연구 배경: 쯔쯔가무시병은 한국에서 상당한 매개체 감염병 부담을 차지하며, 그 전파는 환경 변동성과 기후변화에 의해 점차 조절되고 있다. 감염병 역학에 대한 기상학적 영향이 인식되고 있음에도 불구하고, 온도, 습도, 강수량, 풍속, 일조시간, 식생지수를 포함한 다중 환경 요인의 복잡한 지연 효과는 여전히 불충분하게 규명되어 있다. 본 학위논문은 고급 통계 방법론을 사용한 두 개의 상호 보완적 연구를 통해 이러한 환경 결정 요인을 조사하였다.
연구 방법: 연구 1은 시도 단위 전국 감시 자료(2001-2018)를 사용하여 온도-질병 연관성을 분석하였다. 일반화 가법 모형과 일반화 선형 모형을 통해 13주 지연 후 지역별 임계 온도(13.3-18.5°C)를 확인하고 효과를 정량화 하였다. 장기 기후변화 영향은 연평균 온도를 1971-2000년 기준선과 비교하여 평가하였다.
연구 2는 시군구 단위 81개 행정구역에서 38,525건의 주간 확진 사례(2001-2019)에 대해 분포지연 비선형 모형(DLNM)을 적용하여 14주 누적 지연 기간 동안 다중 환경 노출의 지연된 비선형 용량-반응 관계를 규명하였다. 베이지안 공간 분석을 통해 지리적 이질성을 평가하였고, 인구학적 층화 분석을 통해 연령 및 성별 특이적 취약성을 조사하였으며, 농업 밀도별 층화 분석을 통해 환경 요인 효과의 이질성을 규명하였다. 앙상블 기계학습 방법으로 개발된 예측 모형을 독립적인 2019-2024년 자료로 외부 검증하였다.
연구 결과: 연구 1은 지역별 임계값 이상의 0.1°C 온도 상승이 13주 후 발생률 25% 증가에 해당함을 입증하였고(OR 1.25, 95% CI 1.21-1.29), 30년 기준선 대비 연평균 0.1°C 상승은 33% 증가를 예측하였다(OR 1.33, 95% CI 1.26-1.41).
연구 2는 복잡한 비선형 연관성을 드러냈다: 8.4°C에서 누적 최고위험도(RR 1.51, 95% CI 1.28-1.77), 습도는 통계적으로 유의한 56.3%에서 최고값(RR 1.30, 95% CI 1.11-1.53)을 보였으며, 풍속은 14.8 m/s 초과에서 보호 효과(RR 0.21, 95% CI 0.13-0.34)를 보였다. 식생지수의 임계 효과는 NDVI 0.72에서 통계적으로 유의한 최고값(RR 1.35, 95% CI 1.16-1.56)을 보였다. 농업 인구 고밀도 지역은 저밀도 지역 대비 높은 위험도(ΔRR ≈ 7)와 지연된 유행 정점(7주)을 나타냈다. 중년 남성(45-64세)은 일조시간에 대해 통계적으로 유의한 취약성(RR 2.127, 95% CI 1.256-3.603)을 나타냈으나, 여아(0-14세)는 강수량(RR 6.27, 95% CI 0.009-4346.6)과 식생지수(NDVI RR 3.084, 95% CI 0.408-23.3)에서 높은 점 추정값을 보였으나 신뢰구간이 넓어 통계적 유의성이 없었다. 앙상블 모형은 99.8% 포함률을 달성하였다(RMSE 0.107; MAE 0.026).
고찰: 본 연구들은 기상 및 식생 요인이 쯔쯔가무시병 전파에 복잡하고 지연된 비선형 효과를 미친다는 것을 입증한다. 온도와 습도 임계값, 극한 기상의 보호 효과는 벡터-병원체-숙주 상호작용에 대한 기전적 통찰을 제공한다. 일관된 13주 지연은 조기 경보 체계 개발을 위한 과학적 근거를 제공하며, 인구학적 이질성은 표적 예방이 필요한 취약 집단을 강조하고 특히, 농업인구 고밀도 지역의 지연된 피크는 맞춤형 감시 전략의 필요성을 시사한다. 앙상블 접근법은 감시 정의 변화에도 불구하고 검증된 성능으로 견고한 조기 경보 체계의 실용적 적용 가능성을 보여주며, 지리적 이질성은 지역 특이적 자원 배분의 중요성을 강조한다.
결론: 본 학위논문은 환경 요인이 쯔쯔가무시병 발생에 대해 복잡한 비선형 용량-반응 관계와 복잡한 지연 효과를 나타낸다는 것을 확립함으로써 매개체 감염병 전파에서 기후-보건 관계에 대한 이해를 진전시킨다. 본 연구는 환경 기반 감시 체계 개발을 위한 기초 지식을 제공하고 지구온난화 속에서 공중보건 개입을 위한 과학적 근거를 제시하며, 기후변화가 감염병 역학을 변화시킴에 따라 인구 건강을 보호하는 데 필수적인 표적 예방 전략과 적응형 감시 체계의 개발을 지원한다.

Background: Scrub typhus constitutes a substantial vector-borne disease burden in South Korea, with transmission increasingly modulated by environmental variability and climate change. Despite recognized meteorological influences on infectious disease dynamics, the complex delayed effects of temperature, humidity, precipitation, wind, sunshine, and vegetation remain inadequately characterized. This dissertation investigates these environmental determinants through two complementary investigations employing advanced statistical methodologies.
Methods: The first investigation analyzed temperature–disease associations using national surveillance data (2001–2018) across provinces, employing generalized additive and linear models to identify region-specific thresholds (13.3–18.5°C) and effects at 13-week lag. Long-term climate impacts were assessed relative to the 1971–2000 baseline.
The second investigation employed distributed lag non-linear models (DLNM) to analyze 38,525 weekly confirmed cases (2001–2019) across 81 districts, characterizing complex 14-week lagged dose-response relationships through national-scale pooling (without district/provincial fixed or random effects due to sparse case counts). Bayesian spatial analysis (INLA) assessed district-level geographic heterogeneity, stratified age-sex analyses identified demographic vulnerabilities, and stratified analyses examined effect modification by agricultural density. Ensemble machine-learning approaches provided predictive models with internal and external validation (2019–2024).
Results: The first investigation demonstrated that each 0.1°C temperature increase above regional thresholds corresponded to a 25% rise in incidence (95% CI: 21–29%) after 13 weeks, while each 0.1°C annual rise above the 30-year baseline predicted a 33% increase (95% CI 26–41%).
Second investigation revealed complex non-linear associations emerged over 14-week lags: temperature peaked at 8.4°C (RR 1.51; 95% CI 1.28–1.77), humidity at 56.3% (RR 1.30; 95% CI 1.11–1.53), and protective effects emerged at high wind speeds (14.8 m/s; RR 0.21; 95% CI 0.13–0.34). Vegetation threshold effects were observed at NDVI 0.72 (RR 1.35; 95% CI 1.16–1.56). High agricultural density was associated with substantially increased risk (ΔRR ≈ 7) and delayed peaks (up to 7.14 weeks). Middle-aged males (45–64 years) showed significant increased risk associated with sunshine exposure (RR 2.13; 95% CI 1.26–3.60). Geographic heterogeneity with north-south gradient was observed. Ensemble models achieved high external validation performance (RMSE 0.107; MAE 0.026; 99.8% coverage).
Discussion: These investigations demonstrate that meteorological and vegetation factors exert complex, lagged, non-linear effects on scrub typhus transmission. The consistent 13-week lag could support early warning system development. Temperature thresholds, protective effects of extreme cold, and optimal humidity-vegetation ranges offer mechanistic insights into vector-pathogen-host interactions. Demographic heterogeneity suggests targeted prevention strategies for vulnerable subgroups. Delayed peaks in high-density areas warrant region-adapted surveillance approaches. Ensemble methods demonstrate promise for robust early warning capabilities despite evolving surveillance definitions, and geographic heterogeneity underscores the need for region-specific resource allocation.
Conclusion: This research contributes to understanding climate–health relationships in vector-borne disease transmission by characterizing complex, non-linear dose-response relationships with lagged effects. It provides foundational knowledge that may inform environment-driven surveillance systems and support public health intervention planning amid global warming, with potential to enhance prevention strategies and adaptive surveillance as climate change influences infectious disease dynamics.
