Background: Nonsteroidal anti-inflammatory drugs (NSAIDs) are ubiquitous in the management of pain and inflammation, particularly among older adults burdened by musculoskeletal conditions. While effective, their use is tempered by concerns regarding nephrotoxicity. The aging kidney, characterized by reduced functional reserve and increased susceptibility to insults, may be particularly vulnerable. Although the acute hemodynamic effects of NSAIDs, mediated by prostaglandin inhibition, are well-established causes of acute kidney injury (AKI), the long-term impact of chronic NSAID exposure on the initiation and progression of chronic kidney disease (CKD) in the general senior population remains contentious. Previous studies have yielded conflicting results, often limited by methodological issues such as confounding by indication, short follow-up durations, and inadequate assessment of medication adherence.
Objectives: This study aimed to rigorously investigate the association between chronic NSAID use and the risk of developing early-stage CKD in a large, representative cohort of community-dwelling older adults with initially preserved kidney function. Secondary objectives included evaluating the rate of estimated glomerular filtration rate (eGFR) decline and comparing the risks associated with Cyclooxygenase-1 (COX-1) predominant inhibitors versus COX-2 selective inhibitors.
Methods: This retrospective cohort study utilized data from the National Health Insurance Service-Senior Cohort (NHIS-SC) in South Korea (2009-2019). The study population comprised individuals aged 65 years or older with a baseline eGFR≥60 mL/min/1.73m2. Chronic NSAID exposure was stringently defined using a medication possession ratio (MPR)≥80% for a duration of at least one month. To minimize confounding, 604 NSAID users were matched 1:2 with 1,208 non-users using propensity score matching, balancing demographics, comorbidities (diabetes, hypertension, hyperlipidemia), Charlson Comorbidity Index (CCI), and cardiovascular history. The primary outcome was the development of kidney dysfunction, defined as eGFR<60 mL/min/1.73 m2 coupled with a ≥10% decline from baseline eGFR (calculated via the CKD-EPI equation). Hazard ratios (HRs) and 95% confidence intervals (CIs) for CKD were estimated using Cox proportional hazards regression models.
Results: The matched cohort (N=1812) was well-balanced across baseline characteristics. Over the 10-year follow-up period, chronic NSAID use was significantly associated with an increased risk of developing CKD (HR 1.46; 95% CI 1.11-1.93) compared to non-use. Longitudinal analysis confirmed a faster rate of eGFR decline among NSAID users. Subgroup analyses demonstrated elevated risks for both COX-1 inhibitor (predominant) user group (HR 1.53) and COX-2 selective inhibitors (HR 1.61), suggesting a potential class effect rather than a differential risk based on COX selectivity. The incidence of end-stage renal disease (ESRD) was rare in this early CKD cohort and did not significantly differ between groups.
Conclusion: In this large-scale, population-based study of older adults, chronic use of NSAIDs, regardless of COX selectivity, was independently associated with an increased risk of incident early CKD and accelerated kidney function decline. These findings underscore the necessity for judicious prescribing of NSAIDs in the senior population. Clinicians must carefully weigh the risks and benefits, prioritize alternative pain management strategies when feasible, and implement rigorous renal function monitoring during long-term NSAID therapy.
