    서론: 근감소증 연하장애는 전신 근감소와 삼킴 근육 약화로 인해 발생하는 노인성 연하장애로, 임상적 중요성에도 불구하고 진단 기준, 평가 방법 등에 대한 합의 및 이해가 부족한 실정이다. 본 연구는 근감소증 연하장애의 진단적 접근과 평가 방법의 현황을 분석하고, 근감소증과 연하장애의 역학적 연관성 및 근감소증 연하장애의 임상적 특성을 규명하기 위해 다각적, 통합적 접근을 시도하였다.
    방법: 본 연구는 세 단계로 수행되었다. 첫째, 체계적 문헌고찰을 통해 근감소증 연하장애 관련 기존 연구들의 진단 기준과 평가 방법을 종합적으로 분석하고, 현재 연구 현황과 그 한계를 규명하였다. 둘째, 미국의 65세 이상 Medicare 수혜자를 대상으로 수집된 대규모 코호트(National Health and Aging Trends Study, NHATS, 2011–2019) 분석을 통해, 가중 콕스 비례위험모형을 적용하여 근감소증이 연하장애 발생에 미치는 장기적 위험을 정량화하였다. 셋째, 단면 연구를 통해 상세불명의 연하장애 환자를 근감소증 연하장애의 진단 알고리즘에 따라 분류하고, 이들의 임상적, 기능적 특성을 심층적으로 분석하였다.

    Introduction: Sarcopenic dysphagia—a swallowing disorder resulting from systemic muscle decline and associated weakness of the muscles involved in swallowing—has emerged as a clinically significant condition among older adults. Despite growing attention, current definitions and diagnostic frameworks remain heterogeneous, and epidemiologic evidence on its progression and clinical phenotype continues to be limited. This dissertation aimed to comprehensively elucidate the diagnostic, epidemiological, and clinical characteristics of sarcopenic dysphagia through a three-part, multi-method investigation: (1) a review of existing diagnostic definitions and criteria for sarcopenic dysphagia, (2) a longitudinal cohort analysis using the National Health and Aging Trends Study (NHATS) to examine the temporal relationship between sarcopenia and dysphagia, and (3) a cross-sectional study to objectively characterize the clinical and functional features of sarcopenic dysphagia.
    Methods: First, a systematic review was conducted to critically examine definitions, diagnostic algorithms, and assessment domains of sarcopenic dysphagia. Second, data from NHATS—a nationally representative cohort of U.S. Medicare beneficiaries aged 65 years and older—were analyzed from Rounds 1 to 9 (2011–2019). A survey-weighted Cox proportional hazards model was applied to evaluate the risk of incident dysphagia among older adults with sarcopenia, adjusting for demographic, clinical, and lifestyle covariates. Third, a cross-sectional study recruited patients with unspecified dysphagia and conducted comprehensive, objective assessments of swallowing and sarcopenia-related parameters, including tongue pressure and videofluoroscopic swallowing study (VFSS). Based on the established diagnostic algorithm, participants were subsequently classified into probable or possible sarcopenic dysphagia, and their clinical and functional characteristics were analyzed.
    Results: The systematic review revealed that the diagnostic algorithm proposed in 2017—defining sarcopenic dysphagia as swallowing impairment resulting from systemic sarcopenia—was the most widely used framework. However, some studies defined sarcopenic dysphagia as the coexistence of sarcopenia and dysphagia, demonstrating inconsistency in diagnostic criteria. Moreover, many studies included patients with underlying conditions such as neurological disorders or head and neck cancer, introducing diagnostic ambiguity when interpreting the causal relationship between sarcopenia and dysphagia. The relationship between sarcopenia and dysphagia—whether they share a direct pathophysiological link or coexist as interrelated conditions—remains to be fully clarified. Across studies, dysphagia was primarily assessed using oral intake levels or self-reported questionnaires, whereas objective assessments of swallowing function were included in only a minority of studies. Direct assessment of sarcopenic changes in swallowing-related muscles was limited.
    In the longitudinal analysis of the NHATS over nine years, dysphagia occurred in 13.62% of participants with sarcopenia and in 11.75% of those without sarcopenia, with a shorter mean time to onset in the sarcopenia group (6.77 vs. 7.12 years; p < 0.001). Kaplan–Meier analysis demonstrated significantly reduced dysphagia-free survival in the sarcopenia group (log-rank p < 0.001). In the weighted Cox proportional hazards model, sarcopenia was identified as an independent risk factor for dysphagia (adjusted hazard ratio, 1.214; 95% CI, 1.211–1.217; p < 0.001). Other significant predictors included older age, male sex, higher comorbidity count, and history of stroke, whereas walking for exercise was associated with a lower risk of dysphagia.

While variations in the conceptualization and diagnostic approaches were observed across studies, the findings underscore the need for standardized diagnostic frameworks and objective assessment systems to enhance clinical applicability. Despite these diagnostic limitations, the longitudinal cohort analysis provided population-based evidence that systemic sarcopenia significantly increases the risk of subsequent dysphagia, laying the groundwork for future investigations into their temporal relationship. Furthermore, the clinical study objectively characterized key clinical features of sarcopenic dysphagia. In the context of the rapid transition to a super-aged society and the increasing socioeconomic burden, sarcopenic dysphagia has emerged as a major concern in geriatric health. For early detection and effective intervention, the advancement of integrated and multidisciplinary strategies that incorporate objective diagnostic indicators and address the complex interrelationships between sarcopenia and dysphagia will be essential.
