    배경
    음주와 흡연은 높은 동반 유병률을 보이며 과음은 금연 성공률을 저해하는 것으로 알려져 있다. 기존 연구들은 음주와 흡연 간의 관계를 분석해 왔으나 많은 연구가 단면적 설계에 의존하거나 금연 과정의 단계별 분석이 부족하고 심리사회적 기제를 충분히 고려하지 못하는 한계가 있다. 한국의 경우 강력한 담배 규제 정책에도 불구하고 흡연율 감소가 정체된 반면, 알코올 소비는 세계적으로 높은 수준을 유지하고 있다. 따라서 금연의 각 단계에서 알코올이 어떠한 영향을 미치는지, 서로 다른 알코올 의존 궤적이 금연 성과에 어떠한 결과를 초래하는지, 그리고 이러한 관계를 설명하는 심리사회적 경로가 무엇인지를 규명하는 것이 중요하다. 본 논문은 (1) 음주와 금연 의도, 시도, 성공 간의 연관성, (2) 종단적 알코올 의존 궤적이 금연 성공에 미치는 영향, (3) 고위험 음주가 금연 의도에 미치는 영향을 태도, 사회적 영향, 자기효능감의 매개 경로를 통해 규명하는 것을 목적으로 한다.
    방법
    세 가지 연구는 International Tobacco Control (ITC) Korea Survey 및 한국고령화연구패널조사 (Korean Longitudinal Study of Aging, KLoSA) 자료를 활용하였다. 연구 1에서는 ITC KRA Waves 1–3(2020, 2021, 2023) 자료를 사용하여 세 가지 음주 관련 변수(음주 빈도, 주간 음주량, 폭음 빈도)와 금연 관련 결과 (금연 의도, 금연 시도, 금연 성공) 간의 연관성을 다변량 일반화추정방정식(GEE) 모형으로 분석하였다. 연구 2에서는 KLoSA 1–7차 (2006–2018) 자료를 활용하여 45세 이상 한국 남성의 알코올 의존 궤적을 잠재계층성장분석 (LCGA)으로 도출하고, Cox 비례위험 모형을 이용해 금연 성공과의 연관성을 추정하였다. 연구 3에서는 ITC KRA 1차 (2020년) 자료를 활용하여 태도–사회적 영향–자기효능감으로 구성된 ASE (Attitude–Social Influence–Efficacy) 모형에 기반한 인과 매개분석을 수행하였다. 이를 통해 고위험 음주가 금연 의도에 미치는 총효과, 직접효과, 간접효과를 추정하였으며, 연령, 성별, 소득, 혼인상태, 일일 흡연량을 보정하였다.
    결과
    연구 1에서는 음주 관련 변수들과 금연 의도 간의 유의한 연관성은 관찰되지 않았으나 주간 음주량과 금연 시도 간에는 비선형적 관계가 나타났다. 중간 수준의 주간 음주량은 비음주 및 낮은 수준의 음주량에 비해 금연 시도를 할 오즈가 유의하게 높았다. 반면, 음주 빈도가 월 1회 이하인 경우에는 음주를 하지 않거나 월 2-4회 또는 주 2-3회 음주를 하는 경우에 비해 금연에 성공할 오즈가 높았다. 또한 금연 성공은 빈번한 폭음과 역의 관계를 보였으며 주 1회 이상 폭음을 하는 경우 금연에 성공할 오즈가 낮았다. 연구 2에서는 세 가지 알코올 의존 궤적 (안정적 저수준형, 감소형, 증가형)이 확인되었다. 감소형과 증가형 궤적 모두 안정적 저수준형에 비해 금연 성공률이 유의하게 낮았고 특히 증가형에서 가장 낮은 금연 성공률이 나타났다. 연구 3에서 고위험 음주는 금연 의도와 직접적으로 유의한 관련성을 보이지 않았으나 금연 관련 자기효능감을 매개로 한 간접효과는 유의하게 나타났다. 즉, 고위험 음주는 금연 관련 자기효능감을 낮추고 낮은 자기효능감은 금연 의도를 약화시키는 경로가 확인되었다. 반면, 흡연 관련 태도와 흡연 관련 사회적 영향을 통한 매개효과는 유의하지 않았다.
    결론
    음주는 금연 과정에서 단계별 그리고 궤적별로 다른 영향을 미치며 금연 의도를 저해할 수 있다. 본 연구는 흡연과 음주에 대해 동시에 개입하는 금연 서비스의 필요성, 고위험 알코올 의존 집단에 대한 맞춤형 지원 및 금연 캠페인의 중요성을 시사한다. 이러한 근거를 마련함으로써 한국에서 금연율 정체 문제를 극복하는 데 기여하고자 한다.

    Background
    Alcohol use and tobacco smoking are highly co-prevalent, and heavy drinking may be associated with poorer smoking cessation outcomes. While prior research has examined the relationship between alcohol use and smoking, evidence is inconsistent and often limited by cross-sectional designs, a lack of stage-specific analyses, and insufficient consideration of psychosocial mechanisms. In South Korea, the rate of decline in smoking prevalence has slowed despite strong tobacco control policies, and the prevalence of heavy episodic drinking remains among the highest in the world. In this context, it is important to understand how alcohol use influences smoking cessation outcomes across the distinct stages of the cessation process. In addition, clarifying how changes in alcohol dependence, or trajectories of alcohol dependence, are associated with cessation outcomes, as well as the psychosocial mechanisms linking heavy alcohol use to smoking quit intention, is necessary. This thesis aims to investigate: (1) the stage-specific associations between alcohol use and smoking quit intention, quit attempt, and quit success; (2) the impact of longitudinal alcohol dependence trajectories on smoking cessation outcomes; and (3) the psychosocial mechanism that links high-risk alcohol use to quit intention, with a focus on the mediating role of smoking-related attitudes, social influence and quitting self-efficacy.
    Methods
    Three studies were conducted using nationally representative survey data from the International Tobacco Control (ITC) Korea Survey and the Korean Longitudinal Study of Aging (KLoSA). In Study 1, multivariable generalized estimating equation (GEE) models were used to assess the association between three alcohol use measures (drinking frequency, weekly alcohol consumption, binge drinking frequency) and smoking cessation outcomes (quit intention, quit attempt, quit success) over three survey waves using ITC KRA1-3 data. In Study 2, longitudinal trajectories of alcohol dependence among Korean men aged ≥45 years in waves 1-7 of KLoSA were identified using latent class growth analysis (LCGA). Cox proportional hazards models were then used to examine the association between these trajectories and quit success. In Study 3, a parallel multiple mediation analysis was used to test whether smoking-related attitudes, social norms and quitting self-efficacy mediate the association between high-risk alcohol use and quit intention.
    Results
    In Study 1, none of the drinking-related variables were associated with quit intention, but weekly alcohol consumption showed a nonlinear association with quit attempts, with moderate drinkers more likely to attempt quitting than non-drinkers or light drinkers. Quit success was inversely associated with frequent binge drinking: those binge drinking ≥1/week had lower odds of success compared to never binge drinkers. In Study 2, three alcohol dependence trajectories were identified: Stable Low, Decreasing, and Increasing. Both Decreasing and Increasing trajectories were associated with significantly lower quit success compared to the Stable Low trajectory, with the Increasing trajectory having the lowest quit success. In Study 3, quitting self-efficacy was found to significantly mediate the association between high-risk alcohol use and quit intention, whereas attitudes toward smoking and social norms did not show significant indirect effects.
    Conclusion
    Alcohol use affects smoking cessation in a stage-specific and trajectory-dependent manner and influences quit intention indirectly through reduced quitting self-efficacy. These findings support the implementation of further interventions that address both tobacco and alcohol use among high-risk individuals who smoke and have problem drinking in cessation clinics, as well as antismoking campaign messaging that emphasizes abstinence self-efficacy. Integrating interventions that target the dual behaviors of alcohol and tobacco use may improve quit outcomes at the population level and help overcome the stalling of cessation rates in South Korea.
