    저용량 시스플라틴의 반복 투여는 세뇨관 손상의 누적을 유발하고 회복되지 못하는 비적응성 재생 과정을 촉진하여, 결국 급성 신손상에서 만성 신손상으로의 질병 진행을 유도한다. 유도 중간엽줄기세포에서 유래한 세포외소포체는 다양한 급성 신손상 모델에서 신보호 효과가 보고된 바 있으나, 저용량 시스플라틴 반복 투여와 같이 손상이 축적되는 환경에서의 치료 효과는 아직 명확히 규명되지 않았다. 본 연구는 유도 중간엽줄기세포에서 유래한 세포외소포체가 저용량 시스플라틴 반복 손상 모델에서의 신손상을 완화할 수 있는지를 평가하고자 하였다.
    C57BL/6 마우스에 시스플라틴을 반복적으로 투여하여 모델을 확립한 후, 유도 중간엽줄기세포 유래 세포외소포체를 각각 200 μg 및 400 μg로 투여하였다. 신장 섬유화, 염증, 세뇨관 손상, 세포사멸, 산화스트레스는 면역염색, 면역블롯, qPCR을 통해 평가하였으며, 세포외소포체에 의해 조절되는 분자 경로를 확인하기 위해 전사체 분석(RNA-seq)을 수행하였다.
    세포외소포체 투여는 반복적 저용량 시스플라틴으로 유도된 신손상을 용량 의존적으로 완화하였다. 특히 400 μg 투여군에서 콜라겐 침착, 세뇨관 손상, 면역세포 침윤, 모세혈관 감소가 두드러지게 개선되었으며, 섬유화 및 염증 마커 역시 유의하게 억제되었다. 전사체 분석 결과, 세포외소포체 치료는 반복적 저용량 시스플라틴 신손상 모델에서 나타난 염증, 면역반응, 세포외기질 관련 경로의 변화와 연관됨을 확인하였다.
    유도 중간엽줄기세포에서 유래한 세포외소포체는 반복적 저용량 시스플라틴 신손상 모델에서의 핵심 병리기전을 완화하였으며, 고용량에서 더욱 강력한 치료 효과를 보였다. 이러한 결과는 세포외소포체가 반복적 저용량 시스플라틴 신손상 모델뿐 아니라 급성 신손상에서 만성 신손상으로의 진행을 억제할 수 있는 유망한 세포 유래 치료 전략임을 시사한다.

    Background: Repeated low-dose cisplatin (RLDC) induces cumulative tubular injury and maladaptive repair, ultimately driving the AKI-to-CKD transition. While iMSC-derived extracellular vesicles (EVs) have shown renoprotective effects in various AKI models, their therapeutic potential in cumulative-injury settings such as RLDC remains unclear. This study evaluated whether EVs derived from induced mesenchymal stem cells (iMSC-EVs) can attenuate RLDC-associated kidney injury.
    Methods: RLDC was induced in C57BL/6 mice by weekly intraperitoneal administration cisplatin administration (9 mg/kg) for five cycles. Mice were treated with iMSC-EVs (200 μg or 400 μg) via intravenous injection at 3-day intervals for five doses. Renal fibrosis, inflammation, tubular injury, cell-death pathways, and oxidative stress were assessed by histology, immunoblotting, immunostaining, and qPCR. Transcriptomic profiling was conducted to identify molecular pathways modulated by EV treatment.
    Results: iMSC-EVs ameliorated RLDC-induced renal injury in a dose-dependent manner. The 400μg group displayed the most pronounced reduction in collagen deposition, tubular damage, immune-cell infiltration and loss of peritubular capillaries. Fibrosis-related markers and inflammatory mediators were significantly suppressed by EV treatment. RNA-seq analysis revealed that EV treatment reversed major RLDC-induced transcriptional programs, particularly inflammatory, immune, and extracellular-matrix–related pathways.
    Conclusion: iMSC-EVs mitigate key pathological features of RLDC and exert the strongest effects at higher doses. These findings support iMSC-EVs as a promising cell-free therapeutic strategy for cumulative cisplatin nephrotoxicity and the AKI-to-CKD transition.
