (FOC) framework describes patients' physical and psychosocial needs and the nursing activities required to address them across three dimensions, presenting a positive and trustworthy nurse-patient relationship as the core dimension. In particular, Airborne infection isolation rooms(AIIRs) emphasize the physical and emotional care needs of patients within the restricted environment of isolation. Nurses and patients have the closest contact and spend the most time together, making their relationship even more important. Appropriate nurse-patient interactions are known to provide patients with psychological stability, positively impacting overall health improvement. However, research on relationship-based interactions between nurses and patients isolated due to infectious diseases remains insufficient.
This study aimed to investigate the nurse-patient relationship experienced by patients in AIIRs and to analyze it from the perspective of the nurse-patient relationship within the FOC framework.
Participants were selected based on the criteria of being patients aged 18 years or older who had been hospitalized in AIIRs for infectious diseases for at least 3 days. To ensure the truthfulness of responses and voluntary participation, only patients not directly cared for by the researcher were selected. The study purpose was explained using an information sheet and recruitment notice. Data were collected through semi-structured one-on-one in-depth interviews with 11 patients voluntarily consenting to participate, who were currently residing in AIIRs at a tertiary general hospital. Thematic analysis, following Braun and Clarke (2006), was conducted using a hybrid approach that integrated deductive (pre-determined FOC codes) and inductive (emergent codes) strategies.
The analysis yielded a total of 5 themes and 12 sub-themes.
The first theme, ‘Feeling safe enough to entrust oneself’ describes how patients requiring isolation for infectious diseases came to build trust with nurses in AIIRs. Despite being in an unfamiliar environment and cut off from the outside world, patients formed trust through the nurses’ attitudes and interactions. This theme was reflected in three sub-themes: ‘A sense of safety created by immediate responses’, ‘Reduced anxiety through information sharing’, and ‘Experiencing genuine, sincere care.’
The second theme, ‘Experiencing continuous and attentive care’ addressed how patients in solitary isolation experienced ongoing attention and care from nurses. It revealed nurses asking about what was needed or uncomfortable for the patient's well-being and closely observing changes in the patient's condition. This emerged as two sub-themes: ‘Undivided attention encountered in an isolated space’ and ‘Having subtle changes accurately perceived through meticulous observation.’
The third theme, ‘Experiencing prepared care’ describes situations in which nurses anticipated and responded to the needs of patients in AIIRs, even when these needs were not explicitly expressed. This theme was identified through two sub-themes: ‘Receiving care that was provided proactively without having to ask’ and ‘Predictable care made possible through prior education.’
The fourth theme, ‘Recognizing the nurse as the one who understands me best’ reflects patients' experiences of nurses in AIIRs thoroughly understanding their condition, treatment process, and emotions. This theme manifested in two sub-themes: ‘Comprehensive understanding of oneself and one's situation’ and ‘Difficulty distinguishing nurses due to personal protective equipment.’
The fifth theme, ‘Relationship built together’ reflects patients’ positive evaluations of nurses’ efforts to build the nurse–patient relationship in AIIRs, as well as patients’ own willingness to contribute to fostering this relationship. This was derived into three sub-themes: ‘Observing the nurse’s efforts toward continuous improvement’, ‘The patient's effort to build a relationship with the nurse’, and ‘Desire for continuity in the nurse-patient relationship.’
Analysis revealed that patients in AIIRs experienced positive nurse-patient relationships through building trust with nurses and through nurses’ attentive and responsive care to their needs. Such a constructive relationship helped transform the anxiety associated with isolation into psychological comfort and was found to contribute to the patients’ recovery.
In addition, the fact that both nurses and patients made efforts to develop their relationship indicated that both parties are active participants in that relationship. A relationship built upon mutual respect suggests that warm emotional bonds between individuals underlie its structure.
In conclusion, nurse-patient relationship formation based on the FOC framework within the context of AIIRs was found to contribute to patients' psychological stability and recovery. This suggests the need for FOC framework-based nursing education, systematic communication protocols, and consideration of the AIIRs environment and institutional policies to support relationship-centered care in isolation settings.
