배경:편두통 환자는 발작 간기(interictal phase)에도 감각 자극에 대해 고조된 민감성을 보인다는 증거가 있다. 그러나 이러한 증상의 기저에 있는 신경학적 기전은 여전히 불분명하다. 편두통 발작의 시작과 항상성 조절에 있어 시상하부(hypothalamus)의 역할을 고려할 때, 본 연구는 시상하부가 발작 간기의 감각 처리를 매개하는지 규명하고자 하였다. 이를 위해 휴지 상태 기능적 자기공명영상(resting-state functional MRI, rs-fMRI)을 이용하여 삽화성 편두통(episodic migraine, EM) 환자의 시상하부 기능적 연결성(functional connectivity, FC)과 시각 및 후각 민감도 간의 관계를 조사하였다.
방법: 28명의 삽화성 편두통 환자와 대응된 정상 대조군으로부터 rs-fMRI 스캔을 획득하였다. 발작 간기의 시각 및 후각 증상은 한국형 시각 설문지(Korean photophobia questionnaire)와 자체 개발한 후각 민감도 점수를 사용하여 평가하였다. 시상하부를 시드(seed)으로 하는 시드 기반 FC 분석을 수행하였으며, 신축망 회귀분석(elastic net regression)을 통해 민감도 점수와의 연관성을 조사하였다. 이후 두 표본 t-검정(two-sample t-tests)을 사용하여 집단 간 연결 강도 비교 및 임상적 상관관계를 분석하였다.
결과: 발작 간기의 편두통 환자군은 정상 대조군에 비해 시각 및 후각 민감도 점수가 유의하게 높았다. 신축망 회귀분석 결과, 시각 민감도와 연관된 시상하부 FC 영역 23곳과 후각 민감도와 연관된 영역 44곳이 식별되었으며, 여기에는 감각, 운동, 전두두정(frontoparietal) 및 변연계(limbic) 영역이 포함되었다. 중요하게도, 이들 영역에서 도출된 시상하부 FC의 가중 합(weighted sum)과 임상 점수 간의 양의 선형 상관관계가 정상 대조군에서는 관찰되었으나(시각: r = 0.424, p = 0.025; 후각: r = 0.747, p < 0.001), 환자군에서는 이러한 연관성이 발견되지 않았다(시각: r = 0.016, p = 0.934; 후각: r = -0.191, p = 0.331). 선택된 영역 중 운동 (motor) 및 주의력 (attention) 관련 영역들은 대조군과 비교했을 때 환자군에서 변화된 시상하부 FC를 보였다.
결론: 삽화성 편두통 환자는 정상 대조군과 비교하여 변화된 시상하부 기능적 연결성 패턴을 보였다. 정상 대조군에서 관찰된 시상하부 연결성과 민감도 점수 간의 정상적인 선형 관계가 환자군에서는 감각, 운동 및 주의력 처리와 관련된 피질 영역 전반에 걸쳐 소실되었다. 이는 발작 간기의 감각 처리에 있어 시상하부의 관여가 변화되었음을 시사하며, 이것이 지속적인 감각 과민성의 기저 원인이 될 수 있다.

Background: While neurobiological mechanisms underlying migraine have been revealed, they remain within prespecified migraine phases of interest. The hypothalamus is a central player in homeostatic regulation and has been suggested as a driver of migraine attack initiation. We aimed to investigate the association of the hypothalamus with heightened sensitivity to external stimuli in interictal patients with migraine. Using resting-state functional MRI (rs-fMRI), we assessed associations between hypothalamic functional connectivity (FC) and visual and olfactory sensitivity in patients with episodic migraine (EM).
Methods: We obtained resting-state functional MRI (rs-fMRI) scans from 28 patients with episodic migraine (EM) and matched healthy controls. Interictal visual and olfactory symptoms were assessed using the Korean photophobia questionnaire and an in-house developed olfactory sensitivity score. Seed-based functional connectivity (FC) using the hypothalamus as a seed and its associations with the sensitivity scores were examined using elastic net regression. Then, between-group comparisons of connectivity strength and clinical correlations were analyzed using two-sample t-tests.
Results: Interictal EM patients exhibited significantly higher scores for both visual and olfactory sensitivity compared to healthy controls. Elastic net regression identified 23 regions linked to the hypothalamus and associated with visual sensitivity, and 44 areas related to olfactory sensitivity, encompassing visual, frontoparietal, limbic, and motor regions. Positive correlations between the weighted sum of hypothalamic FC derived from these regions and visual and olfactory sensitivity scores were observed in healthy controls (visual: r = 0.424, p = 0.025; olfactory: r = 0.747, p < 0.001), while no such associations were found in patients (visual: r = 0.016, p = 0.934; olfactory: r = -0.191, p = 0.331). Among the selected regions, motor and attention-related areas showed altered hypothalamic FC in patients when compared to controls.
Conclusion: Patients with EM showed altered hypothalamic functional connectivity patterns compared to healthy controls. The normal linear relationship between hypothalamic connectivity and sensitivity scores observed in healthy controls was disrupted in patients, spanning cortical regions associated with sensory, motor, and attentional processing. This suggests altered hypothalamic involvement in interictal sensory processing that may underlie persistent sensory hypersensitivity.
