Cell-assisted lipotransfer (CAL)은 자가지방이식의 생착률을 향상시키기 위해 기질혈관분획(stromal vascular fraction, SVF) 또는 지방유래줄기세포(adipose-derived stromal cells, ASCs)를 함께 이식하는 기술이다. 그러나 세포 간 생물학적 이질성, 낮은 생존율, 잠재적 종양원성, 그리고 세포치료제에 대한 복잡한 규제 등으로 인해 임상 적용에는 여전히 한계가 있다.
이러한 문제를 극복하기 위해, 최근에는 세포를 직접 이식하지 않으면서도 핵심적인 재생 기능을 유지할 수 있는 무세포(cell-free) 치료 전략이 주목받고 있다. 본 연구에서는 ASCs 유래 엑소좀(exosome)을 이용하여 지방이식의 생착률과 조직 재생을 향상시킬 수 있는 가능성을 평가하고, SVF 보강 지방이식(SVF-enriched lipotransfer)과의 비교를 통해 임상적으로 적용 가능한 비세포성 대안을 제시하고자 하였다.
이를 위해 인체 유래 지방조직을 phosphate-buffered saline (PBS), SVF 세포, 또는 ASCs 유래 엑소좀과 각각 혼합하여 누드마우스 피하에 이식하였다. 이식 후 4주 및 8주 시점에서 micro-CT를 이용해 이식편의 용적 유지율(volume retention ratio)을 측정하였으며, 1주 및 8주에는 조직학적(H&E) 및 면역형광(Perilipin, CD31, F4/80–iNOS, F4/80–CD206) 분석을 통해 조직 반응을 평가하였다.
그 결과, 엑소좀 병용군은 대조군에 비해 일관되게 높은 용적유지율을 보였으며, SVF 보강군보다도 혈관신생 및 항염증 재형성 측면에서 우수한 경향을 나타냈다. 조직학적 분석 결과, 엑소좀은 섬유화, 염증반응, 유낭형성을 현저히 억제하였으며, Perilipin 양성 성숙 지방세포의 보존과 CD31 발현을 통한 신생혈관 형성을 촉진함으로써 이식편의 구조적 완전성을 향상시켰다. 또한, 엑소좀은 M1 거대식세포 침윤을 감소시키고 M2 극화를 유의하게 증가시켜, 초기 재형성 단계에서 뚜렷한 항염·면역조절 효과를 보였다.
종단형 체내 형광영상 분석에서는 엑소좀이 이식 부위에서 약 4주간 안정적으로 잔존하는 것이 확인되었다.
본 연구의 종합적 분석 결과, ASC 유래 엑소좀은 지방이식의 생착률과 조직 재생을 유의하게 향상시키는 것으로 확인되었으며, 이는 SVF 보강 지방이식을 대체할 수 있는 안전하고 임상 적용성이 높은 무세포 치료 전략으로서의 잠재력을 시사한다.

Background: Cell-assisted lipotransfer (CAL) is a fat grafting technique that enhances graft survival by supplementing grafts with autologous stromal vascular fraction (SVF) or adipose-derived stem cells (ASCs). However, its clinical translation has been hindered owing to inherent biological variability, potential cell viability issues, tumorigenic risk, and the complex regulatory landscape associated with cell-based therapies. To overcome these challenges, exosome has gained increasing attention as a promising non-cellular therapeutic modality capable of preserving key regenerative functions while minimizing relative concerns. This study aimed to compare the efficacy of SVF-enriched lipotransfer with fat co transplanted with exosomes from ASCs to identify a reliable and clinically applicable non-cellular strategy for optimizing fat graft outcomes.
Methods: Minced human fat tissues were mixed with phosphate-buffered saline (PBS), SVF cells, or ASC-derived exosomes and transplanted subcutaneously into nude mice. Graft volume retention was evaluated at 4 and 8 weeks using micro-computed tomography. Hematoxylin and eosin (H&E) and immunofluorescence (IF) analyses (perilipin, CD31, F4/80 iNOS, F4/80–CD206) were performed at 1 and 8 weeks to assess the exosome uptake and tissue remodeling. In vivo fluorescence imaging was conducted to track the biodistribution and temporal dynamics of exosomes.
Results: The exosome group showed a significantly higher volume retention rate than the control group at weeks 4 and 8 post-grafting. H&E evaluation revealed markedly reduced fibrosis, inflammatory infiltration, and oil cyst formation in the exosome-treated grafts. Exosomes further demonstrated superior ability to preserve perilipin-positive mature adipocytes, enhance CD31-positive angiogenesis, and modulate inflammation by driving macrophages toward an M2-dominant phenotype. Longitudinal in vivo imaging confirmed that exosomes remained within graft sites for approximately 4 weeks.
Conclusions: Our findings highlight the potential of exosomes in improving fat graft retention and overall tissue regeneration, suggesting that fat co transplanted with exosomes from ASCs could serve as a superior alternative to SVF-enriched lipotransfer.
