뇌의 항상성 유지에는 뇌척수액과 간질액 사이의 효율적인 교환이 필수적이며, 이는 아밀로이드-베타, 타우 등 신경독성 대사산물의 제거를 담당하는 핵심 기전이다. 최근 글림파틱 시스템과 수막 림프관의 발견은 뇌 청소 네트워크에 대한 이해를 근본적으로 확장시켰으나, 이들의 정량적 생체 내 평가와 비침습적 조절 방법은 여전히 큰 과제로 남아 있다. 본 논문은 구조적, 기능적, 동적 영상을 통합한 MRI 기반 정량 평가 프레임워크를 구축하고, 여기에 초음파 기반 조절 기법을 결합하여, 전임상과 임상 수준에서 뇌 청소 기능을 특성화하고 향상시키는 통합적 영상 플랫폼을 제안한다.
제1장에서는 알츠하이머병 신경영상 데이터를 활용하여, 인간에서의 뇌 청소 기능 관련 변화 양상을 특성화하기 위한 다중모달 MRI 접근법을 제시하였다. 확산텐서영상 기반의 알프스 지표, 분획 이방성, 평균 확산도와 휴지상태 fMRI 기반의 전뇌 볼드–뇌척수액 커플링 지표를 통합 분석하였다. 건강한 대조군에서는 미세구조적 무결성, 혈관주변 확산, 뇌척수액 맥동 간의 유기적 상호작용이 뚜렷하게 관찰되었으나, 경도인지장애 및 알츠하이머 환자군에서는 이러한 상관성이 현저히 약화되었다. 본 결과는 알프스 지표, 분획 이방성, 평균 확산과 전뇌 볼드–뇌척수액 커플링의 연관성이 뇌 청소 관련 변화를 반영하는 비침습적 MRI 바이오마커로 활용될 가능성을 보여주었다.
제2장에서는 전임상 모델을 통해 저강도 집속 초음파가 뇌척수액 순환과 글림파틱–수막 림프 배출을 기계적으로 촉진할 수 있음을 입증하였다. 시뮬레이션 및 팬텀 실험을 통해 기울어진 입사 초음파가 효과적인 복사력을 생성함을 확인하였으며, 이를 이용하여 횡정맥동 주변의 수막 림프관 영역에 스위핑 방식으로 자극을 가하였다. 이러한 초음파 자극의 효과를 정량적으로 평가하기 위해 동적 조영증강 MRI 분석을 수행하였으며, 뇌의 영역 및 복셀 단위 분석 결과, 초음파 스위핑 후 뇌실 근처 영역에서 뇌척수액 유입 속도의 증가, 최고 신호 도달 시간의 단축, 청소율의 상승이 관찰되었. 또한, 유도결합플라즈마 질량분석법을 통해 뇌 조직 내 잔류 조영제의 감소가 확인되어, 초음파 자극이 뇌 유체역학을 비침습적으로 조절하고 뇌 청소 기능을 촉진할 수 있음을 입증하였다.
종합적으로, 본 연구는 알프스 지표, 분획 이방성, 평균 확산과 전뇌 볼드–뇌척수액 커플링이 표준 3 T 임상용 MRI 시퀀스만으로도 구현 가능하며, 조영제 주입 없이 재현성 있는 비침습적 정량 바이오마커로 활용될 수 있음을 입증하였다. 반면, 조영제 기반 동적 MRI 기법은 전임상 모델에서 뇌척수액 순환 및 글림파틱–수막 림프 배출을 직접 시각화하고, 그 기전적 근거를 제시하는 데 유용하였다. 또한, 초음파 기반 유체역학적 조절은 글림파틱 효율을 향상시킬 수 있는 비침습적 치료 전략으로서의 가능성을 보여주었다. 결과적으로, 본 논문은 MRI 기반 정량 평가와 물리적 조절 전략을 결합한 전임상–임상 연계 영상·치료 프레임워크를 확립하였다. 이를 통해 뇌 청소 기능의 구조적·기능적 연계를 규명하고, 외부 기계 자극을 통한 청소 효율 향상의 가능성을 제시하였으며, 나아가 뇌 노화 및 신경퇴행성 질환의 조기 진단·예방·치료를 위한 비침습적 영상-치료 융합 기반을 마련하였다.

Maintaining efficient cerebrospinal-fluid (CSF) and interstitial-fluid (ISF) exchange is critical for brain homeostasis and clearance of neurotoxic metabolites such as amyloid-β and tau. The recent identification of glymphatic and meningeal lymphatic systems has redefined this clearance network, but its quantitative in-vivo evaluation and noninvasive modulation remain major challenges. This dissertation proposes an integrative MRI framework that combines structural, functional, and dynamic imaging with ultrasound-based modulation to characterize brain clearance–related processes, bridging preclinical mechanistic studies with clinical MRI observations.
Chapter 1 presents a multimodal MRI approach for characterizing clearance-related alterations in humans using Alzheimer’s Disease Neuroimaging Initiative data. Diffusion-derived indices—fractional anisotropy, mean diffusivity, and the diffusion tensor imaging analysis along the perivascular space (DTI-ALPS)—were integrated with global blood oxygen level dependent (gBOLD)–CSF coupling derived from resting-state fMRI. Significant associations among these metrics in healthy controls indicated coherent interactions between microstructural integrity, perivascular diffusivity, and CSF pulsation, whereas such relationships were attenuated in participants with mild cognitive impairment and AD. These findings demonstrate that coordinated relationships among multiple MRI-derived metrics are a key feature of intact brain clearance physiology, with DTI-ALPS and gBOLD–CSF coupling emerging as complementary clearance-related axes within this interconnected framework.
Chapter 2 extends this framework to a preclinical model, demonstrating that low-intensity focused ultrasound can mechanically enhance CSF circulation and glymphatic–meningeal drainage. Acoustic simulations and phantom experiments verified that angled ultrasound incidence generates effective radiation forces, which were then applied to the mLVs surrounding the transverse sinus in a sweeping manner to accelerate CSF outflow. Dynamic contrast-enhanced MRI was performed to quantitatively evaluate the resulting changes in CSF transport, with region- and voxel-based analyses revealing accelerated inflow, shortened time-to-peak, and increased clearance rate in periventricular regions after ultrasound sweeping. Quantitative tracer analysis confirmed reduced residual tracer in brain tissue, supporting that ultrasound stimulation can noninvasively modulate intracranial fluid dynamics and promote effective brain clearance.
Collectively, this work demonstrates that diffusion- and function-based MRI biomarkers can be implemented using standard 3 T clinical sequences to provide reproducible, contrast-free quantification of brain clearance function. In parallel, dynamic contrast-enhanced MRI, primarily applied in preclinical models, enables direct visualization and kinetic analysis of CSF transport, offering mechanistic validation of glymphatic and meningeal drainage. Furthermore, ultrasound-based hydrodynamic modulation emerges as a feasible and noninvasive strategy to enhance glymphatic efficiency. By integrating MRI-based quantitative evaluation with physical modulation, this dissertation outlines a translational imaging–therapeutic framework that bridges clinical MRI biomarkers with preclinical mechanistic modulation, providing the groundwork for future diagnostic and interventional studies in neurodegenerative diseases.
