    목적: 삼중 음성 유방암은 다른 유방암 아형에 비해 공격적 특성으로 재발과 전이가 흔하며 생존율이 낮다. 면역관문억제제는 삼중 음성 유방암의 표준 치료로 자리잡았으나, 전체 환자 중 치료 효과를 보이는 비율은 제한적이다. 이는 면역치료 반응성을 정밀하게 예측할 수 있는 바이오마커의 필요성을 시사한다. Guanylate-binding protein 5 (GBP5)는 IFN-γ 반응 경로 및 PD-L1 발현과 강한 상관관계를 보이며, 종양 미세환경에서 광범위한 면역 조절 기능을 수행하는 것으로 보고되었다. 그러나 TNBC에서 GBP5가 PD-L1 발현을 조절하는 구체적인 분자 메커니즘과 면역관문억제제 치료 반응에 미치는 영향은 아직 충분히 규명되지 않았다. 본 연구는 TNBC에서 GBP5의 PD-L1 발현 조절 기전을 밝히고, GBP5 발현의 면역관문억제제 치료 반응 예측 바이오마커로서의 가능성을 평가하고자 하였다.

    방법: TCGA Firehose-Legacy와 METABRIC 삼중 음성 유방암 환자 코호트를 분석하여 GBP5와 CD274 (PD-L1) mRNA 발현 간의 상관관계를 평가하였다. CRISPR/Cas9 시스템을 이용해 인간 유래 삼중 음성 유방암 세포주인 MDA-MB-468와 BT-20 세포에서 GBP5 결손 세포주를 구축하고, PD-L1 발현과 PD-1 결합능은 웨스턴 블롯과 유세포분석으로 확인하였다. IFN-γ 자극 여부에 따른 JAK/STAT/PD-L1 신호전달과 IFNGR 발현은 웨스턴 블롯, qRT-PCR, 면역세포화학으로 분석하였다. STAT1 억제 후 GBP5 발현 변화는 qRT-PCR과 웨스턴 블롯으로 확인하였다. GBP5 결손 세포와 인간 PBMC 유래 CD8⁺ T 세포를 공배양하여 caspase-3/7 활성 분석으로 세포사멸을 측정하였다. 4T1 마우스 삼중 음성 유방암 모델에서는 doxycycline과 anti–PD-1 항체를 투여하였으며, 종양 조직은 면역조직화학염색, 종양침윤 CD8⁺ T 세포 유세포분석, 그리고 cleaved caspase-3, granzyme B, perforin의 면역형광염색으로 분석하였다.
    결과: 삼중 음성 유방암 환자 코호트 분석에서 GBP5 mRNA 수준이 높을 때 CD274 발현 또한 유의하게 증가하였다.
    인간 유래 삼중 음성 유방암 세포주에서 CRISPR/Cas9을 이용한 GBP5 결손은 PD-L1 단백질 발현과 PD-1 결합능을 감소시켰다. 또한 JAK1, JAK2, STAT1, STAT3의 mRNA 발현이 감소하였으며, GBP5 결손 세포는 IFN-γ 자극 여부와 관계없이 STAT1 및 PD-L1 단백질 발현이 낮았고, IFN-γ에 의한 발현 유도 또한 현저히 저하되었다. 이러한 변화는 IFNGR1 및 IFNGR2 mRNA 감소와 IFN-γ 자극 하에서도 유지되는 IFNGR1 단백질 발현 감소와 일치하였다. STAT1 인산화를 저해한 경우 GBP5 mRNA 발현이 추가적으로 감소하였으며, CHX 및 MG132 처리 하에서 GBP5 단백질 축적이 현저히 억제되어 GBP5 단백질 안정성 감소가 시사되었다. GBP5 결손은 CD8⁺ T 세포 공배양 실험에서 종양세포의 세포사멸을 증가시켰으며, 이 효과는 anti–PD-1 항체 처리 시 더욱 강화되었다. 4T1 TNBC 마우스 모델에서 GBP5 결손 상태는 anti–PD-1 치료 효과를 증폭시켜 종양 성장 억제 효과가 가장 강하게 나타났으며, 종양 내 CD8⁺ T 세포 침윤과 세포독성 활성 또한 증가하였다.

    결론: 본 연구는 GBP5 발현 수준이 삼중 음성 유방암에서 IFN-γ/STAT1 신호 반응 및 PD-L1 조절에 밀접하게 연관됨을 시사한다. GBP5 결손은 CD8⁺ T 세포 매개 세포독성을 증가시키고 anti–PD-1 치료 반응을 향상시켰다. 이러한 결과는 GBP5가 삼중 음성 유방암에서 면역관문억제제 반응성을 예측할 수 있는 잠재적 바이오마커이자, 면역항암제의 치료 효과를 증진시키기 위한 유망한 표적이 될 수 있음을 시사한다.

    Purpose: Triple-negative breast cancer (TNBC) is an aggressive breast cancer subtype characterized by high recurrence, metastasis, and poor prognosis. Although immune checkpoint inhibitors (ICIs) have become part of the therapeutic landscape, only a subset of patients benefits, underscoring the need for reliable predictive biomarkers. Guanylate-binding protein 5 (GBP5) is implicated in IFN-γ–related immune signaling and has been associated with PD-L1 expression in several cancer types. This study aimed to elucidate the molecular mechanisms by which GBP5 influences PD-L1 regulation and to evaluate its potential as a predictive biomarker for ICI therapy in TNBC.

    Methods: The correlation between GBP5 and CD274 (PD-L1) mRNA expression was analyzed in TNBC samples from the TCGA Firehose-Legacy and METABRIC cohorts. GBP5 knockout human TNBC cell lines including MDA-MB-468 and BT-20 were generated using the CRISPR/Cas9 system, and PD-L1 expression and PD-1 binding capacity were assessed by Western blotting and flow cytometry. IFN-γ–dependent and basal JAK/STAT/PD-L1 signaling, as well as IFNGR expression, were evaluated by Western blotting, qRT-PCR, and immunocytochemistry. GBP5 regulation following STAT1 inhibition was examined using qRT-PCR and Western blotting. CD8⁺ T cell–mediated cytotoxicity was assessed by co-culturing GBP5 knockout cells with human PBMC-derived CD8⁺ T cells, followed by caspase-3/7-based apoptosis assays. In the 4T1 TNBC mouse model, doxycycline and anti–PD-1 monoclonal antibody were administered, and tumor tissues were analyzed by immunohistochemistry, flow cytometry, and immunofluorescence.

    Results: High GBP5 mRNA expression was significantly associated with increased CD274 (PD-L1) levels in TNBC patients from the TCGA and METABRIC cohorts. In human TNBC cell lines, CRISPR/Cas9-mediated GBP5 knockout reduced PD-L1 protein expression and PD-1 binding affinity. Additionally, mRNA levels of JAK1, JAK2, STAT1, and STAT3 were decreased, and GBP5 knockout cells showed reduced STAT1 and PD-L1 protein levels with attenuated induction upon IFN-γ stimulation. These changes were accompanied by decreased IFNGR1 and IFNGR2 mRNA levels and diminished IFNGR1 protein expression, irrespective of IFN-γ stimulation. STAT1 inhibition further decreased GBP5 mRNA expression, and GBP5 protein exhibited markedly attenuated accumulation under CHX or MG132 treatment, suggesting reduced protein stability. GBP5 knockout enhanced CD8⁺ T cell–mediated tumor cell apoptosis in co-culture assays, and this effect was further amplified by anti–PD-1 treatment. In the 4T1 TNBC mouse model, anti–PD-1 efficacy was more pronounced under GBP5 knockout conditions, resulting in stronger tumor growth suppression, increased infiltration of CD8⁺ T cells, and enhanced cytotoxic activity within the tumor microenvironment.

    Conclusion: GBP5 expression strongly reflects IFN-γ/STAT1 signaling activity and the associated regulation of PD-L1 in TNBC. GBP5 knockout markedly enhanced CD8⁺ T cell–mediated cytotoxicity and improved the therapeutic response to anti–PD-1 treatment. These findings support GBP5 as a potential predictive biomarker for ICI responsiveness and a promising target to enhance immunotherapy efficacy in TNBC.

Although immune checkpoint inhibitors (ICIs) have become part of the therapeutic landscape, only a subset of patients benefits, underscoring the need for reliable predictive biomarkers.
