    TM4SF5는 간에서 대사, 간엽-상피전환, 섬유화, 암세포 증식, 전이, 그리고 최근에는 면역회피 기전에 관여함으로써 간에서 다양한 역할을 한다고 보고된 바 있다. 최근 TM4SF5는 세포외기질 생성을 증가시키고 간엽-상피전환 유도를 통한 대사기능장애 관련 지방간염(MASH) 진행에서의 섬유화에 기여하기도 하며, 대식세포와의 상호작용을 통해 대식세포의 분극 재구성을 변화시켜 MASH에 유리한 면역 환경을 조성한다고 알려져 있다. 그러나 MASH에서의 TM4SF5 역할이 광범위하게 연구되어 왔음에도 불구하고, TM4SF5에 의해 조절되는 표적 유전자는 아직 규명되지 않았으며, 질환의 이질적인 특성으로 인해 표적 유전자의 규명에는 한계가 있다.
    본 연구는 MASH의 간 상피세포에서 TM4SF5가 KEAP1을 비정상적인 조절을 발견하였으며, 이어서 CD36의 발현 감소가 MASH의 발병 및 진행에 기여함을 확인하였다. 생리학적 활성의 간 상피세포에서 TM4SF5는 KEAP1과 직접 결합하여 KEAP1을 유비퀴틴화함으로써 프로테아좀 경로를 통해 단백질 수준에서 감소시켰다. 시험관 내에서 MASH와 유사한 조건을 만들기 위해, 팔미트산 또는 지질혼합물을 처리하였고, TM4SF5는 지질처리에 의해 MASH 관련 인자들과 활성산소종의 증가를 야기하였고, 간 상피세포 사멸이 증가함을 확인하였다. 흥미롭게도, 생리학적 활성상태와 달리 TM4SF5의 KEAP1 결합이 억제되고 조절이 무력화되어 KEAP1의 발현을 증가시켰다. 또한, 지방성 간질환에서 증가하는 지방산 운반체인 CD36이 TM4SF5가 과발현 상태에서 지질처리에 의해 감소하였다. 이는 TM4SF5와 CD36, KEAP1과 CD36은 서로 결합을 통해 CD36과 KEAP1과 역상관 관계를 나타냈다. 시험관 내 실험결과와 일치하게, 간 특이적 TM4SF5 과발현된 형질전환 마우스에서 고지방식이와 사염화탄소 병용처리를 통해 만든 생체 내 MASH 모델은 야생형 마우스에 비해 병태생리학적 분석에서 뚜렷한 MASH 특징을 보였으며, 동시에 KEAP1의 현저한 증가가 관찰되었다.     TM4SF5 has been reported to play multiple roles in the liver, including the regulation of metabolism, mesenchymal–epithelial transition, fibrosis, cancer cell proliferation and metastasis, and more recently, immune evasion mechanisms. Recent studies have demonstrated that TM4SF5 promotes extracellular matrix production and induces mesenchymal–epithelial transition, thereby contributing to fibrosis during the progression of metabolic dysfunction-associated steatohepatitis (MASH). In addition, TM4SF5 modulates macrophage polarization through its interaction with macrophages, thereby establishing an immune environment favorable for MASH development. Although many studies have investigated the role of TM4SF5 in MASH, the downstream target genes regulated by TM4SF5 remain unidentified, and the heterogeneous characteristics of the disease pose a significant limitation to identifying these target genes.
    This study identified TM4SF5-mediated abnormal regulation of KEAP1 in hepatocytes of MASH and confirmed that this leads to decreased expression of CD36, thereby contributing to the progression of MASH. In hepatocytes under physiological conditions, TM4SF5 directly binds to KEAP1 and ubiquitinates it, leading to a reduction in KEAP1 protein levels through the proteasomal degradation pathway. To mimic MASH-like conditions in vitro, cells were treated with palmitic acid or lipid mixture, and TM4SF5 induced an increase in MASH-associated factors and reactive oxygen species, resulting in elevated hepatocyte death. Interestingly, unlike under physiological conditions, the binding of TM4SF5 to KEAP1 was inhibited and its regulation was disrupted, resulting in increased KEAP1 expression. The fatty acid transporter CD36 was decreased by lipid treatment under TM4SF5 overexpression, showing an inverse correlation with KEAP1. This effect results from the mutual regulation through direct binding between TM4SF5 and CD36, as well as KEAP1 and CD36. A high-fat diet and carbon tetrachloride treatment-induced MASH models in liver-specific TM4SF5-overexpressing transgenic mice exhibited distinct pathophysiological features of MASH compared to wild-type mice, along with a marked increase in KEAP1 expression. The pathological significance of aberrant KEAP1 upregulation in MASH was confirmed by in vivo studies demonstrating that siRNA-mediated knockdown of KEAP1 resulted in a significant attenuation of MASH-associated phenotypes. Correctively, this study revealed that TM4SF5 aberrantly upregulates KEAP1 expression in MASH, which in turn suppresses CD36, thereby exacerbating MASH pathology. These findings suggest that inhibition of TM4SF5 or normalization of the KEAP1-CD36 axis could represent novel therapeutic targets for MASH.
