    Objectives
    Hypothermia has been associated with poor outcomes among patients with sepsis. However, hypothermic sepsis patients exhibit diverse temperature changes during the initial phase of resuscitation, and the prognostic implications of these thermal trajectories remain unclear. This study aimed to classify hypothermic sepsis patients according to body temperature trajectories and to investigate the association between these trajectories and 28-day mortality.

    Methods
    This was a retrospective analysis of prospectively collected registry data from adult patients diagnosed with sepsis or septic shock who presented to the emergency departments of three university-affiliated teaching hospitals between August 2014 and December 2019. Hypothermic sepsis was defined as an initial body temperature below 36 °C. The change in temperature (“delta temperature”) was calculated by subtracting the temperature at presentation (0 hours) from that measured at 6 hours after emergency department admission. Patients were classified into three groups according to delta temperature: Group A (delta temperature ≤ 0 °C), Group B (0 °C < delta temperature ≤ 1 °C) and Group C (delta temperature > 1 °C). The primary outcome was 28-day mortality. Multivariable Cox proportional hazards regression analysis was performed to evaluate the independent association between temperature trajectories and mortality.

    Results
    Among 7344 patients with sepsis or septic shock, 325 hypothermic patients (5.1%) were included in the final analysis. The overall 28-day mortality rate was 36%. Although the initial body temperature did not differ significantly between survivors and nonsurvivors, survivors exhibited higher body temperatures at 6 hours after presentation. The 28-day mortality rates were 53.1%, 36.0%, and 30.0% in Groups A, B, and C, respectively, with a significantly higher mortality in Group A compared with Group C (p < 0.05). After adjustment for age, sex, APACHE II score, malignancy, and initial temperature, Group C demonstrated a 44.2% reduction in 28-day mortality compared to Group A (adjusted hazard ratio 0.558; 95% confidence interval, 0.330-0.941; p = 0.029).

    Conclusions
    In hypothermic sepsis patients, an increase of 1°C or more in body temperature after the first 6 hours after emergency department admission was independently associated with a reduced risk of 28-day mortality. These findings suggest that dynamic temperature trajectories reflect underlying physiologic recovery and may serve as a simple, clinically useful prognostic indicator in the management of sepsis.
