    세포이식은 손상된 조직의 기능을 회복하고 체내 항상성을 재구축할 수 있는 가장 유망한 재생의학적 접근법 중 하나이다. 특히 췌도 이식은 제1형 당뇨병 환자에서 내인성 인슐린 조절을 회복할 수 있는 잠재력을 보여주었으며, Edmonton protocol을 통해 임상적 타당성이 입증되었다. 그러나 이식 직후 즉각적으로 발생하는 혈액매개 염증반응 (IBMIR)은 이식된 세포의 50% 이상을 초기에 파괴시켜 초기 생착률을 낮추고, 장기적 치료 효과를 저하시키는 주요 장애요인으로 작용한다. 또한, 이식된 세포가 생체 내에서 비정상적으로 증식하거나 원치 않는 호르몬을 분비하는 등 부작용이 발생했을 때 이를 신속하게 제어하거나 제거할 수 있는 안전장치(safety switch)가 부재하다는 점도 임상 확장성을 제한하는 중요한 문제이다. 본 연구는 이러한 두 가지 핵심 난제를 해결하기 위한 이식세포 보호 및 외부 제어 기반의 이중 기능 플랫폼을 개발하는 데 목적을 두었다.
    먼저, 생체적합 고분자 기반의 나노 두께의 캡슐화 기술을 확립하여 초기 혈액매개 염증반응 완화를 목표로 하였다. 고분자 적층과 효소적 가교를 통해 구축된 캡슐화 층은 항응고 및 면역조절 기능을 부여하여 혈액 접촉에 의한 응고 및 트롬빈 활성을 효과적으로 억제하였다. 캡슐화된 췌도는 정상적인 포도당 자극 인슐린 분비능을 유지하였고, 제1형 당뇨 랫드 모델에서도 장기간 혈당 조절을 안정적으로 달성하였다. 더 나아가 비글 모델에서도 이식된 캡슐화 췌도가 외인성 인슐린 요구량을 감소시키는 등 임상적으로 의미 있는 기능 회복을 보였다. 이는 본 플랫폼이 실제 임상 적용 규모에서도 기능적으로 유효함을 시사한다.

    이와 병행하여, 본 연구는 모노페놀기가 도입된 그래핀을 캡슐화 플랫폼에 도입함으로써 외부에서 선택적으로 세포를 제거할 수 있는 광열 효과를 이용한 안전 스위치를 구축하였다. 근적외선 (NIR) 조사 시 그래핀 코팅 부위에서 국소적 열 상승이 유도되어 베타세포 및 부신피질 내분비 세포 스페로이드에서 강력한 세포사멸이 발생하였다. 또한 인슐린 및 코르티코스테론 분비 기능이 완전히 소실되면서, 본 안전 스위치가 단순한 세포 사멸 유도뿐 아니라 내분비 기능 자체를 효과적으로 차단할 수 있음을 확인하였다. 피부 투과 실험에서도 NIR 레이저가 실제 피부조직을 관통해 안전 스위치를 활성화할 수 있음을 검증하였다.
    종합하면, 본 플랫폼은 세포이식의 초기 염증반응 억제와 이식세포의 외부 제어라는 두 가지 임상적 난제를 동시에 해결할 수 있는 새로운 전략을 제시한다. 향후 줄기세포 유래 치료제와의 융합, 대동물 장기 안전성 연구, 임상용 NIR 시스템 개발을 통해 실제 임상 전환 가능성을 크게 높일 수 있을 것으로 기대된다.

    Islet transplantation has emerged as a promising therapeutic strategy for type 1 diabetes mellitus (T1D), offering the potential to restore endogenous insulin secretion and achieve durable glycemic control. Unlike exogenous insulin therapy, which requires lifelong administration and cannot fully replicate physiologic glucose homeostasis, the transplantation of functional pancreatic islets provides a physiologic source of insulin, glucagon, and other regulatory hormones. The Edmonton protocol and subsequent refinements have demonstrated the clinical feasibility of intraportal islet infusion, with many recipients achieving insulin independence for variable periods. However, despite these advances, broad clinical application remains hindered by several challenges. Early graft loss mediated by instant blood-mediated inflammatory reaction (IBMIR), alloimmune rejection, limited donor availability, and the need for long-term immunosuppression severely compromise transplant outcomes. In particular, the acute destruction of a large fraction of islets immediately after transplantation represents a critical barrier to consistent and long-lasting therapeutic success, necessitating innovative approaches in graft protection, immune modulation, and biomaterial engineering.
    To overcome these challenges, a layer-by-layer (LbL) encapsulation strategy using biocompatible polysaccharides and enzymatic crosslinking chemistry was developed in this thesis. This nano-thin hydrogel film allowed conformal coating of individual islets while maintaining nutrient and oxygen permeability. Importantly, anticoagulant heparin was incorporated into the outermost layer to mitigate IBMIR by inhibiting thrombin activation and reducing early clot formation. In rodent and canine transplantation models, this heparin-functionalized encapsulation significantly prolonged graft survival and reduced the need for exogenous insulin injection, demonstrating that biomaterial-based surface modification can effectively modulate the host–graft interface.
    Second, to address the risk of uncontrolled graft proliferation and to provide an additional layer of biosafety, graphene-based safety switch platform was introduced. Monophenol-conjugated graphene were engineered as a photo-thermal responsive coating, enabling selective elimination of transplanted cells upon external near-infrared (NIR) light stimulation. This approach established a controllable mechanism for eliminating dysfunctional or immunogenic grafts without systemic toxicity. In vitro and ex vivo analysis confirmed that the graphene coating didn’t interfere islet viability and insulin secretion under physiological conditions, while permitting rapid and localized graft elimination when triggered. This proof-of-concept demonstrated that the integration of functional nanomaterials can expand the safety profile of cell-based therapies.
    Collectively, these studies highlight a multifunctional strategy to advance islet transplantation for T1D treatment. The LbL encapsulation system with anticoagulant functionalization addresses the immediate inflammatory barriers, the graphene-based safety switch ensures controllable graft removal. Together, these innovations provide a foundation for next-generation cell transplantation therapies that can overcome current clinical limitations and move closer to practical and widespread application in diabetes treatment.
