    수술 후 통증은 회복을 저해할 수 있으나, 수술 후 합병증과의 연관성은 명확하지 않다. 본 연구는 최소침습 대장암 수술을 받은 환자에서 초기 수술 후 통증과 합병증 발생 간의 연관성을 조사하였다.
    본 후향적 연구는 2023년 10월부터 2025년 5월까지 Korea Enhanced Recovery After Surgery (K-ERAS) 등록 데이터를 이용하여 분석하였다. 6개 3차의료기관에서 대장암 절제술을 시행받은 성인 환자를 대상으로 하였다. 수술 후 1일차 통증 강도는 EuroQol 5-Dimension 5-Level 설문지이 통증/불편감 항목을 이용하여 경도, 중등도, 중증으로 분류하였다. 일차 결과변수는 Comprehensive Complication Index (CCI)를 기반으로 한 수술 후 합병증으로, 합병증 없음, 경도(CCI ≥ 8.7 and < 26.2), 중증(CCI ≥ 26.2)으로 분류하였다. 이차 결과변수는 수술 후 입원기간이었다. 통증 정도와 결과 간의 연관성은 순서형 로지스틱 회귀분석과 다변량 선형 회귀분석을 통해 평가하였다.
    총 794명의 환자가 분석에 포함되었다. 중증 통증 군은 경도 통증군에 비해 수술 후 합병증 발생 위험이 유의하게 높았다(OR = 1.88; 95% CI, 1.14–3.17; P = 0.015). 또한 입원 기간은 중등도(LOS ratio = 1.07; 95% CI, 1.01–1.14; P = 0.026) 및 중증 통증군(LOS ratio = 1.15; 95% CI, 1.08–1.23; P < 0.001)에서 경도군보다 유의하게 길었다.
    수술 초기의 중증 통증은 독립적으로 더 높은 합병증 부담과 장기 입원과 연관되어 있었다. 본 연구 결과는 환자의 편안함 향상뿐만 아니라, 수술 후 관리 및 회복 프로그램 최적화를 위한 효과적인 통증 조절의 중요성을 강조한다.

    Methods:
    This retrospective study analyzed prospectively collected data from the Korean Enhanced Recovery After Surgery registry between October 2023 and May 2025. Adult patients who underwent colorectal cancer resection at six tertiary hospitals were included. Pain intensity on postoperative day (POD) 1 was assessed using the pain/discomfort dimension of the EuroQol 5-Dimension 5-Level questionnaire and categorized as mild, moderate, or severe. The primary outcome was postoperative complications, categorized using the Comprehensive Complication Index (CCI) as none, minor (CCI ≥ 8.7 and < 26.2), or major (CCI ≥ 26.2). The secondary outcome was length of postoperative hospital stay (LOS). Associations between pain severity and outcomes were evaluated using ordinal logistic regression and multivariable linear regression.

    Results:
    A total of 794 patients were included. Severe pain was associated with significantly higher odds of postoperative complications compared with mild pain (OR = 1.88; 95% CI, 1.14 to 3.17; P = 0.015). LOS was significantly longer in the moderate (LOS ratio = 1.07; 95% CI, 1.01 to 1.14; P = 0.026) and severe (LOS ratio = 1.15; 95% CI, 1.08 to 1.23; P < 0.001) pain groups compared with the mild group.

    Conclusions:
    Severe early postoperative pain was independently associated with a higher complication burden and longer LOS. These findings underscore the importance of effective pain management for improving patient comfort and providing improvements to perioperative care protocols.
