구역 반사를 유발하는 다양한 요인은 국소적, 의학적, 사회적, 심리적, 의인적, 치과적 요인으로 분류할 수 있다. 국소적 요인에는 비중격 만곡, 코막힘, 후비루, 부비동염이 포함되고, 의학적 요인에는 만성 위염, 위암 및 췌장암, 부분 위절제술이 포함되고, 사회적 요인에는 과민증으로 인한 과도한 흡연, 기침, 만성 카타르, 만성 알코올 중독이 포함된다. 심리적 요인에는 스트레스, 공포증, 알코올 중독, 두려움이 포함되며, 의인적 요인에는 물 및 흡입관, 기구, 국소 마취, 방사선 촬영이 포함된다.

2. 방 법

서울대학교 치과병원 의료정보시스템을 이용하여 2002년 1월 1일부터 2018년 11월 30일까지 구역반사로 인해 전신마취 하에 특수진료실에서 수술을 받은 환자를 검색했다. 마취 전 평가서, 마취 기록, 회복실 기록 등을 열람할 수 있는 환자를 연구 대상으로 선정했다. 일부 환자에서 직접 면담을 통해 구역반사의 원인과 과정, 그리고 구강 상태 평가를 시행하였다.
3. 결 과
총 58증례, 36명의 환자가 구역반사로 인한 치과 치료를 위해 전신마취를 받았고, 평균연령은 36.4세였으며, 남성이 28명, 여성이 8명 이었다. 전신마취의 평균 소요시간은 3시간 22분이었다.

The various factors that trigger the gag reflex can be categorized into local, medical, social, psychological, iatrogenic, and dental factors. Local factors include a deviated septum, nasal obstruction, postnasal drip, and sinusitis. Medical factors include chronic gastritis, gastric and pancreatic cancer, and partial gastrectomy. Social factors include excessive smoking due to anaphylaxis, cough, chronic catarrh, and chronic alcoholism. Psychological factors include stress, phobias, alcoholism, and fear. Iatrogenic factors include water and suction tubes, instruments, local anesthesia, and radiography. Dental factors include poor retention, denture surface finish, inadequate palatal sealing, limited tongue space within the denture, and overly expanded margins.
The estimated prevalence of the gag reflex during dental treatment is approximately 8%. Several studies have shown a strong correlation between dental anxiety and excessive gagging. Some studies suggest that the comorbidity of nausea and dental anxiety may be a consequence of nausea acting as an indirect expression of anxiety. This study aimed to analyze the underlying causes and oral conditions of patients who underwent dental treatment under general anesthesia due to severe gag reflex-induced coordination impairment at Seoul National University Dental Hospital.
2. Methods
We collected patient medical histories, dental treatment history, family history, habits, personal experiences, and physiological and psychogenic triggers through chart review and interviews. Using the Seoul National University Dental Hospital Medical Information System, we searched for patients who underwent surgery under general anesthesia in a specialized treatment unit due to gag reflex from January 1, 2002, to November 30, 2018. Patients with access to pre-anesthetic assessment forms, anesthesia records, and recovery room records were selected for the study. In some patients, we conducted face-to-face interviews to determine the cause and course of the gag reflex and to assess their oral condition.
3. Results
A total of 58 cases (36 patients) underwent general anesthesia for dental treatment due to gag reflex impairment. The average age was 36.4 years, with 28 males and 8 females. The average duration of general anesthesia was 3 hours and 22 minutes.
In the cases presented in the paper, the gag reflex was caused by a combination of psychological trauma, anxiety, and oral hypersensitivity, often exacerbated by past negative dental experiences. Despite various precipitating factors, all subjects experienced difficulty tolerating oral stimulation, which led to avoidance of dental treatment and incomplete self-cleaning. Consequently, their overall oral hygiene was poor. Characteristics included short brushing times, inability to use dental floss, interdental brushes, or tongue cleaners, and reliance on mouthwash. This suggests that the gag reflex significantly impacts daily oral hygiene and professional dental care.
4. Conclusion
The precipitating factors for the gag reflex were diverse. In all cases, the subjects experienced difficulty tolerating oral stimulation, which led to avoidance of dental treatment and incomplete self-cleaning, resulting in poor oral hygiene overall.
