연구 배경: 비만은 단순히 과식하는 습관 및 체중 증가가 아닌 질병으로 인식되고 있다. 아울러 고도 비만 환자는 다수의 동반질환과 마취 시 발생하는 어려움으로 인해 고위험 환자로 인식된다. 수술 후 회복 향상 프로그램 (ERAS)은 수술 후 스트레스 반응을 감소시켜 수술 후 관리의 패러다임을 전환시켰다. 그러나 비만대사 수술을 받는 환자에서의 ERAS 프로토콜은 개인별, 센터별, 국가별로 상이하다. 이에 따라, 이 프로토콜은 비만대사 수술 환자에 대한 근거를 바탕으로 개정되어야 한다. 본 연구에서는 아편유사제 제한 전정맥 마취(TIVA)를 포함한 ERAS 프로토콜이 고도 비만 환자의 비만대사 수술 후 회복에 미치는 영향을 규명하였다.
연구 방법: ERAS 프로토콜의 시행 여부는 개별 마취통증의학과 의사의 진료 지침에 따라 결정되었으며, 본 연구는 이 과정을 추적 관찰하기 위한 관찰 형식으로 진행되었다. 대조군은 수술 전 8시간 이상 금식한 후 아편유사제를 사용한 흡입 마취를 시행받았다. ERAS 군은 수술 전 2시간까지 경구 탄수화물 용액 400ml를 섭취하고 아편유사제 제한 정맥 마취를 시행받았다. 일차 평가 변수는 기능적 재원 일수(FHS)였으며, 이차 평가 변수로는 수술 후 통증 정도, 구조 진통제 투여량, 수술 후 오심 및 구토(PONV) 여부, 그리고 수술 중 아편유사제 투여량이 포함되었다.
연구 결과: 총 54명의 환자를 대상으로 관찰 연구를 수행하였으며, 2명이 중도 탈락하여 최종적으로 각 그룹당 26명의 환자가 연구를 완료하였다. ERAS 군의 보행 시작 시간 (7.52±6.25시간)은 대조군 (12.05±7.88시간)에 비해 유의하게 짧았으며 (p=0.032), 기능적 재원 일수(FHS) 역시 ERAS군 (40.1±9.25시간) 에서 대조군 (50.75±10.73시간)보다 유의하게 단축되었다 (p=0.001).
결론: 본 관찰 연구에서 ERAS 군은 보행 시작 시간과 기능적 입원 기간이 단축되는 결과를 보였다. 이는 ERAS 프로토콜이 수술 후 회복을 향상시키고 환자의 조기 보행과 빠른 퇴원을 촉진함을 시사한다. 이 고위험군 환자들에게 ERAS 프로토콜의 장기적 이점과 잠재적인 수정 방안을 탐색하기 위한 추가 연구가 필요하다.

Background: Obesity is recognized as a disease, rather than a mere habit of overeating and gaining weight. And the morbidly obese are recognized as high risk patients due to many comorbidities and difficulties encountered during anesthesia. Enhanced Recovery After Surgery (ERAS) protocols have shifted the paradigm on postoperative care by reducing surgical stress. However, ERAS protocols in bariatric patients vary from person to person, center to center, and country to country. Therefore, these protocols should be revised based on evidence in the bariatric patients. In this study, we investigated the effect of ERAS protocol including opioid-free total intravenous anesthesia (TIVA), on recovery after bariatric surgery in the morbidly obese.
Methods: The implementation of ERAS protocol was determined according to the practices of individual anesthesiologists, and the study was conducted in an observational format to monitor this process. The control group underwent inhalational anesthesia using opioids during general anesthesia after fasting for more than 8 hours before surgery. The ERAS group consisted of patients who ingested 400 ml of oral carbohydrate solution up to 2 hours before surgery and underwent opioid-free intravenous anesthesia. The primary outcome was functional hospital stay (FHS). Secondary outcomes included the degree of postoperative pain, dose of rescue analgesics, the presence of postoperative nausea and vomiting (PONV), and dose of intraoperative opioid.
Results: An observational study was conducted with a total of 54 patients, of whom 2 dropped out, resulting in 26 patients completing the study in each group. The time to start walking was significantly shorter in the ERAS group (7.52 ± 6.25 hours) compared to the Control group (12.05 ± 7.88 hours), with a p-value of 0.032. In addition, the functional hospital stay was significantly shorter in the ERAS group (40.1 ± 9.25 hours) compared to the Control group (50.75 ± 10.73 hours), with a p-value of 0.001.
Conclusion: In our observational study, the ERAS group demonstrated a shorter time to start walking and a reduced length of the functional hospital stay. These findings suggest that the ERAS protocol enhances postoperative recovery, promoting faster mobilization and discharge. Further research is warranted to explore the long-term benefits and potential modifications of the ERAS protocol in this high-risk population
