실외 초미세먼지 장기노출은 모든 원인 및 원인별 사망, 특히 심혈관 질환과 호흡기 질환으로 인한 사망 위험 증가와 지속적으로 관련이 있음이 보고되고 있다. 초미세먼지 노출로 인한 사망 부담을 정량화하는 것은 근거기반 공중보건 정책 수립에 매우 중요하다. 기존 연구들은 주로 초미세먼지 노출과 사망 연관성에 관한 코호트 연구 결과를 이용하여 비선형 노출-반응 함수를 추정하거나 체계적 문헌고찰에서 수행된 메타분석 결과를 이용하여 건강영향을 추산하였다. 이러한 접근은 개별 연구를 종합하여 일반화할 수 있는 장점이 있으며, 초미세먼지 노출로 인한 글로벌 사망 부담을 정량화 하는데 도움이 된다. 하지만, 대부분의 연구들이 북미·유럽에서 수행된 코호트 연구들이 활용되었으며 아시아 지역에서는 기저 사망률, 노출 농도, 노출 패턴, 사회 경제적 수준, 인구 분포, 문화적 차이 등으로 인하여 직접적인 적용이 제한될 수 있으며, 사망 부담이 과소 또는 과대 추정될 우려가 있다. 또한, 국내 급격한 고령화로 인해 65세 이상 노인의 절대 사망자 수가 증가함에 따라, 노인 인구에서의 초미세먼지 노출로 인한 건강영향 평가가 필요하다.
본 논문은 아시아-태평양 지역 국가에서 수행된 미세먼지 노출과 모든원인 및 원인별 사망 연관성에 관한 체계적 문헌고찰 및 메타분석을 수행하고, 건강보험공단 코호트 자료를 이용하여 국내 노인 인구에서 초미세먼지 장기노출로 인한 사망 연관성 및 사망 부담을 정량화 하고자했다. 마지막으로는 국내 추정치와 글로벌 노출-반응 함수와 비교 및 장래 고령화와 미세먼지 저감에 따른 사망부담 추이에 대한 시나리오 분석을 수행하였다. 연구 결과, 초미세먼지 장기 노출은 국내 노인에서 상당한 초과사망을 초래하는 것으로 나타났으며, 특히 메타분석이나 글로벌 추정치에 기반한 노출–반응 함수를 적용할 때에 비해 국내 코호트 기반 함수 적용 시 초과사망 규모가 상대적으로 낮게 산출되었다. 시나리오 분석에서는 초미세먼지 농도를 저감할 경우 상당한 사망 예방 효과와 공중보건적 편익이 예상되었다. 특히, 현재 초미세먼지 농도 수준에서 점진적으로 농도를 저감할 경우 장기적으로 증가하는 고령화 대비 초과사망자수는 현저히 감소하는 것으로 예상되었다. 종합하면, 본 논문은 아시아-태평양 지역의 미세먼지 노출과 원인별 사망연관성을 규명하고, 국내 노인을 대상으로 한 코호트 기반 초미세먼지 건강영향 평가를 제공하며, 현재와 장래 고령 인구의 사망 부담 및 건강 편익을 다각도로 평가하였다.

Long-term exposure to ambient fine particulate matter (PM2.5) has been consistently associated with increased risks of all-cause and cause-specific mortality, particularly from cardiovascular and respiratory diseases. Therefore, quantifying the mortality burden attributable to PM2.5 is crucial for evidence-based public health policy. Previously, non-linear exposure–response functions were mainly derived from cohort studies, and health impacts were estimated using meta-analyses from systematic reviews (SRs). These approaches allow generalization across individual studies and enable quantification of the global mortality burden attributable to PM2.5. However, most studies were conducted in North America and Europe, possibly limiting direct application to Asian populations owing to differences in baseline mortality rates, exposure concentrations, exposure patterns, socioeconomic factors, population structure, and cultural contexts, potentially leading to under- or overestimation of the mortality burden. In addition, because of the rapid population aging in South Korea, the absolute number of deaths among adults aged ≥65 years has increased. Hence, assessing the health impacts of long-term PM2.5 exposure in the older population is necessary. The aim of this dissertation was to address these gaps by conducting an SR and meta-analysis of PM2.5 exposure and all-cause and cause-specific mortality in Asia-Pacific States, and by quantifying the associations and the mortality burden attributable to long-term PM2.5 exposure among older adults in South Korea using cohort data from the National Health Insurance Service.
Furthermore, domestically derived estimates were compared with global exposure–response functions, and evaluations using scenario analyses were conducted to determine how future population aging and potential reductions in PM2.5 may influence the projected mortality burden. The results indicate that long-term PM2.5 exposure is associated with excess mortality among older adults in South Korea. Mortality estimates were lower when using an exposure–response function derived from cohort data than when using those based on SRs or global exposure-response functions. Scenario analyses revealed that reducing PM2.5 concentrations could significantly reduce deaths, yielding considerable public health benefits. In particular, gradual reductions in PM2.5 may mitigate the excess mortality increase associated with rapid population aging. Overall, the findings elucidate the associations between PM2.5 exposure and cause-specific mortality in the Asia-Pacific States, provide a cohort-based assessment of the health impacts of long-term PM2.5 exposure among older adults in South Korea, and offer a comprehensive evaluation of the current and future mortality burden and potential health benefits under population aging.
