만성 통증은 전 세계적으로 막대한 질병 부담을 초래하며, 외상성 신경 손상 환
자 중 최대 60%가 2년 이상 지속되는 만성 신경병증성 통증을 겪는다고 보고되
어 진다. 그간 통증 연구에서 상당한 진전이 있었음에도 불구하고, 만성 통증에
대한 현재의 치료 전략은 근본적인 기전에 대한 이해 부족과 적절한 전임상 모델
의 부재로 인해 성공적이지 못하며, 결과적으로 효과적인 치료법 개발에 난항을
겪고 있다. 본 연구는 좌골신경 부분적 압착 모델에서 교감신경 및 감각신경의 변
화에 초점을 맞춰 만성 통증의 생체 내 기전 연구를 수행하였으며, 3D 생체 내 말
초신경 플랫폼을 개발함으로써 기존 모델이 가지는 연구의 한계를 극복하는 토대
를 마련하였다.
제 1장에서는 교감신경계의 역할에 초점을 맞춰 부분적 신경 압착 수술 이후
유발되는 통증의 기전을 규명하였다. 손상 유형(절단 vs 압착) 별 등쪽신경절로의
교감신경 발아를 평가하기 위해 면역염색, 유전자 변형 마우스, 화학적 교감신경
차단법을 사용하였다. 연구 결과, 신경 절단 손상의 경우 등쪽신경절 내 뚜렷한
교감신경 발아와 노르에피네프린 수치 상승을 유발하며 교감신경 관여 통증을 생
성하는 반면, 부분적 신경 압착 수술은 교감신경 발아 없이 장기간 지속되는 과민
증을 유발하는 교감신경 비의존성 통증임을 확인하였다.
제2장에서는 통증이 없는 완전 압착 손상 모델과의 비교를 통해, 부분적 압착
모델에서 통증을 매개하는 신경 섬유의 종류를 규명하였다. 섬유 특이적 표지 유
전자 변형 마우스와 선택적 약리학적 비활성화 기법을 사용하여, Nav1.8 발현 통
각 수용체가 손상되지 않고 잔존하여 만성 촉각 과민증을 유발함을 확인하였다.
이 통각 수용체 섬유들을 조기에 제거하자 장기간의 촉각 과민증이 예방되었으며,
이는 잔존 통각 수용체가 부분적 신경 압착 수술 이후 만성 통증의 주요 유발 인자이자 잠재적 치료 표적임을 입증한다.
제 3장에서는 신경병증 기전 검증 및 치료제 테스트를 위한 중개 플랫폼을 제
공하고자 3D 시험관 내 모델링을 구축하였다. 본 연구에서 마우스 일차 배양 감
각신경세포 및 인간 iPSC 유래 감각 뉴런을 활용하여 신경 돌기를 정렬하고, 세
포체와 축삭을 구획하며, 슈반세포 매개 수초화를 형성하는 3D 세포 배양 모델을
구축하였다. 이러한 말초신경 모델에서 탈수초화 약물을 통해 신경병증 질환 특징
을 재현하였으며, 신경 내 NK 세포 침투 분석을 가능하게 함으로써 신경-면역 상
호 작용 연구에 활용될 수 있음을 확인하였다. 확장 가능하고 대용량 스크리닝이
가능한 본 모델은 향후 치료제 개발을 위한 생체 내 및 시험관 내 연구를 연결하
는 역할을 수행할 것으로 기대된다.
종합적으로, 본 연구를 통해 말초신경 손상에 따른 다양한 통증 기전에 대한 기
전적 통찰을 제공하였으며, 말초신경병증 및 통증에 대한 중개 연구를 발전시키기
위한 3D 생체 내 말초신경 플랫폼을 확립하였다.

Chronic pain remains a significant global health burden, and with up to 60%
of traumatic nerve injury patients developing persistent neuropathic pain lasting over
two years. Although substantial progress has been made in pain research, current
therapeutic strategies for chronic pain remain unsuccessful because of the limited
understanding of its underlying mechanisms and the lack of appropriate preclinical
models. I addressed these gaps by investigating the in vivo mechanistic basis of
chronic pain in the partial sciatic nerve crush injury (PCI) model, focusing on
sympathetic neuronal involvement and sensory neuronal changes, and by developing a
3D humanized in vitro translational platform.
In the first chapter, I investigated the mechanisms of pain following PCI by focusing on
the role of the sympathetic nervous system. I examined how different injury types
(transection vs. crush) influence sympathetic sprouting into the dorsal root ganglia
(DRG). Using TH-immunostaining, Phox2b reporter mice, and chemical
sympathectomy, I found that transection injury induced pronounced sympathetic
sprouting and elevated norepinephrine level in the DRG, producing sympathetically
maintained pain, whereas PCI caused long-lasting hypersensitivity without
sympathetic sprouting, indicating sympathetically-independent pain. The second chapter explored the sensory neuronal subtype responsible for the PCIinduced pain model by comparing with non-painful full crush injury. By combining
fiber-specific labeling (Thy1-YFP for Aβ fibers and Nav1.8-tdTomato for nociceptors)
with selective pharmacological silencing, I found that Nav1.8 positive nociceptors
were spared and drove chronic tactile hypersensitivity. Early ablation of these fibers
prevented long-term tactile hypersensitivity, identifying spared nociceptors as key
drivers and potential therapeutic targets for chronic pain.
The third chapter shifts to an in vitro modeling to provide a translational platform for
mechanistic validation and therapeutic testing. I reported a 3D-printed myelinationon-a-chip that aligns sensory neurites, compartmentalizes soma and axons, and
supports Schwann cell–mediated myelination (g-ratio ~0.6) using mouse primary and
human iPSC-derived sensory neurons. The system reproduced disease-like features
upon GGF2-induced demyelination and allowed rapid NK-cell infiltration assays,
supporting its use for neuro-immune interaction studies. Scalable and compatible with
high-throughput screening, this 3D model bridges in vitro and in vivo paradigms for
future therapeutic testing.
Collectively, this research provides mechanistic insights into the diverse pain
mechanisms following peripheral nerve injuries and establishes a humanized 3D in
vitro platform to advance translational research in peripheral neuropathy and pain
