대학생 시기는 사회적으로 음주가 공식적으로 처음 허용되는 시기로, 이 시기의 과도한 알코올 사용과 그로 인한 부정적 결과는 성인 중·후기까지 지속적인 영향을 미칠 수 있다. 선행 연구에 따르면 대학생의 문제 음주는 생활 스트레스와 인지적 위험요인인 음주 기대와 밀접하게 관련된다. 그러나 많은 선행 연구들은 알코올 사용과 부정적 음주 결과를 명확히 구분하지 않아, 음주로 인한 부정적인 결과 자체에 초점을 둔 연구가 상대적으로 부족하였다. 본 연구의 목적은 알코올 사용과 부정적 음주 결과를 명확히 구분하여 대학생의 생활 스트레스, 긍정적 및 부정적 음주 기대, 알코올 사용, 부정적 음주 결과 간의 관계를 규명하고, 음주 기대에 초점을 맞춘 글쓰기 개입이 대학생의 알코올 사용과 부정적 음주 결과에 미치는 효과를 검증하는 데 있다.
연구 1에는 대학생 203명이 참여하였으며, 음주 사용 패턴 척도, 알코올 사용 장애 척도(AUDIT-K), 대학생 생활 스트레스 척도, 음주 효과 기대 척도, 한국판 럿거스 알코올 문제 척도(K-RAPI)를 사용하여 자기보고식 자료를 수집하였다. 상관분석 결과, 부정적 음주 결과는 대학생의 생활 스트레스, 긍정적 음주 기대, 부정적 음주 기대와 모두 정적 상관을 나타냈다. 알코올 사용, 학기 수, 성별을 통제한 상태에서 대학생의 생활 스트레스와 부정적 음주 결과의 관계는 통계적으로 유의한 것으로 나타났으며, 이 관계에서 긍정적 음주 기대와 부정적 음주 기대의 매개 효과가 유의하게 나타났다. 다만, 성별에 따른 추가적인 분석 결과 음주 기대의 매개 효과는 남성 대학생에서는 나타나지 않았으며, 여성 대학생에서만 긍정적 음주 기대의 매개 효과가 확인되었다.
연구 2에서는 대학생 60명을 대상으로 문제 음주를 개선하기 위한 음주 기대 초점 글쓰기 개입의 효과성을 검증하였다. 참여자들은 ‘긍정적 음주 기대-음주 결과 불일치 글쓰기’, ‘음주의 손익-의사결정 균형 글쓰기’, ‘표현적 글쓰기 통제집단’ 중 한 집단에 무선 할당되어 1주일간 온라인을 통해 총 3회의 글쓰기 과제를 수행하였다. 효과 검증을 위해 사전, 사후, 1주일 후 추후 시점에 걸쳐 음주량, 음주 빈도, 음주 기대 및 음주 결과를 측정하였다. 연구 결과, 처치 집단과 측정 시기 간의 상호작용 효과는 통계적으로 유의하지 않았다. 그러나 집단 내 시점별 변화를 분석한 결과, 모든 집단에서 시간 경과에 따라 음주 빈도가 유의하게 감소하였다.
종합하면, 본 연구는 대학생의 문제 음주를 알코올 사용과 부정적 음주 결과로 분리하여 검토함으로써, 생활 스트레스가 알코올 사용뿐만 아니라 부정적 음주 결과와도 밀접하게 연관됨을 검증하였고, 생활 스트레스와 부정적 음주 결과 간 관계에서 긍정적 및 부정적 음주 기대의 매개 역할을 확인하여, 음주 기대가 대학생의 문제 음주에 주요한 인지적 위험요인임을 확인하였다. 또한 음주 기대 초점 글쓰기 단기 개입의 효과성을 탐색하였다는 데 의의가 있다. 마지막으로 본 연구의 의의와 한계, 추후 연구를 위한 제언을 논의하였다.

The college years represent a critical period when alcohol consumption is first socially sanctioned. Excessive alcohol use during this stage, along with its associated negative consequences, can exert a long-lasting impact into middle and late adulthood. While previous research has linked problematic drinking among college students to life stress and cognitive risk factors—specifically alcohol expectancies—many studies have failed to clearly distinguish between 'alcohol use' and 'negative drinking consequences.' Consequently, research focusing specifically on the adverse outcomes of drinking has been relatively sparse.
The purpose of this study was to clearly differentiate between alcohol use and negative drinking consequences to examine the structural relationships among life stress, positive and negative alcohol expectancies, alcohol use, and negative drinking consequences. Furthermore, the study aimed to verify the effectiveness of an alcohol expectancy-focused writing intervention in reducing alcohol use and its negative consequences among college students.
In Study 1, data were collected from 203 college students through self-report scales measuring alcohol use patterns, alcohol use disorders (AUDIT-K), life stress, alcohol expectancies, and alcohol-related problems (K-RAPI). Correlation analysis revealed that negative drinking consequences were positively associated with life stress, as well as both positive and negative alcohol expectancies. Even after controlling for alcohol use, semester enrolled, and gender, the relationship between life stress and negative drinking consequences remained statistically significant. Both positive and negative alcohol expectancies significantly mediated this relationship. However, gender-stratified analyses indicated that this mediation effect was not present in male students; for female students, only the mediating role of positive alcohol expectancy was confirmed.
In Study 2, the effectiveness of an alcohol expectancy-focused writing intervention was tested with 60 college students. Participants were randomly assigned to one of three groups: ‘Positive Expectancy-Drinking Consequence Discrepancy Writing,’ ‘Decisional Balance (Pros/Cons) Writing,’ or an ‘Expressive Writing’ control group. They completed three online writing sessions over one week. Measures were taken at pre-test, post-test, and a one-week follow-up. The results showed no significant interaction effect between the treatment groups and the measurement periods. However, within-group analysis revealed that drinking frequency significantly decreased over time across all three groups.
In conclusion, by separating problematic drinking into alcohol use and negative drinking consequences, this study verified that life stress is closely linked to both aspects. It also confirmed the mediating roles of positive and negative alcohol expectancies, identifying them as key cognitive risk factors. Additionally, this study explored the potential of a short-term, expectancy-focused writing intervention. Finally, the clinical implications, limitations, and suggestions for future research are discussed.
