1. 목 적
경조직 재생은 중간엽 줄기세포가 골형성 또는 상아질형성 계열로 분화하는 과정에 의존한다. MDM2는 p53의 주요 음성 조절자로서, 생체광물화 및 조직 재생과 관련된 전사 프로그램을 조절한다. 따라서 MDM2–p53 축의 조절은 경조직 재생을 촉진할 수 있는 잠재적 치료 전략이 될 수 있다. 이에 따라 본 연구의 목적은 다음과 같다: (1) 혁신적 신약 기술인 PROTAC(Proteolysis-targeting chimera)을 이용해 MDM2 단백질을 선택적으로 분해하는 MDM2-PROTAC을 개발하고 골 및 치아 유래 줄기세포에서 재생 효능을 평가하는 것, (2) 항암제로 개발된 MDM2 inhibitor serdemetan의 drug repurposing을 통한 골재생 효과 및 그 분자적 기전을 규명하여 MDM2–p53 축 기반 경조직 재생 전략의 분자적 근거를 확립하는 것이다.
2. 방 법
우선, Alizarin Red S 기반 골분화 스크리닝을 통해 후보 MDM2 억제제를 발굴하였다. 선별된 억제제는 최적화된 linker를 통해 E3 ligase ligand(VHL 또는 CRBN)와 연결하여 다양한 구조의 MDM2-PROTAC을 설계·합성하였다. PROTAC 후보물질은 MDM2 단백질 감소 여부를 기준으로 1차 스크리닝되었으며, 선별된 리드 화합물(CL144, CL174)에 대해서는 EGFP-표지 MDM2 리포터를 이용한 Western blot 및 live-cell imaging 분석을 통해 MDM2 분해효율 및 화합물의 특성을 평가하였다. 개발된 MDM2-PROTAC(CL144, CL174)과 MDM2 inhibitor인 serdemetan의 골형성 효능은 인간 골수 유래 중간엽 줄기세포(hBMSC)와 인간 치수 줄기세포(hDPSC)에서 평가하였다. 골형성 분화 및 미네랄 침착 정도는 Alizarin Red S 염색 및 알칼리성 인산분해효소(ALP) 염색을 통해 정량화하였으며, qRT-PCR, Western blotting, 면역형광분석을 통해 mRNA 및 단백질 수준에서 골분화 마커의 발현을 분석하여 osteogenic/odontogenic 계열 분화 여부를 확인하였다. 기전 분석을 위해 처리된 세포에서 total RNA를 추출하여 bulk RNA sequencing을 수행하고, 차등 발현 유전자(DEG) 및 GO/KEGG pathway enrichment 분석을 통해 관련 신호경로를 규명하였다.
생체 내 재생 효능은 세 가지 모델(두개골 결손, 난소절제술 유발 골다공증, pulpotomy 기반 상아질 재생 모델)을 통해 평가하였으며, MDM2 표적화합물 (CL144)는 국소 혹은 전신으로 투여하였다. 새롭게 형성된 경조직은 micro-CT를 이용해 정량화하고, H&E, Masson’s trichrome, IHC 분석을 통해 조직 성숙도 및 재생된 조직의 구조적 특성을 평가하였다. 통계적 유의성은 Student’s t-test 또는 일원분산분석(one-way ANOVA)과 적절한 사후 검정을 통해 확인하였다.
3. 결 과
개발된 MDM2-PROTAC(CL144 및 CL174)은 MDM2를 효율적으로 분해하고 p53을 지속적으로 활성화하여 hBMSC와 hDPSC에서 골형성 분화와 생체광물화를 강력하게 증가시켰다. 두 세포 계열 모두에서 PROTAC 처리 시 거의 동일한 분해 효율과 골형성 결과가 나타나, 서로 다른 MSC 유래 계통에서 높은 재현성을 확인하였다. 특히 CL144는 RUNX2 및 BMP2 발현을 현저히 증가시키고 미네랄 매트릭스 형성을 가속화함으로써 가장 우수한 골형성 촉진 효과를 보였다. Bulk RNA-seq 분석 결과, 치수 유래 및 골수 유래 기질 세포 모두에서 ossification 및 odontogenesis 관련 유전자 발현이 증가하고, p53 관련 전사 신호 경로가 활성화되는 것이 확인되었다. 이는 MDM2 분해가 골 및 상아질 재생과 관련된 전사 프로그램을 활성화함을 시사한다. 이러한 분자적 변화는 생체 내에서도 입증되었다. CL144는 두개골 결손 모델과 난소절제술(osteoporosis) 모델에서 의미 있는 신생골 형성을 유도하였으며, pulpotomy 모델에서는 새로운 상아질/경조직 형성을 촉진하였다. 특히 osteoporosis 모델에서 alendronate와 병용 투여 시, 골 재생 효과가 상승하여 단독 투여보다 더 큰 재생 효능을 보였다. 한편, 체계적 스크리닝으로 발굴된 MDM2 inhibitor serdemetan은 hBMSC에서 골형성 분화 및 생체광물화를 촉진하는 동시에, BMM에서 파골세포 분화를 억제하고 골 흡수를 감소시키는 dual-regulation 효과를 나타냈다. 전사체 분석에서도 p53 신호 활성화, 골형성 유전자 증가, 파골세포 관련 유전자 감소가 확인되었다. p53 의존성을 검증하기 위해 유전적 perturbation을 수행한 결과, p53 억제는 골형성 마커를 감소시키고 파골세포 마커를 증가시켰으며, 반대로 p53 과발현은 골형성 마커를 증가시켰고 p53 활성 억제제에 의해 효과가 일부 감소하였다. 이는 PROTAC과 serdemetan의 효과가 p53 의존적으로 매개됨을 입증한다. 종합해보면, MDM2-PROTAC과 serdemetan은 MDM2–p53 축을 조절함으로써 중간엽 줄기세포에서의 골·상아질 계열 분화와 생체광물화를 촉진하고, p53 의존적으로 경조직 재생을 유도하는 새로운 치료 전략임을 확인하였다.

Hard tissue regeneration, including bone and dentin formation, depends on the differentiation of mesenchymal stromal cells into osteogenic or odontogenic lineages. MDM2, a major negative regulator of p53, modulates transcriptional programs associated with biomineralization and tissue regeneration. Thus, modulation of the MDM2–p53 axis represents a potential therapeutic strategy to enhance hard tissue regeneration. Accordingly, the objectives of this study are: (i) to develop MDM2-PROTACs that selectively degrade MDM2 using an innovative PROTAC drug modality and evaluate their regenerative efficacy in bone- and dental-derived mesenchymal stromal cells, and (ii) to investigate the bone-regenerative effects and underlying mechanisms of serdemetan—an anticancer MDM2 inhibitor—using a drug-repurposing approach, thereby establishing the foundation for MDM2–p53–mediated hard-tissue regeneration.
Methods
Candidate MDM2 inhibitors were first identified through Alizarin Red S–based osteogenic screening, and MDM2-PROTAC molecules were subsequently designed by linking the selected inhibitor to an E3 ligase ligand (VHL or CRBN) through an optimized linker. PROTAC candidates were screened for target engagement by evaluating MDM2 protein reduction, and the final lead compounds (CL144 and CL174) were functionally validated by confirming MDM2 degradation and p53 stabilization using Western blotting and live-cell imaging with EGFP-tagged MDM2 reporters. To assess osteogenic potential, developed MDM2-PROTACs (CL144, CL174) and the MDM2 inhibitor serdemetan were applied to human bone marrow–derived stromal cells (hBMSCs) and human dental pulp stem cells (hDPSCs). Osteogenic differentiation and mineral deposition were quantified using Alizarin Red S and alkaline phosphatase staining, and lineage commitment was evaluated by analyzing osteogenic marker expression at both mRNA and protein levels through qRT-PCR, Western blotting, and immunofluorescence. To elucidate the underlying mechanism, total RNA from treated cells was subjected to bulk RNA sequencing, and differentially expressed genes and enriched signaling pathways were identified through DEG and GO/KEGG pathway analyses. To determine translational regenerative efficacy in vivo, MDM2-targeting compounds (CL144) were administered either locally or systemically in multiple models, including critical-sized calvarial defects, ovariectomy-induced osteoporosis, and pulpotomy-based dentin regeneration. Newly formed hard tissue was quantified by micro-computed tomography (micro-CT), and tissue maturation was evaluated by histological and immunohistochemical analyses (H&E, Masson’s trichrome, IHC). Statistical significance was determined using Student’s t-test or one-way ANOVA with appropriate post-hoc tests.
Results
Developed MDM2-PROTACs (CL144 and CL174) efficiently degraded MDM2 and induced sustained activation of p53, resulting in a robust increase in osteogenic differentiation and biomineralization. In both hBMSCs and hDPSCs, PROTAC treatment yielded nearly identical degradation efficiency and osteogenic outcomes, demonstrating high reproducibility across distinct MSC-derived lineages. Among them, CL144 most strongly enhanced osteogenesis, markedly increasing the expression of BMP2 and RUNX2 and accelerating mineral matrix formation. Whole-transcriptome analysis in human dental pulp stromal cells and bone marrow–derived stromal cells showed increased expression of genes associated with hard-tissue regeneration, including those related to ossification and odontogenesis, together with activation of the p53-associated transcriptional pathway, supporting that MDM2 degradation activates transcriptional programs relevant to bone and dentin regeneration. Consistent with these molecular responses, CL144 induced substantial new hard-tissue formation in vivo, enhancing bone regeneration in both osteoporosis and vertical onlay graft models and supporting dentin formation in a pulpotomy model. In the osteoporosis model, combination treatment with alendronate further augmented the regenerative effect, demonstrating a synergistic improvement in bone formation.
Serdemetan, identified as the selected MDM2 inhibitor through systematic screening, enhanced osteogenic differentiation and mineralization while concurrently suppressing osteoclastogenesis and bone resorption, confirming its dual regulation of bone remodeling. Transcriptomic profiling revealed activation of the p53 pathway and upregulation of osteogenic genes, along with suppression of osteoclast-associated markers in osteoclast precursors. To examine the involvement of p53 in the observed phenotypes, p53 expression was modulated through genetic perturbation. In hBMSCs, siRNA-mediated p53 knockdown reduced the expression of osteogenic markers. Conversely, p53 overexpression increased osteogenic marker expression, whereas treatment with the p53 transcriptional activity inhibitor Pifithrin-α partially attenuated these increases. In BMMs, p53 knockdown resulted in higher expression levels of osteoclast markers. Taken together, these results confirm that the observed effects are mediated by p53-dependent regulation.
Conclusion
This study demonstrates that an MDM2-targeting strategy modulates p53 activity to enhance hard-tissue regeneration. MDM2 targeting promoted osteogenic and odontogenic differentiation in human stromal lineages, induced biomineralization, and stimulated bone and dentin formation in vivo. These findings indicate that regulating the MDM2–p53 axis may serve as a therapeutic strategy for hard tissue regeneration
