녹내장은 전 세계적으로 비가역적 실명의 주요 원인이다. 녹내장의 진단 및 관리는 전통적으로 안압(IOP) 측정, 빛간섭단층촬영(OCT)과 같은 장비를 이용한 시신경 유두(ONH) 및 망막신경섬유층(RNFL)의 구조적 평가, 그리고 표준 자동 시야 검사(SAP)를 통한 기능적 평가에 의존해 왔다. 그러나 이러한 전통적인 방법들은 내재적 한계를 가진다. 목적

방법

Glaucoma is a progressive optic neuropathy and a leading cause of irreversible blindness worldwide. The diagnosis and management of glaucoma traditionally rely on the assessment of intraocular pressure (IOP), structural evaluation of the optic nerve head (ONH) and retinal nerve fiber layer (RNFL) using modalities like optical coherence tomography (OCT), and functional assessment via standard automated perimetry (SAP). However, these conventional methods have inherent limitations. SAP results can exhibit significant test-retest variability and often detect damage only after a substantial loss of retinal ganglion cells has occurred. Furthermore, predicting which patients will experience disease progression remains a major clinical challenge, particularly in specific subgroups like myopic normal-tension glaucoma (NTG) patients.
Recently, the advent of artificial intelligence (AI), including machine learning (ML) and deep learning (DL), has offered new paradigms for analyzing complex medical data. These technologies have the potential to integrate high-dimensional data from various sources—such as clinical measurements, OCT scans, and optic disc photographs (ODPs)—to create more robust, accurate, and predictive models, thereby overcoming the limitations of traditional diagnostic and prognostic approaches.
Purpose
The purpose of this series of studies was to develop and validate various artificial intelligence algorithms to enhance the detection, assessment, and prediction of progression in glaucoma. By leveraging different data modalities, this work aimed to demonstrate the feasibility and superiority of AI-based approaches compared to conventional clinical parameters.
This study was performed under three specific aims:
1. To develop a machine learning model to predict future glaucomatous progression in the high-risk subgroup of young, myopic patients with normal-tension glaucoma.
2. To develop a deep learning model for the highly accurate detection of glaucomatous structural changes using SD-OCT images.
3. To develop a deep learning model capable of quantitatively predicting functional loss (SAP Mean Deviation) from structural information captured in monoscopic optic disc photographs.
Methods
Multiple patient cohorts, including healthy participants and glaucoma patients, were enrolled. Data from various sources were collected, including spectral-domain OCT scans (providing GCIPL and RNFL thickness and deviation maps), monoscopic optic disc photographs, and baseline clinical measurements. The data were appropriately partitioned into training, validation, and testing sets for model development and evaluation.
A suite of AI models was constructed. Deep learning classifiers were built using architectures that included pre-trained networks and fully connected layers to analyze image-based data (OCT and ODPs). For tabular clinical data, machine learning classifiers, including Random Forest and Extremely Randomized Trees (Extra-Trees), were employed. The performance of these models was rigorously evaluated using metrics such as the area under the receiver operating characteristic curve (AUC), sensitivity, specificity, correlation coefficients, and mean absolute error (MAE). The results were systematically compared against the performance of established glaucoma diagnostic parameters.
Results
1. Machine Learning-based Prediction of Glaucoma Progression in Young Myopic NTG:
In predicting future progression in young myopic NTG patients, the Extra-Trees machine learning model achieved a high AUC of 0.881. This was significantly more accurate than the Random Forest model (AUC 0.811) and superior to predictions based on any single clinical parameter, including average macular GCIPL thickness (AUC 0.735) and circumpapillary RNFL thickness (AUC 0.691).
2. Deep Learning-based Glaucoma Detection from SD-OCT Images:
The deep learning system, trained on GCIPL and RNFL maps, demonstrated outstanding diagnostic performance. In the test dataset, it achieved an AUC of 0.990 with a sensitivity of 94.7% and a specificity of 100.0%, significantly outperforming conventional OCT thickness parameters and SAP mean deviation (all P < 0.01).
3. Prediction of Functional Loss from Optic Disc Photographs:
The deep learning algorithm successfully predicted the SAP mean deviation (MD) value solely from optic disc photographs. The predicted MD showed a strong correlation (correlation coefficient = 0.755) and good agreement (MAE = 1.94 dB) with the actual measured MD. The model also showed excellent capability in detecting the presence of glaucomatous visual field loss, achieving an AUC of 0.953.
Conclusions
Artificial intelligence algorithms demonstrate powerful capabilities across the full spectrum of glaucoma management. The studies show that deep learning models can detect glaucomatous change from OCT scans with near-perfect accuracy and can also infer functional damage from simple optic disc photographs. Furthermore, machine learning models can effectively stratify risk and predict future disease progression using only baseline clinical data. The consistent outperformance of these AI models compared to traditional clinical parameters suggests that their integration into clinical workflows holds great promise for enabling earlier diagnosis, more precise assessment, and personalized management of glaucoma patients.
