녹내장은 진행성 시신경병증으로, 전 세계 실명 원인 중 가역성이 없는 주요 원인 중 하나이며, 인구 집단에 따라 질환의 진행 양상에 차이를 보인다. 아시아 인구에서는 시야결손전녹내장과 정상안압녹내장이 흔히 관찰되며, 한국의 경우 정상안압녹내장이 전체 녹내장 환자의 약 77%를 차지하는 것으로 보고되어 있다. 그러나 정상안압녹내장의 장기 임상 경과와 특히 초기 단계에서의 진행 위험에 대한 연구는 제한적이며, 빠른 진행에 대한 연구 역시 부족한 실정이다. 본 연구의 제Ⅰ장은 10년 이상 추적 관찰된 시야결손전녹내장의 임상 경과를 분석하고, 원발개방각녹내장으로의 전환에 대한 위험인자를 평가하였다. 시야결손전녹내장 환자 124명의 124안을 대상으로 하였으며, 녹내장 진행은 구조적 또는 기능적 악화로 정의하였다. Cox 비례 위험모형을 이용하여 진행 관련 인자를 분석하였고, Kaplan-Meier 생존분석으로 시신경유두출혈 유무에 따른 누적 진행 확률을 비교하였다. 그 결과, 전체 대상 중 44안(35.5%)이 평균 12.2 ± 4.2년 추적 기간 동안 원발개방각녹내장으로 전환되었으며, 평균 전환 시점은 5.3 ± 2.2년이었다. 전환된 환자 중 기능적 진행은 30안(68.2%), 구조적 진행은 23안(52.3%), 두 가지 모두는 9안(20.5%)에서 관찰되었다. 편두통/두통(HR 0.75, P = 0.009), 안압 하강 정도(HR 0.75, P = 0.009), DH(HR 3.78, P < 0.010)가 유의한 진행 관련 인자로 확인되었으며, 특히 시신경유두출혈이 있는 환자에서 누적 진행 확률이 유의하게 높았다(P = 0.017). 제Ⅱ장에서는 정상안압녹내장 환자에서 빠른 진행군의 특성을 분석하고, 위험 인자를 규명하였다. 국소 점안치료를 시행 받은 정상안압녹내장 환자 274명의 274안을 포함하였으며, 진행 속도에 따라 빠른 진행군(≥ 1 dB MD/년), 중간 진행군(< 1 dB MD/년), 느린 진행군(≤ 0.5 dB MD/년)로 구분하였다. 평균 13.0 ± 5.9년의 추적 결과, 빠른 진행군은 30안(10.9%), 중간 진행군은 44안(16.1%), 느린 진행군은 200안(73.0%)이었다. 빠른 진행군은 평균 -1.67 ± 0.52 dB/년의 시야손실을 보였으며, 시신경유두출혈의 유병률이 높고 안압 하강률이 낮았다. 다변량 분석 결과, 시신경유두출혈, 안압 하강률, 기저 MD가 빠른 기능적 진행의 유의한 위험 인자로 확인되었으며, 특히 시신경유두출혈이 있는 환자에서 진행 속도가 더 빨랐다(P = 0.002). 결론적으로, 본 연구는 아시아인 정상안압녹내장 환자에서 초기 단계부터 빠른 진행군에 이르기까지의 장기적 질병 경과와 진행 위험 요인을 규명하였다. 이러한 결과는 해당 환자들의 임상적 관리의 최적화 및 향후 질환 진행에 관한 연구 방향의 설정에 있어 중요한 통찰력을 제공한다.

Glaucoma is a progressive optic neuropathy and a leading cause of irreversible blindness worldwide, with variations in disease course among different populations. In Asian populations, preperimetric open angle glaucoma (PPG) and normal tension glaucoma (NTG) are commonly observed, with studies indicating that NTG accounts for approximately 77% of glaucoma patients in Korea. Yet, the knowledge regarding the long-term clinical course and risk of progression in NTG, particularly including its early-stage, remains limited. Furthermore, despite the high prevalence of NTG in Asian population, research focusing on fast progression is still lacking. In Chapter Ⅰ, the long-term clinical course of PPG was investigated, and the risk factors associated with conversion to primary open angle glaucoma (POAG) were evaluated. A total of 124 eyes of 124 PPG patients, followed for more than 10 years, were included. Glaucoma progression was defined as either structural or functional deterioration. Cox proportional hazard modeling was employed to identify factors for progression. Kaplan-Meier survival analysis was utilized to compare cumulative progression probability in PPG patients with or without optic disc hemorrhage. As a result, among the PPG subjects included, 44 eyes (35.5%) converted to POAG, with an average mean deviation (MD) change of -0.39 ± 0.64 dB/year over a mean follow-up of 12.2 ± 4.2 years. The mean time to conversion was 5.3 ± 2.2 years. Within conversion group, 30 eyes (68.2%) exhibited functional progression, 23 eyes (52.3%) showed structural progression, and 9 eyes (20.5%) demonstrated both. Factors significantly associated with glaucoma progression were migraine/headache (HR 2.19, P = 0.012), reduction of intraocular pressure (IOP; HR 0.75, P = 0.009), and disc hemorrhage (DH; HR 3.78, P < 0.001). Notably, patients with DH exhibited higher cumulative probability of progression compared to those without DH (P = 0.017). In Chapter II, the clinical characteristics of fast-progressors among NTG over 10 years of follow-up were investigated, and the risk factors associated with fast progression of glaucoma were identified. We included 274 eyes of 274 NTG patients who were treated with topical medication. Patients were divided into three groups; fast-progressor (progression ≥ 1 dB MD/year), moderate-progressor (progression < 1 dB MD/year), slow-progressor (progression ≤ 0.5 dB MD/year). Comparison among slow, moderate, and fast progressing eyes were measured using one-way ANOVA test and Chi-square test. Logistic regression test and multiple regression test were used to analyze the risk factors for fast progression and MD loss, respectively. Over a mean follow-up period of 13.0 ± 5.9 years, 30 eyes (10.9%) showed fast progression, 44 (16.1%) showed moderate progression and 200 (73.0%) showed slow progression. Fast-progressors demonstrated mean MD loss of -1.67 ± 0.52 dB/year, and had higher prevalence of DH and lower percentage reduction in IOP. Risk factors significantly associated with overall rapid rate of MD loss were DH, percentage reduction in IOP and baseline MD. Notably, progressors with DH exhibited faster functional progression compared to those without DH(P = 0.002). In conclusion, our findings highlight the long-term clinical course and risk of glaucoma deterioration from early-stage to fast-progressors in Asian NTG eyes. The results of our study provide valuable insights for optimizing clinical management and guiding future research on disease progression in such patients.
