배경: 간세포암이 대사이상을 기저로 발생하는 경우가 점차 증가하고 있다. 그러나 장 염증이 간의 면역 환경을 어떻게 변화시켜 종양 발생을 촉진하는지는 명확히 규명되지 않았다. 본 연구는 식이로 유도된 간 종양 발생 모델에서 만성 장염이 간세포암의 진행을 가속화하는지를 규명하고, 장–간 축을 매개로 한 장내미생물–숙주 면역 상호작용을 분석하고자 하였다.
방법: 수컷 C57BL/6 마우스에 choline-deficient, L-amino acid–defined high-fat diet (CDAHFD)를 19, 24, 또는 31주 동안 급여하였으며, CDAHFD 단독군(대조군)과 주기적 1% dextran sulfate sodium (DSS) 투여군(DSS군)으로 무작위 배정하였다. 대장과 간의 병리학적 변화, 혈청 생화학 검사, 간 면역세포 아형 분석, 분변 미생물군 및 기능예측 분석을 수행하였다.
결과: DSS 투여군은 대조군에 비해 간 종양 부하가 현저히 증가하였다. 지방변성은 두 군 간 유사하였으며, 염증 관련 지표 또한 큰 차이를 보이지 않았다. DSS는 조직학적으로 경도의 염증 소견에도 불구하고, ZO-1 단백질의 단절 및 소실을 동반한 만성적이고 저등급의 장 염증을 유도하였다. 31주 시점에서 간 내 대식세포는 M2 표현형으로의 편향을 보였다. 장내미생물 분석에서는 α-다양성(Shannon/Simpson, p = 0.015)의 유의한 감소 및 β-다양성(PERMANOVA, p = 0.014)의 군집 분리를 보였으며, 점액분해균인 Akkermansia의 현저한 증식과 부티르산 생성균 Blautia의 감소가 관찰되었다. ANCOM-BC와 Random Forest 분석을 통해 이러한 분류학적 변화를 확인하였고, 네트워크 분석에서는 Akkermansia가 주요 허브로 작용하며 공생균 군집을 대체하는 양상을 보였다. PICRUSt2 기반 기능예측 분석에서는 대사경로 수준의 비율은 유사하였으나, 포도당-6-인산가수분해효소(glucose-6-phosphatase) 등 특정 KEGG ortholog 수준에서 차이를 보였다.
결론: 만성 장 염증은 장 투과성 증가 및 지속적인 장내 미생물 불균형(dysbiosis)을 초래하고, 이를 통해 간 대식세포의 M2 우세 면역 환경을 재편성함으로써 식이 유도 간세포암의 발생을 촉진하였다. 이러한 과정은 전형적인 염증성 사이토카인 상승과는 독립적으로 작용하였다. 따라서 장–간 축 내 미생물군 조성, 장 장벽의 항상성, 대식세포 극성화 조절은 장내 미생물 불균형에 의한 간 발암 기전을 완화시킬 수 있는 잠재적 치료 표적이 될 수 있다.

Background: Hepatocellular carcinoma (HCC) increasingly arises on a background of metabolic dysfunction. How intestinal inflammation shapes hepatic immunity to promote tumorigenesis remains unclear. We investigated whether chronic colitis accelerates HCC in a diet-induced model and delineated microbiome–immune links along the gut–liver axis.
Methods: Male C57BL/6 mice were fed a choline-deficient, L-amino acid–defined high-fat diet (CDAHFD) for 19, 24, or 31 weeks and randomized to CDAHFD alone (control) or CDAHFD plus cyclic 1% dextran sulfate sodium (DSS). Colon and liver pathology, serum biochemistry, hepatic immune cell phenotypes, and fecal microbial analysis with functional predictions were performed.
Results: DSS-treated group showed markedly increased hepatic tumor burden. Steatosis was comparable with similar inflammatory marker levels. DSS induced chronic, low-grade colitis with disrupted ZO-1 despite modest H&E activity. Hepatic macrophages shifted toward an M2 phenotype at 31 weeks. Microbial profiling showed reduced α-diversity (Shannon/Simpson p = 0.015) and distinct β-diversity (PERMANOVA p = 0.014), with dramatic Akkermansia expansion and near-depletion of butyrate-producer Blautia. ANCOM-BC and Random Forest analysis confirmed these discriminatory taxa; network analysis indicated Akkermansia as a central hub replacing commensal communities. PICRUSt2 revealed KO-level differences (e.g., glucose-6-phosphatase) despite similar pathway proportions.
Conclusion: Chronic intestinal inflammation establishes increased gut permeability and persistent dysbiosis that reprograms hepatic macrophages toward an M2-dominant, tumor-permissive milieu and accelerates HCC on CDAHFD, largely independent of overt inflammatory upregulation. Targeting microbiome composition, barrier integrity, and macrophage polarization may mitigate dysbiosis-driven hepatic carcinogenesis.
