    본 연구는 외과계 중환자실 환자의 재입실에 영향을 미치는 요인을 다면적으로 분석하여, 재입실 예방 및 임상 관리 향상에 기여하기 위해 수행되었다. 세부 목표는 외과계 중환자실 환자의 재입실률을 확인하고, 인구학적, 생리적, 상황적 요인 및 증상을 포함한 재입실 예측 요인을 통합적으로 규명하는 데 있다.

    본 연구는 후향적 서술적 조사연구로, 2022년 5월 1일부터 2025년 4월 30일까지 서울 소재 일개 대학병원의 외과계 중환자실에 입실하여 치료를 받고 퇴실한 만 18세 이상 환자를 대상으로 시행되었으며, 총 4,387건의 전자의무기록 데이터를 검토하였다. 재입실군과 비재입실군의 비교를 위해 성별, 연령, 수소이온 농도지수(pH), 호흡수, Glasgow coma scale, Acute Physiology and Chronic Health Evaluation Ⅱ 점수, 기관절개관 유무, 기계적 환기 사용 및 중심정맥관 유무, 지속적 신대체요법 시행 여부, 주 진단명, 수술 여부, 중환자실 재실 시간, 중환자실 입/퇴실 시기, 초조, 통증, 섬망 등 다면적 변수를 수집하여 기술 통계 및 Chi square test, independent t-tes를 시행하였고, 두 군간 차이가 있는 변수를 대상으로 단순 및 다중 로지스틱 회귀분석을 실시하였다.

    연구 결과, 전체 표본의 8.75%가 중환자실 재입실을 경험하였으며, 단순 로지스틱 회귀 분석에서 연령, 성별, Glasgow coma scale, 중심정맥관 여부, 호흡계통 등의 진단명, 입실 시 Acute Physiology and Chronic Health Evaluation Ⅱ 점수, 초조, 통증, 섬망 등이 유의한 변수로 나타났다. 이를 바탕으로 다중 로지스틱 회귀 분석을 시행한 결과 연령의 증가, 신생물 진단 대비 호흡계통 질환의 진단명을 가진 경우, Glasgow coma scale 총점이 낮을수록, 중심정맥관을 가진 경우, 신생물 진단 대비 기타 질환의 진단명을 가진 경우, 초조를 경험한 경우의 순으로 외과계 중환자실 재입실의 유의한 예측 요인인 것으로 분석되었다.

    본 연구는 외과계 중환자실 환자의 재입실에 영향을 미치는 요인을 다면적으로 분석하고, 이에 대한 심층적인 이해를 도모함으로써 중환자실 환자의 재입실 위험 예측에 주요 근거자료로 활용될 수 있을 것이다. 향후에는 추가적인 관련 요인 분석과 더불어 다양한 임상 환경에서의 후속 연구가 필요하며, 해당 결과를 바탕으로 재입실 감소를 위한 예방적 간호중재 개발과 관리 전략 수립이 필요할 것이다.

    This study was conducted to comprehensively analyze the factors influencing readmission to the surgical intensive care unit (ICU) and to contribute to the prevention of readmission and improvement of clinical management. The specific objectives were to identify the rate of readmission among surgical ICU patients and to determine integrative predictive factors—including demographic, physiological, situational variables, and patient symptoms—associated with ICU readmission.
    This retrospective descriptive study was performed on adult patients aged 18 years or older who were admitted to and discharged from the surgical ICU of a university hospital in Seoul between May 1, 2022, and April 30, 2025. A total of 4,387 electronic medical records were reviewed. To compare the readmission and non-readmission groups, various multidimensional variables were collected, including sex, age, potential of hydrogen (pH), respiratory rate, Glasgow Coma Scale (GCS), Acute Physiology and Chronic Health Evaluation II (APACHE II) score, presence of tracheostomy, use of mechanical ventilation, presence of a central venous catheter (CVC), performance of continuous kidney replacement therapy (CKRT), primary diagnosis, surgical status, length of ICU stay, timing of ICU admission and discharge, agitation, pain, and delirium. Descriptive statistics, Chi-square tests, and independent t-tests were performed, followed by simple and multiple logistic regression analyses for variables showing significant differences between groups.
    The results indicated that 8.75% of all patients experienced ICU readmission. Simple logistic regression identified age, sex, GCS, presence of CVC, respiratory system–related diagnoses, APACHE II score at admission, agitation, pain, delirium and others as significant variables. Based on these results, multiple logistic regression analysis revealed that increased age, diagnosis of respiratory disease compared to neoplasm, lower GCS score, presence of a CVC, diagnosis of other diseases compared to neoplasm, and the presence of agitation were significant predictors of surgical ICU readmission.
    This study provides a comprehensive analysis and in-depth understanding of the factors influencing surgical ICU readmission. The findings can serve as fundamental evidence for predicting readmission risk among ICU patients. Future studies should explore additional related factors and conduct research across diverse clinical environments. Based on these results, preventive nursing interventions and management strategies should be developed to reduce ICU readmission rates.
