본 연구는 활동지원서비스의 서비스 이용 과정에서 부모의 선택이 어떻게 작용하고 있는지 살펴봄으로써 이용자중심서비스의 현실과 그 한계를 살펴보는 것을 목적으로 한다. 활동지원서비스는 장애당사자의 주도적인 서비스 선택을 핵심 이념으로 하는 이용자중심서비스를 표방하고 있지만, 실제 서비스 이용 과정에서는 가족이 관여되는 경우가 많다는 지적이 선행연구를 통해 반복적으로 언급되고 있다. 그럼에도 불구하고 기존의 활동지원서비스의 이용자 선택권을 둘러싼 논의들은 주로 이용자 개인의 선택을 전제로 하여 예산 구조, 제공기관, 제공인력 등 제공자 측면의 개선 방안을 모색하는 것에 집중되어 왔다. 이에 본 연구는 서비스 전달체계에서 장애당사자에게 서비스를 최종적으로 전달하는 실질적 행위자인 부모의 역할에 주목하여, 서비스 이용 과정 속에서 부모의 선택이 어떠한 경향을 보이는지를 살펴봄으로써 이용자중심서비스의 작동 현실을 보다 구체적으로 파악해보고자 했다. 이를 위해 본 연구는 부모의 돌봄 역동이 명시적으로 작용하는 생애 전반기 단계에 초점을 맞추었으며, 시간의 변화가 내재된 서비스 이용 궤적을 통해 부모의 선택이 어떠한 방식으로 나타나는지 살펴보았다.
연구의 목적에 대한 심층적 탐색과 포괄적 이해를 위해 본 연구는 양적연구를 먼저 수행한 뒤, 그 결과를 토대로 질적연구를 추가로 실시하는 순차적 혼합연구 설계(sequential mixed-methods design)의 방식을 적용하였다. 먼저, 양적연구를 통해 활동지원서비스의 이용 궤적 유형 속에서 부모의 선택 경향이 어떻게 나타나는지 파악하고자 장애당사자와 부모의 입장을 구분하여 분석하였다. 양적연구 단계에서는 “생애 전반기 서비스 이용 궤적은 어떠한 유형을 보이는가?”, “서비스 이용 궤적 유형별 장애당사자가 생각하는 의사결정주체는 시간이 지나며 어떻게 달라지는가?”, “서비스 이용 궤적 유형에 대한 서비스 선택 요인 집합은 장애당사자와 부모 각각에게 단계별로 어떠한 상대적 설명력을 가지는가?”, “서비스 이용 궤적 유형을 결정짓는 장애당사자와 부모의 주요 요인은 유형 간 어떻게 상이하게 작용하는가?”의 네 가지 연구문제에 대해 살펴보았다.
각 연구문제에 답을 하기 위해 장애 등록 이후 초기 경험의 변화를 살피기 적절함과 동시에 장애당사자와 부모의 데이터를 구분하여 살펴보기 용이한 ‘장애인삶 패널조사’를 이용하였으며, 1차년도에서 5차년도까지 총 5개년도의 패널 자료를 활용하였다. 본 연구는 부모 돌봄 역동이 명확하게 존재하는 생애 전반기 연령대인 39세 이하의 장애당사자와 그의 부모 827쌍(장애당사자: 827명, 부모: 827명)의 5개년도 자료를 연결하여 균형 패널 데이터를 구축하였으며, 총 4,135개 관측치를 분석 대상으로 하였다. 분석에서는 먼저 집단중심추세모형(GBTM)을 이용하여 서비스 이용 궤적 유형을 도출한 다음, 조건부전이확률분석을 통해 서비스 이용 궤적 유형별로 장애당사자가 인식하는 의사결정주체의 변화 흐름을 살펴보았다. 이어 위계적 다항로지스틱 회귀분석을 활용하여 서비스 선택 요인 집합의 설명력 구조를 장애당사자와 부모 각각의 입장에서 비교하였으며, 마지막으로 다항로지스틱 회귀분석을 통해 서비스 이용 궤적 유형을 결정짓는 장애당사자와 부모의 주요 요인을 각각 분석하였다. 각 연구 문제에 따른 주요 연구 결과는 다음과 같다.
첫째, 생애 전반기 장애당사자의 활동지원서비스 이용 궤적은 시간의 경과에 따른 선택이 누적된 결과로서 다양한 유형으로 나타났다. 분석 결과, 생애 전반기 서비스 이용 궤적은 ‘서비스 빠른 진입-이용 만족형’, ‘서비스 미인지형’, ‘서비스 미이용형’, ‘서비스 이용 지속형’의 네 가지 유형으로 도출되었다. ‘서비스 빠른 진입-이용 만족형’은 초기에는 제도 외부에 있다가 비교적 이른 시점에 서비스 이용에 진입한 후 이용 만족 상태의 흐름으로 이어지는 유형이었으며, ‘서비스 미인지형’은 서비스 자격 연령에 도달하지 않았거나 자격을 충족하였음에도 서비스에 대한 인지의 상태까지는 이르지 못하는 유형으로 나타났다. ‘서비스 미이용형’은 서비스를 인지하고 있음에도 불구하고 분석 기간 전반에 걸쳐 서비스를 이용하지 않는 상태를 유지하는 유형이며, ‘서비스 이용 지속형’은 서비스에 대한 만족과 불만족의 상태가 교차하는 가운데 서비스를 지속적으로 이용하는 양상을 보이는 유형이다. 이러한 이용 궤적 유형의 분석 결과를 통해 생애 전반기 동안 서비스 이용 흐름이 단일한 경로가 아닌 서로 다른 이용 양상으로 전개됨을 확인할 수 있었다.
둘째, 장애당사자가 인식하는 의사결정주체는 모든 서비스 이용 궤적 유형에서 시간이 경과할수록 전반적으로 부모 중심으로 이동하는 경향을 보였으며, 이러한 양상은 ‘서비스 이용 지속형’에서 가장 두드러지게 나타났다. 서비스를 이용하지 않는 유형(서비스 미인지형, 서비스 미이용형)의 경우, 상대적으로 시간이 지나도 장애당사자가 의사결정주체가 된다는 응답의 비중이 적게나마 유지되었다. 반면, 서비스를 실제로 이용하는 유형(서비스 빠른 진입-이용 만족형, 서비스 이용 지속형)에서는 장애당사자가 의사결정주체가 된다는 응답의 비율이 전반적으로 감소하는 경향을 보였으며, 특히 ‘서비스 이용 지속형’에서 부모 중심의 의사결정 구조가 가장 강하게 나타났다. 이러한 결과는 서비스 이용이 지속되는 과정에서 부모가 의사결정에 보다 중심적인 역할을 하게 될 가능성을 시사한다.
셋째, 서비스 이용 궤적 유형에 대한 서비스 선택 요인 집합의 설명력 구조는 장애당사자와 부모 간에 상이한 양상을 보였다. 장애당사자와 부모 모두 선행요인, 가능요인, 욕구요인 집합을 단계적으로 투입할수록 모형의 설명력이 유의하게 변화하여, 앤더슨 행동모형에 따른 서비스 이용 선택의 경로를 보인다는 것을 확인할 수 있었다. 하지만, 장애당사자의 경우에는 요인 집합이 순차적으로 추가될수록 설명력이 비교적 일관되게 향상된 반면, 부모의 경우에는 선행요인과 가능요인 집합의 단계보다 욕구요인 집합이 추가되는 마지막 단계에서 설명력의 증가가 보다 두드러지게 나타났다. 이는 서비스 이용 과정에는 장애당사자의 선택과 관련된 요인만이 아닌 부모의 요인 또한 함께 영향을 주며, 부모의 욕구요인이 서비스 선택에 크게 작용할 수 있다는 점을 파악해 볼 수 있는 결과이다.
넷째, 서비스 이용 궤적 유형의 결정요인 분석을 통해 각 유형은 장애당사자와 부모의 서비스 선택과 관련된 복합적인 요인들에 의해 구분되는 것으로 나타났다. 서비스 이용 선택과 관련된 주요 결과로는, ‘서비스 미인지형’ 대비 ‘서비스 빠른 진입-이용 만족형’에 속할 가능성은 장애당사자가 거주하는 지역의 객관적 구분(대도시/중소도시)과는 통계적으로 유의하게 나타나지 않은 반면, 지역 환경에 대한 부모의 주관적 인식은 해당 유형에 속할 가능성과 유의한 관련을 보였다. 이는 서비스 이용에 있어 장애당사자가 속한 지역의 절대적 기준보다 부모의 인식과 판단이 중요하게 작용할 수 있음을 시사한다. 또한 서비스를 이용하는 유형들(서비스 빠른 진입-이용 만족형, 서비스 이용 지속형)에서는 장애당사자의 장애 정도가 중증일수록, 그리고 부모가 자녀의 일상생활 지원 필요성을 높게 인식할수록 ‘서비스 미인지형’ 대비 해당 유형에 속할 가능성이 높게 나타났다. 반면, ‘서비스 미이용형’에서는 장애당사자의 장애 정도와 부모의 일상 도움 필요 정도 모두 통계적 유의성이 나타나지 않아, 서비스 이용 여부의 결정에는 장애당사자의 장애 정도와 부모가 생각하는 일상 도움 필요 정도가 영향을 주고 있는 것을 살펴볼 수 있었다. 무엇보다, 부모의 최근 건강 상태는 대부분의 유형에서 통계적으로 유의하지 않았으나, ‘서비스 이용 지속형’에서만 부모의 건강 상태가 좋은 편일수록 ‘서비스 미인지형’ 대비 해당 유형에 속할 가능성이 높게 나타나, 이에 대한 추가 질적 탐색의 필요성이 제기되었다.
양적 자료의 주요 분석 결과는 서비스 이용 과정에서 장애당사자의 요인뿐 아니라 부모의 요인이 복합적으로 작용하며, 경우에 따라서는 장애당사자의 의견보다 부모의 주관적인 판단이나 욕구가 서비스 선택에 더 결정적인 영향을 미칠 수 있음을 시사한다. 그러나 이러한 양적 자료의 분석만으로는 실제 서비스 이용 과정에서 부모가 어떠한 방식과 맥락 속에서 주도적인 선택의 역할을 수행하는지에 대해서는 충분히 설명하기 어렵다. 이에 본 연구는 양적연구의 결과를 보완하기 위해 추가적인 질적연구를 수행하였으며, 부모 중심의 의사결정 양상이 두드러지고 부모의 건강 상태와 관련하여 다른 유형과 구별되는 특성을 보인 ‘서비스 이용 지속형’에 주목하였다. 해당 유형에 속한 부모 10인을 대상으로 심층 인터뷰를 실시하였으며, 이를 통해 서비스 선택권이 실제 이용 과정에서 어떻게 작동하는지를 보다 구체적으로 탐색하고자 하였다.
양적연구의 결과를 보완하기 위해 추가적으로 수행된 질적연구의 단계에서는, “서비스를 이용 지속하고 있는 부모들은 서비스에 대한 선택을 어떻게 하고 있는가?”, “서비스를 이용 지속하고 있는 부모들이 생각하는 서비스 이용자는 누구이며 서비스 선택의 비중은 어떠한가?”, “현재 활동지원서비스에서 부모는 장애당사자의 선택권에 대해 어떻게 생각하는가?”의 세 가지 질문에 대답해보고자 했다. 이를 위해 양적연구에서 도출된 ‘서비스 이용 지속형’ 집단의 내적 특성을 고려하여, 6세 이상 18세 미만의 장애 자녀를 양육하면서 5년 이상 활동지원서비스를 지속적으로 이용해 온 부모를 연구참여자로 선정하였다. 수집된 질적 자료는 서비스 선택권에 관한 이론적 틀을 적용하여 연역적 주제분석법(framework analysis)을 통해 분석하였다.
서비스 이용 지속 부모를 대상으로 수집된 질적 자료를 분석한 결과, 부모들은 서비스를 지속적으로 이용해오는 과정에서 장애 자녀의 입장보다는 부모 자신의 상황과 필요를 중심으로 서비스 선택을 주도하고 있었으며, 스스로를 실질적인 서비스 이용자로 인식하고 있는 것으로 나타났다. 부모들은 서비스 이용의 시작 여부, 세부 이용 내용, 이용 중단과 지속에 이르기까지 서비스 이용 전반에서 부모의 필요와 상황을 우선적으로 고려하여 결정을 내리고 있었다. 서비스 질에 대한 불만이 존재하더라도 장애 자녀의 선호를 반영하기보다는 부모 자신의 판단과 선호를 우선시하는 경향이 두드러졌다. 또한 장애 자녀는 부모의 판단에 따라 사전에 제한된 범위의 서비스 선택지만을 제시받는 경우가 많았으며, 이는 활동지원사와의 소통 과정에서 발생할 수 있는 갈등을 최소화하려는 부모의 전략적 선택으로 나타났다. 더 나아가 서비스 이용 과정 전반에 필요한 정보를 수집하고 이를 바탕으로 활동지원사 및 제공기관과 소통하는 역할 역시 부모에게 집중되어 있었기에, 부모는 적극적으로 일선에서 판단과 조정을 담당하는 주체가 될 수밖에 없는 구조에 놓여 있었다. 정책적 결정 과정에서도 부모들은 자신의 시간 자원을 확보하기 위해 정해진 제도적 틀 안에서 자녀의 장애 정도를 과하게 강조하거나, 안정적인 서비스 이용을 위해 실제 근무 시간보다 더 많은 시간 급여를 활동지원사에게 배정하는 등 제도적 규칙을 조정하며 대응하는 양상을 보였다.
이러한 선택 구조 속에서 부모들은 자신을 실질적인 서비스 이용 주체로 인식하고 있었으며, 그 과정에서 활동지원서비스가 표방하는 정책적 이념과 실제 이용 현실 간의 간극이 분명하게 드러났다. 부모들은 활동지원서비스의 핵심 원칙인 ‘장애당사자의 우선적 선택권’에 대해 충분히 인지하지 못하고 있거나, 해당 이념이 현실적으로 실현되기 어렵다고 생각하고 있었다. 특히 부모들은 인지적 어려움이 있는 장애인의 경우 서비스 이용 과정에서 장애당사자의 선호가 우선적으로 반영되기 위해 의사 표현 방식과 선호 변화에 대한 체계적인 기록을 축적하고, 이를 돌봄 제공자 간에 공유할 수 있는 제도적 기반이 필요하다는 점을 강조하였다. 또한 국가 차원에서는 장애당사자가 실제로 원하는 서비스를 선택하고 이용할 수 있도록 서비스 질을 전반적으로 개선하고, 의사소통 지원을 강화하기 위한 제도적 노력이 병행되어야 함이 부모들의 인터뷰를 통해 확인되었다.
본 연구는 생애 전반기 활동지원서비스의 이용 궤적 유형 속에서 나타나는 부모의 선택 경향을 혼합연구 설계를 통해 다각도로 분석함으로써, 서비스 이용 과정에서 장애당사자뿐 아니라 부모 또한 서비스 이용의 실질적 행위 주체로 작동하고 있음을 규명하였다. 이를 통해 기존 사회서비스 전달체계의 논의에서 비공식 영역에만 머물러 있던 부모를 공식적 행위 주체로 확장하고, 장애당사자의 선택권을 전제로 설계된 이용자중심서비스가 현실에서는 부모의 “대리적 선택권(proxy choice)”에 의해 작동할 수 있음을 밝힘으로써 돌봄 서비스에서 선택권 개념의 재검토 필요성을 제기하였다. 또한 서비스 이용 결정이 개인의 특성만으로는 충분히 설명되기 어렵다는 점을 확인하며, 앤더슨 행동모형을 비공식 돌봄자의 서비스 선택 행위까지 포괄하는 방향으로 확장할 필요성을 제시하였다. 나아가 본 연구는 부모의 판단이 장애당사자의 선택권을 대체하지 않으면서도 긍정적인 관여 방향으로 조정될 수 있는 전달체계의 필요성을 제기하고, 공동적 의사결정 구조와 당사자의 선호 및 의사를 반영할 수 있는 법적·실천적 장치, 부모의 역할을 재위치화하는 정책적·실천적 방향을 제시하였다. 이러한 논의는 이용자중심서비스의 정책적 이념을 현실에서 구현하기 위한 구체적 논의의 토대를 제공하였으며, 향후 돌봄 서비스 전달체계의 질적 성장을 위해 고려되어야 할 후속 연구 과제를 제안하였다.

This study aims to examine the realities and limitations of user-centered services by exploring how parental choices operate within the Personal Assistance Service (PAS) utilization process. Although PAS is designed as a user-centered service emphasizing the autonomy and primary role of persons with disabilities in service choice, prior studies have repeatedly pointed out that family members—particularly parents—are deeply involved in the actual process. Nevertheless, existing discussions on user choice in PAS have largely assumed the individual service user as the sole decision-maker, focusing mainly on provider-side issues such as budget structures and service agencies. Against this backdrop, this study shifts attention to parents as practical actors who ultimately mediate service delivery. By examining patterns of parental choice, this study seeks to provide a more concrete understanding of how user-centered services operate in practice. The analysis focuses on the early life-course stage, where parental caregiving dynamics are particularly salient, investigating how parental choices emerge through service utilization trajectories.
To achieve an in-depth and comprehensive understanding of the research purpose, this study adopted a sequential mixed-methods design, in which quantitative analysis was conducted first, followed by qualitative research to further elaborate on the quantitative findings. In the quantitative phase, analyses were carried out by distinguishing between the perspectives of persons with disabilities and their parents to examine how parental choice patterns emerge across different service utilization trajectory types. Specifically, the quantitative analysis addressed four research questions: (1) “What types of service utilization trajectories are observed during the early life-course stage?”, (2) “How do the decision-making agents perceived by persons with disabilities change over time across different service utilization trajectory types?”, (3) “How do sets of service choice factors demonstrate different patterns of relative explanatory power across trajectory types for persons with disabilities and parents at each analytical stage?”, and (4) “How do the key determinants shaping service utilization trajectory types differ between persons with disabilities and parents?”
To address each research question, this study utilized the Disability and Life Dynamics Panel, which is well-suited for examining changes in early experiences following disability registration and allows for a clear distinction between data from persons with disabilities and their parents. Five waves of panel data, spanning from Year 1 to Year 5, were analyzed. The analytical sample consisted of 827 dyads, comprising persons with disabilities under the age of 39 and their parents (a total of 827 individuals with disabilities and 827 parents), representing the early life-course stage in which parental caregiving dynamics are particularly salient. By linking five years of data, a balanced panel dataset comprising 4,135 observations was constructed. First, service utilization trajectory types were identified using the Group-Based Trajectory Model (GBTM). Subsequently, conditional transition probability analysis was conducted to examine changes over time in the decision-making agents perceived by persons with disabilities across different service utilization trajectory types. Next, hierarchical multinomial logistic regression analyses were employed to compare the relative explanatory power of service choice factor sets between persons with disabilities and parents. Finally, multinomial logistic regression analyses were conducted to identify and compare the key determinants shaping service utilization trajectory types for persons with disabilities and parents. The main findings corresponding to each research question are presented below.
First, service utilization trajectories of PAS among persons with disabilities in the early life-course stage emerged as diverse patterns shaped by the cumulative effects of choices over time. The analysis identified four distinct trajectory types: "Early Entry–Satisfaction," "Service Unawareness," "Non-utilization," and "Continuous Utilization." The Early Entry–Satisfaction type was characterized by an initial period outside the service system, followed by relatively early entry into service use and a transition to a satisfied state. The Service Unawareness type included individuals who had not yet reached the age of eligibility or who, despite meeting eligibility criteria, remained unaware of the service. The Non-utilization type consisted of individuals who were aware of the service but did not utilize it throughout the entire observation period. Finally, the Continuous Utilization type was marked by sustained service use accompanied by alternating states of satisfaction and dissatisfaction. These findings demonstrate that service utilization in the early life-course stage does not follow a single linear pathway but unfolds through multiple, distinct trajectories over time.
Second, the decision-making agents perceived by persons with disabilities showed an overall shift toward parent-centered decision-making over time across all service utilization trajectory types; this pattern was most pronounced in the Continuous Utilization trajectory. Among the non-using trajectories (Service Unawareness and Non-utilization), a small but persistent proportion of respondents continued to identify the person with disabilities as the primary decision-making agent over time. In contrast, among the service-using trajectories (Early Entry–Satisfaction and Continuous Utilization), the proportion of responses identifying the person with disabilities as the decision-maker generally declined as time progressed. This tendency was particularly evident in the Continuous Utilization trajectory, where parent-centered decision-making became most strongly established. Taken together, these findings suggest that as service utilization is sustained over time, parents are increasingly likely to assume a central role in service-related decision-making.
Third, the explanatory power structure of service choice factor sets for service utilization trajectory types differed between persons with disabilities and their parents. For both groups, the stepwise inclusion of predisposing, enabling, and need factors led to statistically significant changes in model fit, indicating that service utilization decisions generally followed the pathways suggested by the Andersen Behavioral Model. However, significant differences emerged between the two groups. For persons with disabilities, explanatory power increased relatively consistently as each factor set was added sequentially. In contrast, for parents, the increase in explanatory power was more pronounced at the final step—when need factors were introduced—rather than during the transition from predisposing to enabling factors. These findings suggest that service utilization processes are shaped not only by factors related to the choices of persons with disabilities but also by parental factors, with parental need-related considerations playing a particularly influential role in service choice.
Fourth, the determinant analysis revealed that service utilization trajectory types were distinguished by a complex interplay of factors related to both persons with disabilities and their parents. Regarding key findings on service choice, the likelihood of belonging to the Early Entry–Satisfaction trajectory, compared to the Service Unawareness trajectory, was not significantly associated with the objective classification of the residential area (metropolitan vs. non-metropolitan). In contrast, parents’ subjective perceptions of regional convenience were significantly associated with a higher likelihood of belonging to this trajectory. This finding suggests that parental perceptions and judgments may play a more influential role in service utilization than objective regional characteristics. In addition, for service-using trajectories (Early Entry–Satisfaction and Continuous Utilization), a higher likelihood of belonging to these groups was observed among persons with disabilities with more severe disabilities and among parents who perceived higher care needs for their offspring, compared to the Service Unawareness trajectory. By contrast, in the Non-utilization trajectory, neither disability severity nor parents’ perceived care needs for their offspring showed statistically significant associations, indicating that these factors were more strongly related to whether services were used at all rather than to specific patterns of non-use. Notably, parents’ recent health status was not statistically significant across most trajectory types; however, only in the Continuous Utilization trajectory did better parental health significantly increase the likelihood of belonging to this group, compared to the Service Unawareness trajectory. This distinctive pattern highlighted the need for further qualitative exploration.
The main quantitative findings indicate that service utilization is shaped by a complex interplay of factors related not only to persons with disabilities but also to their parents. In some cases, parents’ subjective judgments and needs may exert a more decisive influence on service choice than the preferences of persons with disabilities themselves. However, quantitative analysis alone is insufficient to fully explain how and under what circumstances parents assume a dominant role in service-related decision-making within the actual process of service use. To complement the quantitative findings, this study conducted an additional qualitative investigation, focusing on the Continuous Utilization trajectory, in which parent-centered decision-making was most pronounced and parental health status showed distinct characteristics compared to other trajectory types. In-depth interviews were conducted with ten parents belonging to this group to explore in greater detail how service choice operates in practice within the service utilization process.
In the qualitative phase conducted to complement the quantitative findings, this study sought to address the following three questions: (1) “How do parents in the Continuous Utilization trajectory make decisions regarding service choice?”, (2) “Whom do these parents perceive as the actual service user, and how do they understand the relative weight of service choice?”, and (3) “How do parents perceive users’ right to choice within the current PAS system?” To this end, drawing on the characteristics of the Continuous Utilization trajectory identified in the quantitative analysis, the study recruited research participants who were raising a child with a disability aged 6 to 18 and who had used the PAS continuously for more than five years. The qualitative data were analyzed using framework analysis, applying a theoretical framework of service choice.
Analysis of the qualitative data collected from parents in the Continuous Utilization group revealed that, over the course of sustained service utilization, parents tended to lead service-related decisions based primarily on their own circumstances and needs rather than those of their children with disabilities, often perceiving themselves as the de facto service users. Parents played a central role in decision-making across the entire service utilization process, including decisions regarding service initiation, specific content, and the continuation or termination of use. Even when dissatisfaction with service quality was expressed, parents were more likely to prioritize their own judgments and preferences over their children’s expressed or perceived preferences. In addition, children with disabilities were often presented with a pre-filtered and limited set of service options determined by parental judgment—a strategy parents described as a way to minimize potential conflicts in communication with personal assistants. Because the responsibility for acquiring information and coordinating communication with providers was also concentrated on parents, they were structurally positioned as the primary agents of judgment and adjustment. Furthermore, in policy-related contexts, parents were found to strategically adapt institutional rules—for example, by emphasizing the severity of their child’s disability more strongly or allocating more paid service hours than actual working time—to secure time resources and ensure stable service provision.
Within this structure of service choice, parents came to perceive themselves as the actual service users, revealing a clear gap between the policy ideals promoted by the PAS and the realities of service use in practice. Many parents demonstrated limited awareness of the service’s core principle of the primacy of the choice of persons with disabilities, or viewed this principle as difficult to realize under real-world conditions. In particular, parents emphasized that for persons with cognitive disabilities, the meaningful prioritization of their preferences requires the systematic documentation of communication methods and changes in preferences, as well as an institutional infrastructure that enables such records to be shared among care providers. Furthermore, parents’ accounts highlighted the need for state-level efforts to improve overall service quality and to strengthen communication support systems, so that persons with disabilities can meaningfully choose and use services aligned with their own preferences rather than relying on proxy decision-making.
This study examined parental decision-making patterns within service utilization trajectories of the PAS during the early life-course stage through a mixed-methods design, demonstrating that parents function not merely as contextual actors but as substantive agents in the service utilization process alongside persons with disabilities. By doing so, the study extends the role of parents—previously positioned only within the informal sphere—into the set of formal actors within the social service delivery system and reveals that user-centered services designed around the primacy of the choice of persons with disabilities may, in practice, operate through parents’ "proxy choice." These findings call for a critical reconsideration of the concept of choice within care services. The results further show that service utilization decisions cannot be sufficiently explained by individual characteristics alone, suggesting the need to extend the Andersen Behavioral Model to more fully incorporate the service decision-making roles of informal caregivers. Beyond theoretical contributions, this study highlights the necessity of a service delivery system in which parental judgment does not substitute for, but is recalibrated to support, the choice of persons with disabilities. To this end, the study proposes policy and practice directions that include shared decision-making structures, legal and practical mechanisms to reflect users’ preferences and intentions, and the repositioning of parents’ roles within the service system. Together, these contributions provide a concrete foundation for translating the policy ideals of user-centered services into practice and identify key directions for future research aimed at enhancing the quality and sustainability of care service delivery systems.
