    본 연구는 집속 초음파 (Focused Ultrasound, FUS)가 힘줄 치유를 촉진하는 치료적 가능성을 평가하고, 힘줄 손상에 대한 비침습적 치료 방법으로서의 활용 가능성을 탐색하고자 수행되었다. 특히 아킬레스건 병변과 같은 힘줄 손상은 흔한 근골격계 질환으로, 회복 기간이 길고 재손상의 위험이 높다. 현재의 치료 방법은 대부분 침습적이거나 일관된 효과를 보이지 않아, 조직 손상을 최소화하면서 치유를 촉진할 수 있는 대체 치료 전략의 필요성이 제기되고 있다.
    Sprague-Dawley Rat 모델에서 Collagenase IA을 아킬레스건에 주입시켜서 손상 모델을 만들었다. 모델을 만든 지 7일후에 첫번째 그룹을 Sacrifice 시켰고 28일후에 두번째 그룹을 Sacrifice 시킨 다음에 아킬레스건을 채취하여 Hematoxylin & Eosin (H&E) 염색을 통해 조직학적 분석을 실시하였다. 세포 밀도와 신생혈관 점수를 기준으로 정량적 평가를 수행한 결과 Tendinopathy 군과 대조군을 비교했을 때 Tendinopathy를 유도한 힘줄 조직에서 염증세포가 침윤되어 있는 것으로 보아 Tendinopathy모델이 잘 구축됨을 알 수 있었다.
    총 12마리의 생후 13주령 수컷 SD Rat를 이용한 전임상 실험이 수행되었다. 각 Rat의 양측 아킬레스건에 집속 초음파를 적용하였으며, 적용 조건은 주파수 2 MHz, 강도 1 kW/cm², 듀티 사이클 10%, PRF 10 Hz로 설정하였다. 치료는 4~5일 간격으로 총 5회 시행되었으며, 각 세션은 180초 동안 진행되었다. 대조군은 동일한 실험 조건 하에서 FUS를 조사하지 않았다. 마지막 FUS 치료 10일 후, 모든 동물을 Sacrifice 후 아킬레스건을 채취하여 H&E 염색을 통해 조직학적 분석을 실시하였다.
    세포 밀도 및 신생혈관 형성 점수를 기준으로 정량적 평가를 수행하였다. 조직학적 분석 결과, FUS 처리군은 대조군과 비교했을 때 콜라겐 섬유의 배열이 비슷한 정도로 정돈되어 있었고, 염증 소견이 없는 것으로 보아 대조군과 큰 차이가 없음을 확인했다. 이러한 결과를 통해 집속 초음파 치료를 위해 조직학적으로 안전한 초음파 조건을 확립함을 알 수 있었다. 이 전임상 실험이 안전하다는 것을 확인했으므로 추후 본 연구를 통해 확립한 초음파 조건을 이용하여 추가 실험을 통해 세포학적 개선을 유도하여 힘줄 재생을 효과적으로 촉진하는 연구를 할 필요가 있다. FUS의 비침습적 특성과 긍정적인 조직학적 변화는 힘줄 손상 치료에 있어 새로운 치료 방법으로서의 가능성을 제시한다. 본 연구는 향후 재생의학 및 근골격계 치료 분야의 임상 응용을 위한 기초 데이터를 제공하는 전임상적 근거로서 의의를 가진다.

    This study was conducted to evaluate the therapeutic potential of focused ultrasound (FUS) in promoting tendon healing and to explore its applicability as a non-invasive treatment modality for tendon injuries. Tendon disorders such as Achilles tendinopathy are common musculoskeletal conditions characterized by long recovery periods and high risk of re-injury. Current treatment options are largely invasive or show inconsistent outcomes, highlighting the need for alternative therapeutic strategies that can minimize tissue damage while enhancing the healing process.
    A tendon injury model was established in Sprague–Dawley (SD) rats by injecting Collagenase IA into the Achilles tendon. The first group was sacrificed 7 days after model induction, and the second group was sacrificed at 28 days. Achilles tendons were harvested and subjected to histological evaluation using Hematoxylin & Eosin (H&E) staining. Quantitative assessment based on cell density and neovascularization scores revealed notable infiltration of inflammatory cells in the tendinopathy group compared with the control group, confirming successful establishment of the tendinopathy model.
    A preclinical experiment was then conducted using twelve 13-week-old male SD rats. FUS was applied to both Achilles tendons of each rat under the following parameters: frequency 2 MHz, intensity 1 kW/cm², duty cycle 10%, and pulse repetition frequency (PRF) 10 Hz. Treatments were administered five times at 4–5-day intervals, with each session lasting 180 seconds. The control group underwent identical procedures without FUS exposure. Ten days after the final FUS treatment, all animals were sacrificed, and Achilles tendons were collected for H&E-based histological evaluation.
    Quantitative analyses were performed based on cell density and neovascularization. Histological results showed that the FUS-treated group exhibited similarly well-organized collagen fiber alignment and no inflammatory findings compared to the control group, indicating no significant difference between groups. These findings demonstrate that the ultrasound parameters used in this study are histologically safe for FUS treatment. Having confirmed the safety of these conditions in a preclinical setting, further studies using the established parameters are warranted to investigate cellular-level improvements and the potential of FUS to effectively promote tendon regeneration.
    The non-invasive nature of FUS and its favorable histological profile suggest its potential as a novel therapeutic method for tendon injuries. This study provides foundational preclinical evidence that may contribute to future clinical applications in regenerative medicine and musculoskeletal therapy.
