한국사회에서 미혼모는 심리사회적 고립, 제한된 자원, 경제적 어려움, 사회적 낙인 등 복합적인 문제에 직면하며 이는 초기 산후 회복과 모성적응을 저해한다. 기존 대면 프로그램은 시간·장소의 제약과 개인정보 노출 우려 등의 문제로 참여가 제한되는 한계가 있어, 메타버스 기반 중재가 대안으로 주목된다. 본 연구는 미혼모의 초기 모성적응을 지원하기 위해 SMILE-MOM (Single Moms in the Metaverse for Interaction, Learning, and Encouragement) 프로그램을 개발하고 타당성을 검증하고자 하였다.
본 연구는 Seels와 Richey의 ADDIE (Analysis, Design, Development, Implementation, Evaluation) 교수 설계 모형 중 분석(Analysis), 설계(Design), 개발(Development)의 3단계를 적용한 방법론적 연구로 진행되었다. 분석 단계에서는 통합적 문헌고찰을 통해 국내외 미혼모 대상 모성 관련 프로그램과 메타버스 활용 간호교육 프로그램의 특성을 분석하였으며, 영유아(만 6세 이하)를 양육하는 미혼모 10명을 대상으로 심층면담을 실시하여 교육내용과 전달방식에 대한 요구를 파악하였다. 설계 단계에서는 Mercer의 어머니 되어가기(Becoming a Mother) 모델, Keller의 주의집중, 관련성, 자신감, 만족감(Attention, Relevance, Confidence, Satisfaction) 모델과 Picciano의 온라인 교육을 위한 다중양식 모델(Multimodal Model for Online Education)을 통합한 이론적 기틀과 분석 단계의 결과를 반영하여 교육 목표, 교수·학습전략, 플랫폼 구성 방안을 마련하였다. 개발 단계에서는 설계안을 기반으로 교육자료와 시나리오 기반 온라인 게임(영아의 안전, 영아의 신호)을 포함한 학습 자료를 제작하고, 컨퍼런스 룸, 가정집, 도서관, 운동실, 상담실, 카페 등 주요 기능별 공간으로 구성된 ZEP 플랫폼 기반 운영 환경을 구축한 후 전문가 평가를 진행하였다.
최종 프로그램은 주 1회, 총 4회기의 소그룹 교육 세션(산후 준비와 회복, 영아 질환 및 안전, 양육 및 모아상호작용, 정서적 웰빙)과 부가 지원 서비스(정보 제공 서비스, 개별 상담 서비스, 산후 운동 격려 서비스)로 이루어졌다. 프로그램의 내용타당도는 6명의 모성·아동 건강 전문가에 의해 평가되었으며(Content Validity Index), 모든 문항에서 0.80 이상의 점수를 받았다. 사용성은 확장현실(Extended Reality) 간호교육 전문가 4명이 시스템 사용성 척도(System Usability Scale)로 측정하여 평균 85.6점으로 ‘높음’ 수준을 보였다. 프로그램 개발 과정은 건강 중재 연구 보고 기준인 보건의료 중재개발 연구 보고 지침(Guidance for Reporting Intervention Development Studies in Health Research)에 따라 체계적으로 평가되었으며, 전문가 피드백을 반영하여 최종 SMILE-MOM이 완성되었다.
본 연구에서 개발된 SMILE-MOM은 미혼모의 초기 모성 적응과 심리·사회적 지지를 촉진하기 위해, 이론적 근거와 요구분석을 바탕으로 설계된 메타버스 기반 프로그램이다. 이러한 특성 덕분에 시·공간 제약 없이 참여할 수 있으며, 사회적 낙인을 완화하고, 실습 중심의 상호작용을 통해 학습 효과와 참여 지속성을 높일 수 있다. 이는 취약 집단을 위한 디지털 간호중재의 확장 가능성을 보여주며, 향후 실제 적용과 효과 평가, 모듈 확장 및 다양한 집단으로의 적용이 요구된다.

Background:
Single mothers in South Korea often confront multiple challenges—including psychosocial isolation, limited resources, economic strain, and stigma—that interrupt postpartum recovery and hinder adaptation to early motherhood. Traditional face-to-face programs remain difficult to access due to constraints related to time, location, and concerns regarding personal information disclosure. To address these limitations, this study developed and validated SMILE-MOM (Single Moms in the Metaverse for Interaction, Learning, and Encouragement), a metaverse-based support program tailored to the needs of single mothers in early motherhood.
Methods:
This methodological study applied the first three phases of the ADDIE instructional design model—Analysis, Design, and Development. During the analysis phase, an integrative literature review was conducted to examine existing maternal support programs and metaverse-based nursing interventions. In-depth interviews were then carried out with ten single mothers raising children under six years of age to identify program needs and preferences. The design phase integrated Mercer’s Becoming a Mother framework, Keller’s ARCS motivational model, and Picciano’s Multimodal Model for Online Education to develop learning goals, instructional strategies, and the structure of the metaverse platform. During the development phase, learning materials, scenario-based online games, and the virtual environment within the ZEP platform were produced, followed by expert review to evaluate content validity and system usability.
Results:
The completed SMILE-MOM program consists of four weekly small-group sessions (maternal preparation and postpartum recovery, infant illness and safety, parenting and mother–infant interaction, and emotional well-being) and supplementary support services, including tailored information services, individual consultation, and postpartum exercise encouragement. Content validity assessed by six experts demonstrated CVI values exceeding 0.80 across all items. Usability evaluated by four extended-reality nursing education specialists yielded a mean System Usability Scale score of 85.6, indicating high usability. Program development procedures were systematically confirmed using the GUIDED reporting framework, and expert feedback was incorporated to refine the final version.
Conclusions:
SMILE-MOM is a theoretically grounded, needs-based metaverse intervention designed to promote early maternal adaptation and psychosocial well-being among single mothers. By enabling participation without temporal or geographical limitations and providing psychologically safe, interactive learning experiences, the program demonstrates potential as a digital nursing intervention for vulnerable populations. Future research should examine its effectiveness in real-world settings, expand modular components, and test applicability among diverse target groups.
