    근위축성측삭경화증(ALS)은 유전학 및 분자 병리학 등 다양한 분야에서 활발히 연구되고 있으며, 병변이 대뇌 피질에서 시작되어 전신으로 퍼져나간다는 가설이 지배적이다. 본 연구는 ALS의 주요 임상적 특징인 근육 약화에 주목하여, 정량적 근력 데이터를 이용해 전신에 걸친 근육 부위 간의 의존성 네트워크를 규명하고자 한다. 이를 위해 EMPOWER 임상시험 데이터를 활용하여 16개 좌우 근육 부위 간의 관계를 모델링하는 베이지안 네트워크 프레임워크를 제안하였다. 시계열 데이터의 복잡성과 결측치 문제를 해결하기 위해 근력 변화량을 분석 변수로 사용하였으며, 해당 데이터가 영 과잉 분포를 따르는 점을 고려하여 Hurdle 모형을 적용하였다. 추정된 네트워크에서 인접 부위로의 전파와 한쪽 사지 안의 여러 근육군이 균일하지 않게 위축되는 현상(split limb phenomena) 등 임상적으로 관찰되는 주요 증상을 확인하였다. 또한 파라미터 추정을 통해 조건부 확률 기반의 추론을 가능하게 하였으며, 교차 검증을 통해 모형의 예측 성능을 입증하였다. 향후 연구로는 근육 기능이 보존되어 변화가 없는 경우와 비가역적 퇴행으로 인하여 변화가 없는 경우를 구분하여 모델링하는 방안, 동적 베이지안 네트워크의 적용, 그리고 환자의 표현형에 따른 하위 그룹 분석 등이 제안된다.

    Amyotrophic lateral sclerosis (ALS) is being actively studied in various fields such as genetics and molecular pathology, and the hypothesis that ALS begins in the cerebral cortex and spreads throughout the body has gained increasing attention. This study focuses on muscle weakness, the primary clinical feature of ALS, and aims to identify dependency networks among muscle regions across the whole body using quantitative muscle strength data. To this end, a Bayesian network framework was proposed to model relationships between 16 bilateral muscle regions using EMPOWER clinical trial data. To address the complexity and missing data issues in time series data, the change in muscle strength was used as the analysis variable, and a hurdle model was applied to account for the zero-inflated distribution. Major clinically observed patterns were identified, such as propagation to contiguous areas and the split limb phenomenon. In addition, conditional probability-based inference was made possible through parameter estimation, and the predictive performance of the model was verified through cross-validation. Future research directions include modeling distinct data-generating mechanisms underlying zero muscle change due to preserved muscle function versus irreversible degeneration, extending the framework to dynamic Bayesian networks, and conducting subgroup analyses based on patient phenotypes.
