산업재해 근로자의 수는 꾸준히 증가하고 있으며 산재보험 제도를 통해 의료서비스를 지원받고 있다. 그러나 현행법상 요양 종결이 의학적 완치가 아닌 증상 고정을 기준으로 이루어짐에 따라 산재 근로자는 치료 종결 이후에도 본인부담금, 비급여 항목, 시간적 제약 등 다양한 요인으로 인해 여전히 미충족 의료를 경험하고 있다. 산재보험패널 3차 코호트 분석 결과 미충족 의료 경험률(6.0%)은 2차 코호트(5.7%) 대비 개선되지 않고 정체되는 경향을 보인다. 특히 고용 안정성, 소득수준과 밀접한 관련이 있는 종사상 지위는 산재 근로자의 의료 접근성에 주요한 영향을 미치는 요인으로 예상되나 기존 연구들은 주로 경제활동 여부에 초점을 맞추어 종사상 지위에 따른 차이를 심층적으로 분석한 연구는 부족하다. 따라서 본 연구는 최신 산재보험 패널 3차 코호트 자료를 활용하여 산재 근로자의 종사상 지위가 미충족 의료 경험에 미치는 영향을 규명하고 고용 형태별 의료 접근성 개선을 위한 정책적 근거를 마련하고자 하였다.
본 연구는 2022년에 요양이 종결된 산재 근로자를 대상으로 한 산재보험패널 3차 코호트 1차년도 자료(2023년)를 활용하였으며 미충족 의료 경험에 응답한 3,124명을 최종 분석 대상으로 선정하였다. 앤더슨의 모형을 기반으로 연구모형을 설정하였으며 종속변수는 미충족 의료 경험 여부, 독립변수는 산재 근로자의 현재 종사상 지위(상용직, 임시·일용직, 자영업자, 미취업자)로 정의하였다. 분석에는 앤더슨 모형의 소인 요인, 가능 요인, 욕구 요인을 통제 변수로 두고 로지스틱 회귀분석을 실시하였다. 미충족 의료 경험자를 대상으로 원인별(시간 부족 및 경제적 이유)로 로지스틱 회귀분석을 추가로 수행하였다.
분석 결과, 산재 근로자의 종사상 지위는 미충족 의료 경험에 유의한 영향을 미치는 것으로 나타났다. 다른 변수를 고려하지 않았을 때 상용직 근로자의 미충족 의료 경험 비율은 다른 집단보다 실제로 낮았으나 개인의 인구 사회학적 특성, 소득, 건강 상태 등 관련 요인들을 모두 통제한 후에는 결과가 역전되었다. 즉, 다른 조건이 동일할 때 오히려 상용직 근로자가 미충족 의료를 경험할 가능성이 유의하게 높아지는 것으로 나타났다. 미충족 의료의 주된 원인 역시 뚜렷한 차이를 보였는데 다른 집단은 경제적 부담을 가장 큰 원인으로 꼽은 반면, 상용직 근로자는 시간 부족 때문이라는 응답이 압도적으로 높았다. 이는 종사상 지위에 따라 미충족 의료의 발생 가능성뿐만 아니라 그 원인에도 차이가 있음을 시사한다.
본 연구는 산재 근로자의 종사상 지위가 미충족 의료 경험의 중요한 예측 요인임을 확인하였다. 특히 안정적인 일자리를 가진 상용직 근로자가 시간 제약으로 인해 의료 이용에 더 큰 장벽을 겪는다는 점은 중요한 시사점이다. 따라서 산재 근로자의 건강권 보장과 성공적인 사회복귀를 위해서 종사상 지위의 특성을 고려한 맞춤형 정책 설계가 필요하다. 불안정 고용 근로자에게는 직업 복귀 지원을 강화하고 상용직 근로자에게는 유연근무제나 유급 질병 휴가제도를 법적으로 의무화하는 방안을 고려해야 한다.

This study analyzed the impact of employment status on unmet healthcare needs among workers with industrial accidents. Despite Workers' Compensation Insurance, many injured workers experience unmet needs because treatment is terminated based on ‘symptom fixation rather than a complete medical cure. According to the PSWCI, the rate of unmet healthcare needs (6.0%) has remained stagnant compared to the previous cohort (5.7%). The specific impact of employment status on their healthcare access has been understudied, as previous research focused mainly on economic activity. Therefore, this study aims to provide a policy basis by identifying this impact.
This study used data from the first wave (2023) of the 3rd cohort of the Panel Study of Workers' Compensation Insurance (PSWCI), analyzing 3,124 workers whose medical treatment ended in 2022. Based on Andersen's model, logistic regression was conducted to analyze the effect of employment status (salaried, temporary/daily, self-employed, non-employed) on unmet healthcare needs, controlling for predisposing, enabling, and need factors. Additional logistic regressions were also performed by cause (lack of time and economic reasons).
Results indicated that employment status was a predictor for unmet needs. In a simple analysis, salaried workers had a lower prevalence of unmet needs. However, this result was reversed after controlling for all covariates such as socio-demographic characteristics, income, and health status. This means that when other conditions are equal,salaried workers were actually more likely to experience unmet needs. The main reasons for unmet needs also varied distinctly by group. For other groups, economic burden was the main problem. For salaried workers, however, the biggest reason was overwhelmingly lack of time.
This study confirms employment status as a predictor of unmet healthcare needs for injured workers. The fact that salaried workers with stable jobs have barriers due to time constraints is an important finding. Specific policies are needed based on this. We suggest strengthening return-to-work support for precariously employed workers. For salaried workers, measures like legally mandating flexible work schedules or paid sick leave should be considered.
